Global and Spatial Compartmental Interrelationships of Bone Density, Microstructure, Geometry and Biomechanics in the Distal Radius in a Colles' Fracture Study Using HR-pQCT.

BACKGROUND: Bone parameters derived from HR-pQCT have been investigated on a parameter-by-parameter basis for different clinical conditions. However, little is known regarding the interrelationships of bone parameters and the spatial distribution of these interrelationships. In this work: 1) we investigate compartmental interrelationships of bone parameters; 2) assess the spatial distribution of interrelationships of bone parameters; and 3) compare interrelationships of bone parameters between postmenopausal women with and without a recent Colles' fracture. METHODS: Images from the unaffected

Physeal fractures of the distal radius and ulna: long-term prognosis.

OBJECTIVES: The long-term prognosis of injuries to the distal physis of forearm bones, including complications such as radioulnar length discrepancy and styloid nonunion, has not been extensively studied. Reliable radiographic prognostic criteria to predict physeal disturbance at trauma are also lacking. The aim of this study is to investigate both issues. DESIGN: Retrospective study. SETTING: University hospital. PATIENTS/PARTICIPANTS: One hundred sixty-three lesions to the distal physis of the forearm bones in 157 patients were available for a long-term follow-up. Seventy-seven injuries o…

Occupational Dermatitis in a Paint Mixer: Impact of Symptom Neglect and Strategies for Early Intervention and Prevention.

A 34-year-old paint mixer presented with mild, localized dermatitis on the forearms, traced to low-level exposure to a paint additive. Immediate interventions, including improved personal protective equipment (PPE) and modified job tasks, led to rapid symptom resolution. This report highlights early identification, appropriate workplace controls, and follow-up measures that prevent long-term complications. The case underscores the importance of prompt, targeted interventions and an organizational commitment to worker health.

Extranodal Castleman disease of the extremities: a case report and review of the literature.

Castleman disease is a rare lymphoproliferative disorder of unknown etiology that most commonly presents as a mediastinal nodal mass or, in the extranodal form of the disease, a mass located in the mediastinum or retroperitoneum. It is exceptionally uncommon for Castleman disease to present in the extremities. We report a rare case of extranodal Castleman disease presenting as a muscular forearm mass. We compare our case with the seven other reported cases in which Castleman disease presented as an isolated soft tissue mass in the extremities.

Clostridium perfringens gas gangrene at a wrist intravenous line insertion.

A patient admitted to the intensive care unit for management of hypotension following a multiple medications overdose subsequently deteriorated rapidly with sepsis. A cannula site was noted to be bruised, swollen and erythematous and the X-ray demonstrated gas sitting within the tissues surrounding the metacarpal bones. The patient was referred to the orthopaedic surgeons and quickly taken for debridement of the affected area and fasciotomies of the forearm. Microbiological investigation confirmed Clostridium perfringens to be present in multiple fluid samples taken from the affected site.

Hirayama disease: a frequently undiagnosed condition with simple inexpensive treatment.

Hirayama disease is a non-progressive asymmetric juvenile muscular atrophy involving C7-T1 myotomes leading to weakness and atrophy of intrinsic muscles of hand and forearm with relative sparing of the brachioradialis muscle. This benign focal cervical poliopathy is believed to be caused by forward displacement of the posterior cervical dural sac. In this case report we highlight the importance of dynamic MRI of cervical spine for diagnosis of the above condition and the use of hard cervical collar to prevent further flexion injury.

Wristwatch-associated ski injury.

Upper limb injuries are a common occurrence in snowboarders, and these may be reduced by wearing wrist guards that transmit the force of the fall across the forearm. We present a case of a 26-year-old man who experienced a displaced left radial and ulnar fracture related to wearing a wristwatch while snowboarding. The wristwatch acted as a fulcrum around which the arm was fractured, serving as a stress riser that focused the force of the fall. The fracture was treated using open reduction and internal fixation.

Transient vascular insufficiency after axillary brachial plexus block in a child.

UNLABELLED: Axillary block is used in children for procedures on the hand and forearm. We report on a child with an amputation of the thumb in whom an axillary block was given, after which the limb became pale and pulseless. The pulses returned spontaneously in 15 min. The awareness of this possibility and chances of spontaneous recovery should be considered. IMPLICATIONS: Transient vascular insufficiency of the upper limb may happen as a rare complication after axillary block. Knowledge of this complication can help the anesthesiologist in the management of this problem.

Radial nerve compression by a large cephalic vein aneurysm: case report.

A 43-year-old man had weakness of the extensor muscles in the right forearm and could not extend the right wrist. The apparent disorder was caused by radial nerve compression by a large aneurysm in the cephalic vein in the antecubital fossa. Surgical resection of the aneurysm resolved the symptoms. This exceedingly rare complication of venous aneurysm is discussed. Venous aneurysm should be included in the differential diagnosis of a subcutaneous mass, and diagnosis is best made with Duplex ultrasound scanning. Surgical excision is the appropriate treatment.

Breast cancer in Poland syndrome.

A 33-year-old African-American woman with a severe manifestation of Poland syndrome developed breast cancer in the ipsilateral breast. She had a severely hypoplastic upper extremity, including symbrachydactyly, and a hypoplastic forearm and upper arm. In addition, she lacked the sternal origin of the pectoralis muscle. She had a very small nipple-areola complex and no axillary hair. This is the first case report of breast cancer developing in the ipsilateral breast of a patient with Poland syndrome.

[Method of plastic replacement of extended laryngopharyngeal and cervical esophagus defects ].

Filling of extended pharyngoesophagostomas were made in 13 patients operated for advanced laryngeal cancer by means of microsurgical autotransplantation by the forearm radial flap. The latter was created in the form of two cutaneous elements on common fascial base. After transfer of the autotransplant in the recipient's bed, one cutaneous element filled the defect of the laryngopharyngeal and esophageal inner coating, the second filled the defect of the outer coating. This method is reliable, effective, low traumatic, helps to avoid abdominal surgery.

Isolated traumatic denervation of the extensor pollicis longus.

This report describes a case of isolated denervation of the extensor pollicis longus (EPL), which has not previously been reported. The patient was a healthy 13-year-old boy who sustained multiple lacerations to the left forearm secondary to a dog bite. No motor responses were evident in the EPL, but the remainder of the exam findings were normal. Electromyography confirmed complete isolated denervation of the EPL. This patient was managed nonoperatively and had no significant functional deficits at final follow-up.

Incidence of a septum within the first dorsal compartment of the wrist.

A septum is not unusual within the first dorsal compartment of the wrist. The right and left forearms of 50 cadavers were dissected. A septum was identified 34% of the time. The septum separated the abductor pollicis longus tendon from the extensor pollicis brevis tendon. If the septum was present on one side, it was usually present on the other side. The septum created a separate compartment for the extensor pollicis brevis tendon. The effect of a separate compartment on the operative and nonoperative treatment of de Quervain's disease is discussed.

Chronic hypertrophy of the ulna in the professional rodeo cowboy.

Radiographs were taken of the forearm of 56 rodeo contestants who performed regularly in active competition. All of the professional bareback riders revealed a significant hypertrophy in the ulna. This hypertrophy is considered to be a chronic circumferential periostitis, secondary to contact of the subcutaneous ulna with the anterior iliac crest, and to the heavily resined chaps worn by the rodeo cowboy. There is no significant treatment with exception of padding, etc. The clinical complaints of "soreness" disappear with the cessation of the contact between the subcutaneous ulna and iliac cre…

[Compression osteosynthesis with soft-tissue damage (author's transl)].

The advisability of open reduction with internal fixation by means of compression plates, screws, or tension band is dependant upon the localization and type of fracture, the kind and extent of soft-tissue damage, and also the time factor. Fractures in the vicinity of joints and intra-articular fracture dislocations are practically absolute indications. Its application is less frequent when the metaphysis is involved, except in the case of the forearm. Caution is suggested in the presence of chronic soft-tissue damage subsequent to peripheral circulatory disorders.

Utilizing venous occlusion plethysmography to assess vascular effects: A study with buloxibutid, an angiotensin II type 2 receptor agonist.

Buloxibutid (also known as C21) is a potent and selective angiotensin II type 2 receptor (AT2R) agonist, in development for oral treatment of fibrotic lung disease. This phase I, open-label, pharmacodynamic study investigated vascular effects of buloxibutid in five healthy male volunteers. Subjects were administered intra-arterial infusions of buloxibutid for 5 min in ascending doses of 3, 10, 30, 100, and 200 μg/min, infused sequentially in the forearm. Infusions of sodium nitroprusside (SNP) solution in doses of 0.8-3.2 μg/min were administered as a positive control. Forearm blood flow (FBF)…

Neurofibroma with glomus-like bodies: A novel association-Thoughts about origin.

A neurofibroma with focal glomus-like body differentiation is an unusual phenomenon recently encountered in an excision specimen from the right lateral distal forearm of a 26-year-old man. Glomus cells are modified smooth muscle cells normally present in glomus-like bodies but can also be found in glomus tumors (GT) or lesions considered in the spectrum of GT, including myopericytoma, myofibroma, and angiolipoma. Neurofibromas are peripheral nerve sheath tumors derived from the neural crest cells. While both GT and its variants and neurofibroma are thought to be derived from different cell typ…

Reconstruction of Prehension in C7-T1 Brachial Plexus Injury with Nerve and Tendon Transfers: A Case Report.

CASE: Reconstruction of function in C7-T1 lower brachial plexus injury requires restoration of finger flexion and extension with nerve or tendon transfers. A 22-year-old man was involved in an all-terrain vehicle rollover accident. First-stage nerve transfers included extensor carpi radialis brevis to the anterior interosseous nerve and supinator to the posterior interosseous nerve. After second-stage (15 months later) side-to-side flexor digitorum profundus and Camitz abductorplasty, he was able to achieve tripod pinch and power grip. CONCLUSION: A combined approach with nerve transfers foll

In vivo pressure-flow relation of human cutaneous vessels following prolonged iterative exposures to hypergravity.

The study examined intra- and interlimb variations in cutaneous vessel responsiveness to acute and repeated transmural pressure elevations. In 11 healthy men, red blood cell flux was assessed via laser-Doppler flowmetry on both glabrous and nonglabrous skin regions of an arm (finger and forearm) and leg (toe and lower leg), across a wide range of stepwise increasing distending pressures imposed in the vessels of each limb separately. The pressure-flux cutaneous responses were evaluated before and after 5 wk of intermittent (40 min, 3 sessions per week) exposures to hypergravity (∼2.6-3.3 G; G …

Evaluation of the treatment of distal radial volar fracture by different methods sparing the pronator quadratus.

OBJECTIVE: The traditional volar approach requires the release of the pronator quadratus (PQ) muscle in the treatment of distal radius fractures. However, intraoperative repair of the PQ muscle often fails due to tissue injury and unstable muscle repair. This study compared the outcomes of different methods of sparing the PQ muscle combined with the volar plate in treating distal radius fractures. METHODS: A total of 95 patients with distal radius fractures sparing the PQ muscle were enrolled with the brachioradialis (BR) splitting approach (group A, 33 people), the volar plating insertion PQ
