Reducing global neonatal mortality is possible.

BACKGROUND: The global neonatal mortality of 28/1,000 live births is unacceptably high. Furthermore, the huge difference in mortality between high- and low-income countries and regions is presently one of the most burning human rights issues. The decline in neonatal mortality has been slow and is a main reason the Millennium Development Goal 4 does not seem to have been reached. OBJECTIVES: Several countries have shown it is possible to reduce neonatal mortality quickly and dramatically without much cost. METHODS: It is important to learn from the successful countries and focus on the 3-4 ma

[Epidemiologic survey for hospitalized neonates in China].

OBJECTIVE: To understand the disease spectrum and treatment outcome on hospitalized neonates in China, a nationwide epidemiologic survey was performed. This paper reports the investigation results. METHODS: A retrospective study of 43 289 hospitalized neonates from 86 hospitals in 47 Chinese cities (22 provinces) between January 1, 2005 and December 31, 2005 was performed. RESULTS: (1) The sex ratio of male to female was 1.73:1. (2) Premature infants accounted for 26.2% of the hospitalized neonates, which was higher than that reported in 2002 (19.7%). (3) The top three diseases that the neon

Clinical signs that predict severe illness in children under age 2 months: a multicentre study.

BACKGROUND: Neonatal illness, particularly in the first week of life, is a leading cause of death worldwide. Improving identification of young infants who require referral for severe illness is of major public-health importance. METHODS: Infants under 2 months of age brought with illness to health facilities in Bangladesh, Bolivia, Ghana, India, Pakistan, and South Africa were recruited in two age-groups: 0-6 days and 7-59 days. A trained health worker recorded 31 symptoms and clinical signs. An expert paediatrician assessed each case independently for severe illness that required hospital ad

Perinatal death associated with planned home birth in Australia: population based study.

OBJECTIVE: To assess the risk of perinatal death in planned home births in Australia. DESIGN: Comparison of data on planned home births during 1985-90, notified to Homebirth Australia, with national data on perinatal deaths and outcomes of home births internationally. RESULTS: 50 perinatal deaths occurred in 7002 planned home births in Australia during 1985-90: 7.1 per 1000 (95% confidence interval 5.2 to 9.1) according to Australian definitions and 6.4 per 1000 (4.6 to 8.3) according to World Health Organisation definitions. The perinatal death rate in infants weighing more than 2500 g was

A comparison of pre-discharge survival and morbidity in singleton and twin very low birth weight infants.

The perinatal mortality rate of twins is four to 11 times higher than that of singletons, and twins are widely reported to have more morbidity than singletons, mainly because of a higher preterm birth rate. However, it is not clear that live-born preterm birth rate. However, it is not clear that live-born preterm twins suffer greater morbidity than comparable singletons. In fact, twins have been reported to develop pulmonary maturity earlier than singletons, which might result in decreased morbidity relative to comparable preterm singletons. We conducted this retrospective review of 496 consec

Eliminating Risk of Intubation in Very Preterm Infants with Noninvasive Cardiorespiratory Support in the Delivery Room and Neonatal Intensive Care Unit.

INTRODUCTION: Avoiding intubation and promoting noninvasive modes of ventilator support including continuous positive airway pressure (CPAP) in preterm infants minimizes lung injury and optimizes neonatal outcomes. Discharge home on oxygen is an expensive morbidity in very preterm infants (VPI) with lung disease. In 2007 a standardized bundle was introduced for VPI admitted to the neonatal care unit (NICU) which included delayed cord clamping (DCC) at birth and noninvasive ventilation as first-line cardiorespiratory support in the delivery room (DR), followed by bubble CPAP upon NICU admission

Gastroschisis: a plea for risk categorization.

BACKGROUND: The incidence of gastroschisis has increased in the past decade. A differing clinical course between "complex" (those with atresias, perforation, or stenosis) and "simple" cases has prompted a review of risk assessment factors. METHODS: A retrospective chart review was conducted of 103 infants with gastroschisis over 5 years (1992 to 1997). RESULTS: Of 103 infants, 52 were girls and 51 were boys. Seventy-one infants (69%) had a simple defect, and 32 (31%) were complex. The simple group had an average estimated gestational age of 37.5 weeks (range, 26 to 40), and a birth weight of

Trends and Causes of Neonatal Mortality in Canadian Neonatal Intensive Care Units.

OBJECTIVE: To determine patterns and causes of neonatal deaths in Canadian neonatal intensive care units (NICUs) over the last decade. STUDY DESIGN: All neonatal deaths reported by NICUs participating in the Canadian Neonatal Network 2010-2022 were divided retrospectively into 2 groups by gestational age (GA), <33 weeks and ≥33 weeks. Cause of death was assigned using prespecified classification criteria adapted from published criteria. Demographic, perinatal, and neonatal characteristics of newborns who died were compared with those of the survivors. Trends in birth characteristics and cause

Impact of reduced institutional delivery coverage on neonatal survival during the peak of coronavirus disease 2019 pandemic in Nepal: Estimates using Lives Saved Tool model.

BACKGROUND: An alarming observation from high-volume obstetric facilities in Nepal indicating a decreased institutional delivery rate and increased institutional neonatal mortality rate after the initial nationwide lockdown signaled the adverse population-level impact of the pandemic on the national trajectory of neonatal survival. OBJECTIVES: We aimed to estimate the impact of change in institutional delivery coverage on cause-specific neonatal mortality during the coronavirus disease 2019 pandemic in Nepal. DESIGN: Modeling-based study. METHODS: We used the open-access Lives Saved Tool, b

Determinants of neonatal mortality at Kamenge Teaching Hospital, Burundi: a prospective cohort study.

BACKGROUND: Despite the implementation of a free healthcare policy for pregnant women and children under five since 2006, neonatal mortality remains high in Burundi. Indeed, twenty-one neonates per 1,000 live births died in 2019. This study aimed to assess neonatal survival and identify factors associated with neonatal mortality in one tertiary hospital of Bujumbura, Burundi. METHODS: We recruited 885 babies whose births occurred between October and December 2020 in Kamenge Teaching Hospital of the University of Burundi and followed them over four months. We applied life table methods and imp

Enhancing the Classification of Congenital Heart Defects for Outcome Association Studies in Birth Defects Registries.

INTRODUCTION: Traditional strategies for grouping congenital heart defects (CHDs) using birth defect registry data do not adequately address differences in expected clinical consequences between different combinations of CHDs. We report a lesion-specific classification system for birth defect registry-based outcome studies. METHODS: For Core Cardiac Lesion Outcome Classifications (C-CLOC) groups, common CHDs expected to have reasonable clinical homogeneity were defined. Criteria based on combinations of Centers for Disease and Control-modified British Pediatric Association (BPA) codes were de

Systematic review of Apgar scores & cyanosis in Black, Asian, and ethnic minority infants.

BACKGROUND: Apgar score and cyanosis assessment may disadvantage darker-skinned babies. This review explored cyanosis and Apgar score assessments in Black, Asian, or minority ethnic neonates compared to White neonates. MATERIAL AND METHODS: Four databases were searched. Studies of any methodology were included. A narrative synthesis was undertaken. RESULTS: Ten studies were included. Three studies involving over 39 million neonates showed Apgar score ≤3 was predictive of neonatal mortality across all ethnicities. Black babies with Apgar score ≤3 had lower mortality rates before 28 days, howe

Effect of community-initiated kangaroo mother care on breastfeeding performance in low birthweight infants: A randomized clinical trial.

This individually randomized trial was conducted to estimate the effect of promoting community-initiated kangaroo mother care (ciKMC) in low birthweight (LBW) infants on infant breastfeeding performance. It was designed as a substudy within a larger primary trial on ciKMC and infant survival. Five hundred fifty stable LBW mother-infant dyads (1500-2250 g) who provided consent, were consecutively enroled for breastfeeding performance assessment. The ciKMC intervention included promotion and support of continuous skin-to-skin contact and exclusive breastfeeding (EBF) through home visits during t
