OBJECTIVE: To determine the end result of patients who underwent delayed fasciotomy, i.e., more than 35 hours for an established lower limb compartment syndrome.
DESIGN: A retrospective review of patients undergoing delayed treatment for a closed injury of the lower extremity, where fasciotomy should ideally have been performed earlier.
MATERIALS AND METHODS: Nine fasciotomies in five patients were identified where there was a delay of more than 35 hours after the injury. The average ischemic time was 56 hours (range 35-96 hours).

A case of unique acute bone marrow toxicity and pancytopenia following subcutaneous exposure to lead chromate, xylene, and ethylbenzene in a previously healthy patient is reported. The patient sustained an extensive degloving injury to his lower extremity. The wound was contaminated with traffic paint containing lead chromate pigment along with a large volume of xylene and ethylbenzene solvent. Consequences of the patient's clinical course and management of degloving injuries are discussed.

To determine the frequency and type of arterial injuries associated with complete dislocation of the knee and to correlate the findings at physical examination with those at arteriography, a computerized search for patients with a diagnosis of dislocation of the knee at discharge from an urban trauma hospital was performed. Functional limb salvage requires motor and sensory reconstruction of the extremity. An avulsion of the tibial nerve in the popliteal fossa after arthroscopy is presented. Functional limb recovery necessitated intraoperative nerve mapping by awake stimulation, neurotization of the gastrocnemius muscle bellies, and interfascicular posterior tibial nerve grafting. Eighteen-month follow-up demonstrated good gastrocnemius motor function and protective sensation on the plantar surface. The patient is ambulatory without the need of an assistive device.

Previous studies of the neurocontrol of movement in spinal cord injury (SCI) subjects revealed that even those without volitional movement may retain some degree of preservation of distal brain influence. Traumatic arteriovenous fistulas are very common, especially with penetrating injuries. Undiagnosed traumatic arteriovenous fistulas for long periods, as in this case, however, are less frequent because the patient often experiences clinical manifestations associated with arteriovenous shunting. This patient denied any history of problems that might be associated with this arteriovenous shunting. He was operated on for the arteriovenous fistula and had an uneventful postoperative course.

Five hundred questionnaires were distributed to long-distance runners who had used, or who were using orthotic shoe inserts for symptomatic relief of lower extremity complaints. The causes of chronic hematogenic and posttraumatic forms of osteomyelitis of the long tubular bones in 383 patients are analysed. Delayed diagnosis and irrational surgical treatment was the main cause of the development of chronic hematogenic osteomyelitis in 97 patients. One hundred five military recruits were evaluated in our outpatient clinic for pain in the lower limb following prolonged sustained physical effort and strain. Plain x-ray investigations augmented by technetium-99m polyphosphate demonstrated the presence of 80 stress fractures in 52.4% of the patients. Although few trauma patients sustain fatal pulmonary embolism, a large population is at risk from nonfatal embolism due to unrecognized deep vein thrombosis (DVT). One-third of the adolescent patients followed up in our prosthetic clinic have developed patellar dislocation. Four cases are described, and radiographs are evaluated for femoral-tibial alignment, patellar height, and tibial length. Patella alta was present in each patient. We propose that the forces associated with the patellar tendon-bearing prosthesis may produce elongation of the patellar tendon in young patients and predispose them to patellar instability.

Motor responses to standardized stimuli were evaluated in 18 comatose patients with abnormal motor reactions. Painful stimuli were applied to three areas: the supraorbital, sternal, and medial aspects of the arm. Stimulations were carried out with the upper limbs first flexed and then extended. Patients with knee and hip joint diseases or fractures in a limb with a distal portion amputated were successfully treated with total joint arthroplasties. Two patients with below-knee amputations recieved total joint replacements. One underwent a total hip arthroplasty and the other had a total knee arthryplasty on the side of the amputation. Physical therapy was modified minimally for the two patients in comparison to other patients with total joint replacements. Prosthetic changes were required in both cases. With new prostheses, both patients were able to return to work.
