Acute pyelonephritis in adults: prediction of mortality and failure of treatment.

BACKGROUND: To formulate a classification tool for early recognition of patients admitted with acute pyelonephritis (AP) who are at high risk for failure of treatment or for death. METHODS: A retrospective chart review of 225 patients (102 men) admitted with AP. We considered 13 potential risk factors in a multivariate analysis. RESULTS: Recent hospitalization, previous use of antibiotics, and immunosuppression were found to be independent correlates of the prevalence of resistant pathogens in both sexes. Additional predictors included nephrolithiasis in women and a history of recurrent AP i

Derivation and identification of questions that act as predictors of abdominal pain of musculoskeletal origin.

OBJECTIVE: Although most causes of abdominal pain have a visceral origin, the musculoskeletal system must be considered when the cause is not obvious. This prospective study aimed to identify questions that would aid the diagnosis of patients with abdominal pain of musculoskeletal origin. DESIGN: Assessment of consecutive patients with abdominal pain recruited from gastroenterological outpatient clinics to develop diagnostic pointers to identify abdominal pain arising from musculoskeletal disorders. PARTICIPANTS: Subjects with benign abdominal pain, with or without a change in bowel habit, w

Evidence of autoantibodies to cell membrane associated DNA (cultured lymphocytes): a new specific marker for rapid identification of systemic lupus erythematosus.

OBJECTIVE: Autoantibodies to cell membrane associated DNA are described in systemic lupus erythematosus (SLE). The specificity of these antibodies differ from antibodies to nuclear DNA. METHODS: Using indirect immunofluorescence, a specific IgG was detected giving a characteristic pattern of continuous peripheral membrane fluorescence on cultured B-lymphocytes. RESULTS: This pattern was observed in 53 of 80 serum samples of SLE patients but absent in the serum samples of the control populations: 15 rheumatoid arthritis, 38 ankylosing spondylarthritis, 17 non-inflammatory osteopenic patients,

Decoding cytokine networks in ulcerative colitis to identify pathogenic mechanisms and therapeutic targets.

Ulcerative colitis (UC) is a chronic inflammatory disorder of the gastrointestinal tract that is characterized by dysregulated cytokine signaling. Treatment outcomes for patients with UC remain suboptimal despite the advent of cytokine-targeting therapies, necessitating a better understanding of the interconnected cytokine signaling networks that are perturbed in patients with UC. To address this, we undertook systems immunology modeling of single-cell transcriptomics data from colonic biopsies of treatment-naive and treatment-exposed patients with UC to build complex cytokine signaling networ

The multidimensional needs of chronic heart failure patients and caregivers from a dyadic perspective: a scoping review.

Despite advances in medical and device management, people with chronic heart failure continue to experience persistent physical and non-physical symptoms. Consequently, heart failure guidelines advocate for the early integration of palliative care. Nevertheless, the multidimensional needs of patients and their informal caregivers remain insufficiently examined. This scoping review aimed to explore the physical and non-physical needs of adults living with chronic heart failure, along with those of their informal caregivers, across Europe. Following Arksey and O’Malley’s framework, a systematic

Misdiagnosis of diabetic ketoacidosis: a case report.

BACKGROUND: Abdominal pain caused by diabetic ketoacidosis is uncommon and can easily be misdiagnosed as abdominal pain caused by ureteral stones. This case highlights the importance of distinguishing between these etiologies, particularly in patients with diabetes, as delayed recognition of diabetic ketoacidosis can result in life-threatening complications. The novelty lies in emphasizing the diagnostic challenges and the necessity for clinicians to consider diabetic ketoacidosis even when imaging suggests alternative causes. CASE PRESENTATION: This case report describes a 59-year-old Han fem

Role of CT Coronary Angiography at Initial Presentation in Kawasaki Disease-Insights from a Tertiary Care Center in North India.

Background: Kawasaki disease (KD) is a systemic vasculitis and the leading cause of acquired heart disease in children. Early identification of coronary artery abnormalities (CAAs) is crucial to guide treatment and improve outcomes. While transthoracic 2D echocardiography (TTE) remains the first-line imaging modality, it has limitations, particularly in visualizing distal coronary artery segments and detecting thrombi. Computed tomography coronary angiography (CTCA) offers enhanced visualization, but its role at initial presentation of KD remains underexplored. Methods: We reviewed the records

Respiratory muscle dysfunction in older critically ill patients: Implications for rehabilitation.

Older patients represent a rapidly expanding and clinically vulnerable population within intensive care units (ICUs), often presenting with complex, multifactorial health challenges. The physiological changes associated with aging, when compounded by critical illness, result in significant dysfunction of both peripheral and respiratory musculature. These alterations are further aggravated by clinical frailty, a multidimensional syndrome now recognized as a key determinant of adverse outcomes in critically ill patients. Respiratory muscle dysfunction in this population contributes to prolonged

Association of pleural effusion on thoracic ultrasound with severe acute pancreatitis and mortality.

BACKGROUND: Acute pancreatitis (AP) is an inflammatory condition associated with significant morbidity and mortality that often leads to thoracic complications. Timely identification of these complications, including pleural effusion (PE), is of utmost importance because it may indicate AP severity. METHODS: This prospective cohort aimed to determine the association between the presence of PE and other lung findings identified by thoracic ultrasound (TUS) and AP severity. We included patients diagnosed with AP according to the Atlanta classification. TUS was performed within the first 48 h of

Interdisciplinary recommendations for recurrent hyperkalaemia: insights from the GUARDIAN-HK European Steering Committee.

Recurrent hyperkalaemia (HK) is associated with increased morbidity and mortality, and is common among patients with cardiorenal disease. Many of these patients require renin-angiotensin-aldosterone system inhibitor (RAASi) therapies that further enhance the risk of HK. Every acute HK episode constitutes an opportunity to treat and prevent recurrent HK. This report aims to support multidisciplinary team efforts in managing patients who may be affected by recurrent HK. A panel of nine European experts in the management of HK (four nephrologists, four cardiologists, one internist) reviewed exist

A New Easy-to-Perform Flow Cytometry Assay for Determining Bacterial- and Viral-Infection-Induced Polymorphonuclear Neutrophil and Monocyte Membrane Marker Modulation in Febrile Patients.

We developed a flow cytometry (FC) assay enabling the rapid and accurate identification of bacterial and viral infections using whole blood samples. The streamlined flow cytometry assay is designed to be user-friendly, making it accessible even for operators with limited experience in FC techniques. The key components of the assay focus on the expression levels of specific surface markers-CD64 on polymorphonuclear neutrophils (PMN) as a marker for bacterial infection, and CD169 on monocytes (MO) for viral infection. The strong performance indicated by an area under the receiver operating chara

The Predicted Outcome of Spinal Cord Stimulation in Patients With a Psychopathological Disorder and Persistent Spinal Pain Syndrome Type 2: A Systematic Review From 2009 to 2021.

OBJECTIVES: Psychologic screening is often included as a mandatory component of evaluation of the impact of psychopathology disorders on the predicted outcome of spinal cord stimulation (SCS) for patients with chronic pain due to persistent spinal pain syndrome type 2 (PSPS type 2). The conclusion of such screenings can influence the decision to offer SCS therapy to a patient. However, evidence on the impact of psychopathology on SCS outcomes is still scarce. MATERIALS AND METHODS: To address this knowledge gap, we systematically reviewed the literature from 2009 to 2021 to explore the correl

Increased SOCS1 and SOCS3 expression in papillary thyroid carcinoma and its association with prognosis.

OBJECTIVE: The majority of patients with papillary thyroid carcinoma (PTC) have good outcomes, although the identification of new predictors of a poor prognosis would be beneficial. Chronic thyroiditis is a precancerous condition in which proinflammatory cytokines enhance biologically aggressive features. This study investigated the expression of suppressor of cytokine signaling proteins (SOCS) 1 and 3, which are negative feedback inhibitors, in PTC and benign thyroid nodules (BTN), and analyzed the relations among biomarker expression, pathological prognosis, and clinical features.
