Women have always been part of the HIV/AIDS epidemic. As with other populations affected by HIV, for many years the only prevention strategy available was behavior change. Behavioral interventions for women were developed and evaluated, with some success. Because women did not control the use of male condoms, efficacious interventions needed to build skills for partner negotiation. Woman-controlled technologies such as the female condom and vaginal spermicide were unable to solve the problem of male control of the condom and enable the development of safe methods for women to protect themselve

Breast deformity, in post-burn patients, is a common problem leading to lower self-esteem and reclusive behavior that impairs quality of life. The authors present the course of treatment of an 18-year-old immigrant girl who suffered second- to third-degree burns over approximately 20% of her total body surface area in her early childhood. The second- to third-degree burns were located on her right trunk and abdomen, as well as her right shoulder, neck, and right groin area. Since it was not offered in her home country, reconstructive surgery, including microsurgical breast reconstruction, was

Obesity is a world-wide problem, and a risk factor for cardiovascular disease, diabetes, cancer and other diseases. It is well established that sex differences influence fat storage. Males and females exhibit differences in anatomical fat distribution, utilization of fat stores, levels of adipose tissue-derived hormones, and obesity co-morbidities. The basis for these sex differences may be parsed into the effects of male vs. female gonadal hormones and the effects of XX vs. XY chromosome complement. Studies employing mouse models that allow the distinction of gonadal from chromosomal effects

A "risk of harm" protocol to identify youth in need of immediate emergency assistance in a study on mental health and HIV in Rwanda among 680 youth ages 10-17 is described. Cases are presented that describe the experience in using this protocol to ensure safety of participants, with ethical and logistical challenges considered. Among the population of the study, 3.2% were deemed "risk of harm." The most prevalent presenting problem was non-fatal suicidal behavior (91% of risk of harm cases), with 36% having a history of a reported previous attempt. Challenges included: acute food insecurity/si

Incidental splenic lesions are frequently encountered at imaging performed for unrelated causes. Splenic cysts, hemangiomas, and lymphomatous involvement are the most frequently encountered entities. Computed tomography and sonography are commonly used for initial evaluation with magnetic resonance imaging reserved as a useful problem-solving tool for characterizing atypical and uncommon lesions. The value of magnetic resonance imaging lies in classifying these lesions as either benign or malignant by virtue of their signal-intensity characteristics on T1- and T2-weighted imaging and optimal d

Pathological gambling (PG) has been identified in Parkinson's disease (PD), but such gambling behaviors may also occur in amyotrophic lateral sclerosis (ALS). We sought to estimate the prevalence of PG amongst members of a web-based community, PatientsLikeMe.com. A survey was constructed, consisting of demographic information, the South Oaks Gambling Screen (SOGS), the K-6 measure of distress, and items related to motivation for gambling. Data were obtained from 236 ALS patients and 208 PD patients. Of the PD patients, 13% were classified as problem gamblers compared with 3% of ALS patients (c

Since the 1970s, many studies have enhanced a social scientific understanding of the lethal and non-lethal physical abuse of women during and after separation and divorce, but less than a handful have examined sexual assaults on rural women who want to leave, are trying to leave, or who have left spouses or live-in male partners. Further, none of the work done so far on this problem has examined the role of collective efficacy. The results presented here help fill these research gaps and call into question the common assumption that there is more collective control on criminal behavior in rura

Errorless compliance training is a noncoercive, success-focused approach to treatment of problem behavior in children. The intervention involves graduated exposure of a child to increasingly more challenging requests at a slow enough rate to ensure that noncompliance rarely occurs, providing parents with many opportunities to reinforce cooperative responses and rendering punishment unnecessary. The authors evaluated this approach with three boys with characteristics of Asperger syndrome. Mothers first delivered a range of requests to their children and recorded child responses. For each child,…

Marijuana is the most frequently used illicit drug in the United States. The health risks associated with its use are underestimated by adolescents, their parents, and health care providers. This article identifies current trends regarding marijuana use among adolescents and provides a summary of current information about the risks of marijuana use for both users and society at large. It also urges physicians and other health care providers to be aware of the dangers of marijuana as well as the subtle and seemingly unexplained changes in adolescent behavior belying its use. In addition, it cal

The present study merged problem behavior and social ecological theories to examine how mental health and environmental factors, including culture, were associated with American Indian youth tobacco use. A stratified random sample of 205 reservation and 196 urban American Indian adolescents living in a Southwestern area was interviewed in 2001. Two-thirds of the reservation youth and half of the urban youth in this sample reported lifetime tobacco use. Logistic regression showed that, when controlling for age and location, a mental health factor (substance abuse/dependence) and environmental f…

Eating disorders, i.e. anorexia and bulimia nervosa, are disorders of eating behavior and body weight regulation. Most likely because there are few, if any, effective treatments, eating disorders are considered to be chronic disorders interrupted only by intermittent periods of short-lived remission. The neurobiology of eating, most of which explores hypothalamic mechanisms, has had no influence on the treatment of eating disorders, with the exception of psychopharmacology. However, while most patients are treated with psychoactive drugs, there is no evidence that these are effective. This may

A comparison of empirical measures and perceived gambling severity among youth was conducted. Participants (N=980), mean age of 18.6 years, completed several widely accepted measures of pathological gambling [Diagnostic and Statistical Manual of Mental Disorders-IV-Juveniles (DSM-IV-J), South Oaks Gambling Screen-Revised for Adolescents (SOGS-RA), and Gambler's Anonymous 20 Questions (GA 20)] and a questionnaire assessing gambling behavior. Findings revealed that while the DSM-IV-J, SOGS-RA, and GA 20 identified between 3.4% and 5.8% of participants as probable pathological gamblers, only 1.1%

Physicians and other mental health professionals in our state should continue, based on Tennessee's child abuse statutes, to report possible abuse and neglect when indicated even if the behavior is considered culturally normal. In addition, professionals attempting to serve diverse populations must incorporate an understanding of culture, traditions, religious beliefs and other information specific to family interactions. They should include family history of mental illness into the design of their individual treatment plans. Providers should form working partnerships with communities in order

We examined the effect of the apolipoprotein E (apo E) epsilon4 allele on the relationship between self-reported stress and mood in caregivers of patients with Alzheimer's disease. Eighty-six female subjects between the ages of 28 and 82 years who were community-dwelling AD patient caregivers participated in the study. A cross-sectional analysis of stress and mood was performed using the Revised Memory and Behavior Problem Checklist and the Geriatric Depression Scale. All subjects were evaluated for normal cognitive function (Mini-Mental Status Examination) and apo E genotype. The results indi

The Problem Oriented Screening Instrument for Teenagers (POSIT) and the Beck Depression Inventory (BDI) were administered to 55 disadvantaged adolescent mothers who abused drugs during pregnancy and 49 nondrug-abusing disadvantaged adolescent mothers. Results suggested that the drug-abusing mothers were depressed (BDI = 14) while the nondrug-abusing mothers were nondepressed (BDI = 6). In addition, the drug-abusing mothers reported more mental and physical health problems, more problematic family and peer relationships, poorer social skills, more aggressive behavior, less constructive use of l

As rape becomes recognized as a public health issue, new paradigms must be constructed to discover viable solutions to this highly prevalent problem. Although the injury prevention field has begun to examine rape and offer solutions, physical injuries surrounding rape tend to become the focus in injury prevention literature, thereby minimizing the trauma of rape itself. This article applies William Haddon's ten general strategies for injury prevention to rape, in order to shift our focus away from women and their behavior onto the systemic causes of rape. These strategies have the advantage of

"Pendelluft", or out-of-phase movement of the airway gas between the intact and flait-chest-side lungs has long been believed to be the major contributor to respiratory dysfunction in patients with flail chest. However, conflicting findings have also been reported mainly from animal studies. The aim of this study was to provide a mathematical projection on this classical problem. We measured respiratory impedance (ZRS) of dogs with flail chest using a pseudorandom forced oscillation method. A mathematical model implementing flail chest was fitted toZRS. The fitted results were used in simulati

Although there are many treatment alternative open to people with drinking problems, health professionals still exhibit negative attitudes towards alcoholics. In a previous study, the author demonstrated that those patients who were self-labelled alcoholics were treated in a less preferential manner than those who did not identify as such. This study used both overt and unobtrusive measures to determine whether negative attitudes of intake interviewers towards problem drinkers were elicited by the patients' self-label as an alcoholic or by other variables related to perceived treatment outcome

Recent developments in alcoholism : an official publication of the American Medical Society on Alcoholism, the Research Society on Alcoholism, and the National Council on Alcoholism1984eng
Thus far, we know relatively little about alcoholism among the elderly across cultures. However, extrapolation from available reports suggests certain common features across cultures. These include an inverse relationship between age and problem-producing dosages, increased risk of elderly alcoholics to trauma, greater efficacy of milieu changes with age, and a concentration of elderly alcoholics in cities. Conversely, certain other aspects of aging that vary across cultures may influence alcoholism in the elderly. These include the biological concomitants of aging, social status and the role
