Arthroscopic treatment of acromioclavicular joint injuries and results.

Injuries and conditions that affect the AC joint are common. Low-grade separations, degenerative conditions, and osteolysis of the distal clavicle are frequently dealt with by the treating physician. Proper assessment requires a thorough history, examination, and radiologic work-up. An injection of bupivicaine into the AC joint can be a very useful test to evaluate the source of pain about the symptomatic shoulder. Most conditions affecting the AC joint can be treated conservatively, but patients who do not respond to these treatments or athletes who do not wish to modify their activities may

AI-Assisted MRI Interpretation in Diagnosing Bankart and Reverse Bankart Lesions.

BACKGROUND: Accurate detection of glenoid labral tears remains challenging; however, it is essential for guiding treatment and return-to-play decisions in athletes. Magnetic resonance arthrography (MRA) improves diagnostic performance over noncontrast magnetic resonance imaging (MRI) but is invasive and more costly. Recent studies have shown that artificial intelligence (AI) models can accurately detect Bankart lesions on MRI and MRA, but whether AI assistance can improve clinician interpretation remains unknown. PURPOSE: To evaluate whether AI assistance improves clinician sensitivity and di

Effects of Adding Muscle Energy Technique to Joint Mobilization on Pain, Range of Motion, and Shoulder Function After Arthroscopic Rotator Cuff Repair: An Observational Cohort Study.

BACKGROUND Although muscle energy technique (MET) and joint mobilization (JM) are individually used in shoulder rehabilitation, evidence for their combined application following arthroscopic rotator cuff repair (ARCR) remains limited. This study evaluated whether adding MET to JM yields superior outcomes compared with JM in patients after ARCR. MATERIAL AND METHODS Forty patients who underwent ARCR were enrolled in this observational cohort study and assigned to either a control group (n=20) receiving JM or an experimental group (n=20) receiving MET in addition to the same JM. Shoulder functio

Comparison of ultrasound and MRI shows equivalent accuracy and reliability in acromial index measurement.

The acromial index, a radiological parameter assessing the subacromial space, is vital for diagnosing conditions like rotator cuff tears and subacromial impingement syndrome as the acromial Index influences the choice of therapy. Traditionally determined via X-ray, AI is increasingly assessed using MRI due to its superior image quality. However, MRI's cost and limited availability highlight the need for alternative methods. Ultrasound offers a promising, cost-effective alternative, providing real-time imaging without radiation exposure. This study aims to compare the accuracy and reliability o

Reverse shoulder arthroplasty for massive rotator cuff tears without glenohumeral arthritis can improve clinical outcomes despite history of prior rotator cuff repair: A systematic review.

IMPORTANCE: Reverse shoulder arthroplasty (RSA) is often used to surgically address massive irreparable rotator cuff tears (MIRCT) without arthritis. The impact of prior attempted rotator cuff repair (RCR) on outcomes is unclear. OBJECTIVE: The purpose of this systematic review was to compare functional outcomes, range of motion, and complications in patients with a MIRCT without arthritis who underwent RSA as a primary procedure versus after prior RCR. EVIDENCE REVIEW: A systematic review was performed on RSA for a MIRCT. The search was conducted from February to March of 2022 using the MED

Similar outcomes in collision athletes with subcritical glenoid bone loss and on-Track Hill Sachs lesion versus off-track Hill Sachs lesion managed with open Bankart repair plus inferior capsular shift.

INTRODUCTION: Open Bankart repair plus inferior capsular shift has not yet been tested under the concept of glenoid track as a predictor of failure. The aim of this study was to compare the subjective and objective outcomes in collision athletes with subcritical glenoid bone loss and on-track Hill Sachs lesions versus those with off-track Hill Sachs lesions, all treated with open Bankart repair. METHODS: Two study groups were created: 50 patients had on-track Hill Sachs lesions, while 38 had off-track lesions. The subcritical glenoid bone loss was ≤ 10%. A minimum follow-up period of 3 years

Pathological formation of subcoracoid bursa effusion on magnetic resonance imaging studies.

BACKGROUND: The subcoracoid space includes the subcoracoid bursa (SCB) and subscapular bursa (SSB). We aimed to clarify the relationship between the presence or amount of SCB effusion and other structures around the SCB on magnetic resonance imaging (MRI) and to discuss the pathological formation of SCB effusion. METHODS: Medical records and MR images of patients who had undergone MRI examinations for shoulder pain were retrospectively reviewed. The presence or absence of SCB, SSB, and subacromial-subdeltoid bursa (SASDB) effusion was evaluated. If SCB effusion was present, the largest diamet

Risk factors of acromial and scapular spine stress fractures differ by indication: a study by the ASES Complications of Reverse Shoulder Arthroplasty Multicenter Research Group.

BACKGROUND: Both patient and implant related variables have been implicated in the incidence of acromial (ASF) and scapular spine fractures (SSF) following reverse shoulder arthroplasty (RSA); however, previous studies have not characterized nor differentiated risk profiles for varying indications including primary glenohumeral arthritis with intact rotator cuff (GHOA), rotator cuff arthropathy (CTA), and massive irreparable rotator cuff tear (MCT). The purpose of this study was to determine patient factors predictive of cumulative ASF/SSF risk for varying preoperative diagnosis and rotator cu

Metallosis-Induced Conversion Shoulder Arthroplasty: A Unique Experience and Literature Review.

BACKGROUND: Total shoulder arthroplasty (TSA) can fail for several reasons, such as component loosening, periprosthetic fracture, instability, infection, soft tissue failure, or joint overstuffing. Severe metallosis without loose glenoid components after TSA may result in the need for revision to reverse TSA. CASE PRESENTATION: Four years before the current presentation, an 86-year-old woman suffered from right shoulder pain and swelling. The initial diagnosis was osteoarthritis of the shoulder joint, for which she underwent TSA. Four years later, she complained of shoulder joint pain, swelli

Biomechanical comparison of combined latissimus dorsi and teres major tendon transfer vs. latissimus dorsi tendon transfer in shoulders with irreparable anterosuperior rotator cuff tears.

BACKGROUND: Irreparable anterosuperior rotator cuff tears (IASRCTs) can result in a gradual loss of active elevation and internal rotation, superior and anterior translation of the humeral head, and cuff tear arthropathy. Joint-preserving treatment options for IASRCTs in young and high-demand elderly patients remain a subject of ongoing debate. The aim of the study was to evaluate the biomechanical efficacy of the combined latissimus dorsi and teres major tendon (LDTM) transfer and compare it to an isolated latissimus dorsi (LD) transfer in a cadaveric IASRCT model. METHODS: Eight cadaveric s

Group format rehabilitation is equally effective as individual therapy in patients with surgically repaired rotator cuff tears.

OBJECTIVE: Does group physiotherapy methods produce different results when compared with individual physiotherapy in persons with surgically repaired rotator cuff tears? No studies to date have examined the feasibility or effectiveness of such therapy formats. Given the prevalence of rotator cuff tears, a study examining group format therapy is warranted and can produce evidence to transform care for rotator cuff tear rehabilitation. METHODS: We analyzed the electronic medical records of persons that underwent rotator cuff repair surgery followed by the prescribedpost-surgical physiotherapy a

[Tendinopathies of the shoulder and elbow].

BACKGROUND: Tendinopathies of the shoulder and elbow joint are a common problem. According to the current state of knowledge tendinopathies can be separated into acute and chronic tendinitis as well as degenerative tendinosis. ORIGIN: The causes of tendinopathy can be intrinsic, extrinsic or a combination of both. A false straining or overuse with repetitive microtrauma is often the cause. Particularly affected are tendons of the rotator cuff, the long biceps tendons and lower arm extensors. TREATMENT: Priority is given to conservative appproaches for these disease processes. Following appro

Lateral femoral cutaneous nerve palsy following shoulder surgery in the beach chair position: a report of 4 cases.

Neuropathy of the lateral femoral cutaneous nerve can present as pain, decreased sensation, and/or burning or tingling on the anterolateral thigh. We present 4 cases of lateral femoral cutaneous nerve palsy following shoulder surgery in the beach chair position, all of which occurred in obese patients. This complication, to our knowledge, has never been reported in conjunction with the beach chair position. We believe that the neurapraxia was due to external compression by the patients' abdominal pannus. Full resolution of symptoms can be expected within 6 months following conservative managem

Clinics in diagnostic imaging (151). Acromioclavicular joint geyser sign with chronic full-thickness supraspinatus tendon (SST) tear.

An 82-year-old man presented with neck pain, right upper limb radiculopathy and right shoulder pain. Physical examination revealed a soft lump over the right shoulder joint, as well as reduced range of shoulder movements. On magnetic resonance imaging, the soft lump was shown to be a cystic mass over the acromioclavicular joint and was related to a full-thickness supraspinatus tendon tear. This is the classic geyser sign. The pathophysiology and clinical features of the geyser sign, and its imaging features with various imaging modalities, are discussed.
