[Natural orifice trans-luminal endoscopic surgery (notes)--a new era in general surgery].

Four years ago, a new surgical technique was presented, the natural orifice trans-luminal endoscopic surgery (NOTES). This technique provides an incisionless operation. The surgical devices are inserted into the peritoneal cavity through the gastrointestinal or the urogenital tracts. Today, a cholecystectomy can be performed using an advanced endoscope inserted through the stomach or the vagina. The advantages of NOTES are: reduced post operative pain, no hernias, no surgical wounds infections and better cosmetic results. The disadvantages are: difficulties in achieving safe enterotomy closure

Visual hallucinations and pontine demyelination in a child: possible REM dissociation?

An 11 year-old-boy acutely developed complex visual and acoustic hallucinations. Hallucinations, consisting of visions of a threatening, evil character of the Harry Potter saga, persisted for 3 days. Neurological and psychiatric examinations were normal. Ictal EEG was negative. MRI documented 3 small areas of hyperintense signal in the brainstem, along the paramedian and lateral portions of pontine tegmentum, one of which showed post-contrast enhancement. These lesions were likely of inflammatory origin, and treatment with immunoglobulins was started. Polysomnography was normal, multiple sleep

Ophthalmic artery microembolism in giant cell arteritis.

A 70-year-old man presented with a history of headache and sudden loss of vision of the left eye. Funduscopic examination showed sector retinal edema and hemorrhage as well as optic disc swelling consistent with anterior ischemic optic neuropathy. The Westergren sedimentation rate was 66 mm/h. Temporal artery biopsy was consistent with giant cell arteritis. Routine transcranial Doppler testing performed on a Pioneer 2020 instrument (Nicolet Vascular, Inc., Golden, CO) equipped with special software for microembolus detection showed a microembolic signal in the left ophthalmic artery. During a

Clinical profile of herpes zoster ophthalmicus in Ethiopians.

We conducted a prospective study of 100 consecutive Ethiopian patients with herpes zoster ophthalmicus (HZO); this study revealed a high incidence of HZO among the young (mean age, 35 years). Eighty-one (95%) of 85 patients who underwent serological testing were seropositive for antibodies to human immunodeficiency virus (HIV). Unlike previous investigators, we found a marked increase in the incidence and severity of eyelid (25%) and ocular (78%) complications as well as postherpetic neuralgia (55%). Visual loss occurred in 56% of the cases. Lack of medication, delay in presentation, severity

[Isolated vasculitis of the central nervous system and involvement of the 8th cranial nerve: rare manifestations of acquired immunodeficiency syndrome].

The authors report the case of an AIDS patient with rare neurologic manifestations: primary vasculitis of the central nervous system and VIII cranial nerve dysfunction. The authors make a review on the subject, and call special attention for the differential diagnosis. In fact, the patient, a 36 year old woman, with promiscuous life, presented with dizziness, gait ataxia, nausea, headache and hypoacusia. Seven days after the admission, she noted blurred vision in both eyes and soon she became blind. The physical examination showed bilateral optic neuritis and vestibulocochlear dysfunction, sti

Rhodopsin Thr58Arg mutation in a family with autosomal dominant retinitis pigmentosa.

The authors report a family in which a Thr58Arg rhodopsin mutation co-segregates with the disease phenotype of autosomal dominant retinitis pigmentosa (RP) in 16 family members. DNA sequence determination confirms the presence of the same mutation reported previously for one family apparently unrelated to the pedigree now reported. Features of RP in this family included a later onset of symptoms, with night blindness first noticed between ages 12 to 24 years. Although symptoms worsened with age, no complete blindness was observed even with advanced age. Results of psychophysical and electrophy

Differences in asymmetric perception of facial expression in free-vision chimeric stimuli and reaction time.

Two forms of a Chimeric Faces Free-Vision Task used to estimate cerebral asymmetry for perceiving facial expression were given to young male subjects, n = 60 in a first session and n = 40 in a retest session. Test-retest and split-half reliabilities were high. As expected from assumptions of right-hemisphere specialization for processing expression, faces with left-sided smiles were more frequently judged as looking happier than those with right-sided smiles in both forms. However, there were individual differences in direction and extent of bias, which were systematically related to reaction

Unmet need for presbyopia correction and its associated factors among school teachers in Hawassa city, South Ethiopia.

BACKGROUND: Presbyopia is a decline in the amplitude of accommodation with the onset in the age range 40-45 years affecting near visual task performance. As the age of presbyopia onset coincides with productive age, it results in great productivity loss especially in those with high near visual demand like teachers. A maximum near vision potential is essential for teachers in ensuring the quality of education, as most of the students' evaluations and scripts are assessed manually in Ethiopia. The prevalence of unmet need for presbyopia correction among school teachers ranges from 38.5-70.4% wo

Prospective study of brief neuropsychological measures to assess crash risk in older primary care patients.

BACKGROUND: Practicing primary care physicians often encounter the difficult clinical situation of evaluating the older driver. We wanted to investigate the relation between self-reported driving behavior, neuropsychological measures, and crash risk to inform the development of a test battery that could predict unsafe driving behavior and was feasible for use by primary care physicians. METHODS: This study was a prospective follow-up of 107 drivers aged 65 years and older recruited from a primary care setting in 1995. Tests of attention, visual information processing, spatial orientation, and

Super-additive multi-dimensional self-balancing deficit: implication for flight mishaps and falling.

We report super-additive impairment in participants using a joystick to stay upright and avoid crashing while riding in a motion simulator programmed to topple as an inverted pendulum away from an upright direction of balance about two axes (2D) relative to 1D. The 2D test configuration provided participants with full vestibular/somatosensory cues about tilt relative to gravity. The 2D impairment was a) only marginally mitigated by vision relative to blindfolded conditions, and b) worse than a 1D decrement in a 0-g analog condition which precluded gravitational tilt cues by having participants

A Unique Case of Compressive Optic Neuropathy From an Internal Carotid Artery Aneurysm.

Compressive optic neuropathy (CON) is a condition characterized by optic nerve damage caused by abnormal mechanical pressure. Here, we report a rare case of bilateral CON due to a large, right supraclinoid internal carotid artery (ICA) aneurysm. A 66-year-old female presented to the clinic with complaints of prior, intermittent headaches and decreased visual acuity. Notably, no relative afferent pupillary defect (RAPD) was present on exam. Visual field testing revealed an inferior altitudinal defect oculus dexter (OD) and a superotemporal defect oculus sinister (OS). This visual field pattern

Surgical approach to model facial nerve paralysis in an ovine model for in vivo testing of a novel intervention.

Facial nerve paralysis (FNP) disrupts neural inputs to the facial muscles, the most significant being those responsible for blinking. This loss often results in discomfort, corneal exposure, and if improperly treated, vision loss. FNP typically presents unilaterally, resulting in an asymmetric face, causing distress and social isolation. The Bionic Lid Implant for Natural Closure (BLINC) was developed to restore functional blinking in patients with FNP by mechanically actuating eyelid movement via an upper eyelid sling, with a focus on restoring a natural and symmetric appearance. To move this

Visually relevant on-bench through-focus analysis of intraocular lenses.

Cataract surgery involves the implantation of an intraocular lens (IOL) to replace the opacified crystalline lens. Monofocal IOLs, the most common type, are intended to have the eye in focus at a given distance, usually at infinity. Simultaneous vision IOLs (SVIOLs) and extended depth of focus (EDOF) aim to minimize postoperative dependence on spectacles by providing either multiple foci or an extended depth of focus. These lenses utilize a variety of diffractive and refractive designs to achieve varied focal depths. While common optical testing methods based on the IOL's modulation transfer f

Recurrent Cerebral Infarction Due to Moyamoya Disease Complicated With Systemic Lupus Erythematosus: A Case Report and Literature Review.

INTRODUCTION: We report a rare case of moyamoya disease caused by an RNF213 mutation, complicated with systemic lupus erythematosus. CASE REPORT: A 32-year-old woman experienced 4 cerebral ischemia stroke events within 6 months. The main symptom was left limb weakness with blurred vision in the right eye. Results of digital subtraction angiography conducted at another hospital were consistent with moyamoya disease. On genetic testing, we found that the patient carried 2 mutations in the moyamoya disease-related gene RNF213 (p.R4810K, p.T1727M). On the basis of the laboratory immunologic indic

A case of protein-losing gastroenteropathy due to Sjögren's syndrome detected by foggy vision with refractive error of the intraocular lens.

A woman in her forties visited an ophthalmologist for rapidly progressive foggy vision. Naked visual acuity had decreased to 0.15, and although her eyes showed no abnormalities, internal disease was suspected and albumin 2.6 g/dL was found. Protein leakage from the intestinal tract was suspected since there was no urinary protein excretion. 99mTechnetium-labeled albumin D scintigraphy showed protein leakage from the intestinal tract. A stool α1-antitrypsin clearance test showed an increase to 56.3 mL/day, leading to a diagnosis of protein-losing gastroenteropathy. Blood biochemistry revealed a

Review of encephalocraniocutaneous lipomatosis.

Encephalocraniocutaneous lipomatosis (ECCL), also known as Haberland syndrome, is a sporadic tumor predisposition neurocutaneous disorder, included in the oculoectodermal syndrome group of mosaic RASopathies. ECCL primarily affects the skin, central nervous system and eyes. Key diagnostic features include nevus psiloliparus, a hallmark subcutaneous lipomatous hamartoma associated with alopecia, along with subcutaneous lipomas, focal skin aplasia, and patchy alopecia. Neurologically, intracranial lipomas, particularly in the cerebellopontine angle, are prevalent, along with cortical dysplasia,
