The surgical treatment of bone metastases.

The Authors consider the surgical treatment of 122 patients (125 operations) with metastatic lesions of the skeleton. They outline the most commonly used surgical techniques (arthro- and endoprostheses, endomedullary implants, plates). Operations need to be planned with special standards and methods since the bone tissue is undergoing serious structural changes and the patients need rapid rehabilitation. To this end, the utility is emphasized of the substitution with prostheses even of extensive skeletal segments and the use of acrylic cement is recommended to replace loss of substance caused

[Treatment of Periprosthetic Distal Femoral Fractures].

PURPOSE OF THE STUDY Periprosthetic distal femoral fractures (PDFF) constitute an unpleasant complication in patients with a total knee replacement (TKR). The incidence reported in literature is 0.3-2.5 %. The number of periprosthetic knee fractures has been increasing due to the ageing of population, a growing number of implants, a longer life expectancy of patients, a more intensive physical activity of patients, and osteoporosis. Most of these fractures are treated surgically, non-surgical treatment is reserved solely for patients unable to undergo a surgery for general health conditions. M

Epiphysiodesis for the treatment of tall stature and leg length discrepancy.

Painful orthopedic conditions associated with extreme tall stature and leg length discrepancy (LLD) include back pain and adopting bad posture. After failure of conservative treatment options, blocking of the growth plates (epiphysiodesis) around the knee emerged as gold standard in patients with tall stature and LLD in the growing skeleton. Surgical planning includes growth prediction and evaluation of bone age. Since growth prediction is associated with a certain potential error, adequate planning and timing of epiphysiodesis are the key for success of the treatment. LLD corrections up to 5

Anterior minimal invasive internal fixator versus open plating in treatment of unstable pelvic ring injuries.

BACKGROUND: Pelvic fractures are frequently presented to major trauma centers and are mostly associated with high morbidity, especially in polytrauma patients. While stable fractures are managed non-operatively in most cases, unstable patterns necessitate surgical fixation because of the significant morbidity and mortality related to unstable pelvic fractures. OBJECTIVE: To compare the clinical, radiological, and functional outcomes in patients with unstable pelvic ring fractures treated with anterior minimal invasive internal fixator (INFIX) versus plate fixation. STUDY DESIGN: Retrospectiv

Locking Plate with Cerclage Wiring Versus Hook Plate Fixation for Unstable Distal Clavicle Fractures: Is There Still a Role for Hook Plates?

Background and Objectives: Hook plate fixation has been widely used for unstable distal clavicle fractures, but concerns remain regarding implant-related complications and the need for secondary removal. Locking plate fixation with supplementary cerclage wiring has been proposed as an alternative that may provide stability while reducing complications. This study compared the clinical and radiologic outcomes of locking plate fixation with cerclage wiring versus hook plate fixation. Materials and Methods: A retrospective review was performed on patients who underwent open reduction and internal

A Novel Technique for Intraoperative Navigation in Occipitocervical Fusion: A Preliminary Report.

BACKGROUND: Intraoperative navigation improves screw placement accuracy in pediatric occipitocervical fusion. All reports of navigated occipitocervical fusion using occipital plates in any age patient describe navigation placed at case onset to guide occipital and cervical fixation, or navigation placed at case onset to guide cervical screw placement alone before occipital fixation. We describe a novel technique for navigated occipitocervical fusion and analyze short-term outcomes in a pediatric case series. METHODS: We describe a novel technique for occipitocervical fusion using intraoperati

Anterior Locking Plate Osteosynthesis of Patellar Factures - Analysis of Complications and Functional Outcome.

BACKGROUND: Tension band wiring is the standard procedure for patellar fractures, but is associated with a high rate of implant related complications and implant failure. Tension band wiring may fail, especially with multifragmentary and comminuted fractures. Plate fixation of complex patellar fractures seems to be superior to wiring, both clinically and biomechanically. The aim of this study was to evaluate complications after locking plate fixation in patellar fractures two years after surgery and to access the functional outcome. MATERIAL AND METHODS: As part of a prospective case series,

Intraosseous Sliding Plate Fixation Used in Double Osteotomy Bunionectomy.

BACKGROUND:: Distal chevron metatarsal osteotomy bunionectomy is a commonly performed procedure for the treatment of mild to moderate hallux valgus. We continue to use the intraosseous sliding osteotomy plate system for fixation of the distal metatarsal osteotomy. With the addition of the Akin osteotomy, we are able to obtain reliable, reproducible correction with better cosmesis and increased patient satisfaction for more advanced deformities. METHODS:: We prospectively evaluated 138 (145 feet) consecutive patients who underwent double osteotomy bunionectomy using the intramedullary plate sy

Biomechanical analysis of clavicle hook plate implantation with different hook angles in the acromioclavicular joint.

PURPOSE: A clavicle hook plate is a simple and effective method for treating acromioclavicular dislocation and distal clavicle fractures. However, subacromial osteolysis and peri-implant fractures are complicated for surgeons to manage. This study uses finite element analysis (FEA) to investigate the post-implantation biomechanics of clavicle hook plates with different hook angles. METHODS: This FEA study constructed a model with a clavicle, acromion, clavicle hook plate, and screws to simulate the implantation of clavicle hook plates at different hook angles (90°, 95°, 100°, 105°, and 110°)

Challenges in Subtrochanteric Femur Fracture Management.

Subtrochanteric femur fractures present significant treatment challenges. The deforming muscle forces make fracture reduction difficult. Treatment options include cephalomedullary nailing and various types of plate fixation. There is a high rate of treatment complications, including malunion, delayed union, nonunion, and implant failure.

Paradoxical response to mechanical unloading in bone loss, microarchitecture, and bone turnover markers.

BACKGROUND: Sclerostin, encoded by the SOST gene, has been implicated in the response to mechanical loading in bone. Some studies demonstrated that unloading leads to up-regulated SOST expression, which may induce bone loss. PURPOSE: Most reported studies regarding the changes caused by mechanical unloading were only based on a single site. Considering that the longitudinal bone growth leads to cells of different age with different sensitivity to unloading, we hypothesized that bone turnover in response to unloading is site specific. METHODS: We established a disuse rat model by sciatic neur

Emerging concepts in upper extremity trauma: humeral shaft fractures.

Fractures of the humeral shaft are common in low-energy and high-energy trauma, and optimal clinical management remains controversial. Nonsurgical management has been supported as the preferred treatment based on high union rates and minimal functional deficit due to a rich vascular supply from overlying muscle and the wide motion available at the glenohumeral joint. Recent studies of nonoperative management have challenged surgeons' understanding of these fractures and the perception of favorable outcomes. Current considerations support expanded operative indications with traditional open-pla

Tibialis anterior tendon entrapment after ankle tightrope insertion for acute syndesmosis injury.

Ankle syndesmotic injuries often require anatomic reduction and fixation. Syndesmosis screw fixation is commonly used; however, this prevents physiological motion at the joint, often requires a second operation for screw removal, and may limit postoperative weight bearing. The Ankle Tightrope (Arthrex, Naples, FL) was introduced to allow physiological tibiofibular micromotion, early ankle range of motion, and weight bearing, and did not require a second operation for removal. For Maisonneuve injuries, or more severe syndesmosis instability, a second tightrope is required. This is advised to be
