Polyglycolide bioabsorbable screws in the treatment of ankle fractures.

Twenty-one patients with unstable medial malleolar, bimalleolar, or trimalleolar ankle fractures underwent open reduction and internal fixation of the medial malleolus with 4.5-mm polyglycolide screws. All lateral malleolar fractures were internally fixed with standard metallic implants. Radiographic and clinical follow-up results were available on 16 of 21 patients. All fractures healed at an average of 3.4 months (range, 3-6 months), and there were no medial wound infections. Eight of 16 patients developed an inflammatory reaction to the biodegradable polyester at 3 to 4 months after implant

Clinical biocompatibility and degradation of polylevolactide screws in the ankle.

The clinical biocompatibility and degradation behavior of absorbable fracture fixation devices are still incompletely known in humans. Fifty-one patients with displaced fracture of the ankle treated by open reduction and internal fixation with absorbable screws made of polylevolactide were seen in followup for at least 3 years. The mean duration of followup time was 52 months. The tissue tolerance and degradation of the devices were studied clinically and radiographically using computed tomographic scans. In addition, biopsy specimens for histologic examination were taken in 5 patients. The sc

Free patellar graft for the reconstruction of juxtaarticular defects in the treatment of giant-cell tumors.

The patella may solve the problem of an osteocartilaginous autograft for the articular reconstruction of the knee after resection of certain bone neoplasms. Such a solution may be adopted mainly after the resection of tumors that have attacked part of the tibial plate or that have necessitated the removal of the tibial plate to ensure a radical procedure. The cartilaginous surface of either one or both of the patellar articular facets is well suited for restoring the continuity of the tibial articular surface lost through resection. Furthermore, all of the remaining part of the patella provide

[Is a radial nerve paralysis in a humeral shaft fracture an indication for surgery? Indications].

Surgery for humeral shaft fractures. A series of 78 operative treated humeral shaft fractures was studied retrospectively. Sixteen times we found a radial nerve palsy, preoperatively. In this group we evaluated the type of fracture, the operation indication, the time of treatment and the postoperative recovery of the radial nerve palsy. Fourteen times we did an early exploration. The indication to operate was 9 times the conducting polytrauma, 5 times the radial nerve palsy itself and 2 times a pseudarthrosis. In 13 of the 16 patients nerve function recovered either clinically and/or electromy

[Pharyngo-esophageal risk of surgery of the cervical spine using an anterior approach].

The authors report six cases of pharyngo-oesophageal damage associated with fractures of the cervical spine treated by osteo-synthesis via the anterior route. These complications, not previously described, may have life threatening (especially mediastinitis) and functional consequences. They discuss the relative responsibility of: the initial trauma, which should be better defined by an initial endoscopic assessment. the surgical approach, the principles of which and its dangers and preventive aspects are discussed, early or late fixation failure justifying careful surveillance of the osteosyn

Slipped capital femoral epiphysis long-term results after 10--38 years.

Thirty-four patients with chronic slipped capital femoral epiphysis (SCFE) involving 45 hips were followed over a period of no less than 10 years and as great as 38 years. Excluding those who had had intraoperative complications, 30 hips evaluated between 10 and 20 years follow-up were found to have good or excellent results. Fifteen hips followed greater than 20 years, however, showed fair to poor grading in 10 of 15 hips. This progessive change seemed to be related most closely to the bilateralism of the disease. These observations suggest that while follow-up of a 20 year duration may show

Axis 4-screw technique for occipital-cervical fixation in the treatment of atlantoaxial dislocation with axis osseous deformity: a retrospective study with a minimum 1-year follow-up.

OBJECTIVE: Atlantoaxial dislocation (AAD) with axis osseous deformity poses significant surgical challenges, particularly in complex anatomical cases. This study compares the efficacy of 2-screw versus 4-screw occipital-cervical fixation in improving neurological outcomes and reducing complications in patients with AAD and axis deformity. METHODS: A retrospective study was conducted on 37 patients with AAD and axis osseous deformity treated at the authors' institution from 2017 to 2023. The cohort included 19 patients treated with bilateral pars screws and translaminar screws (4-screw group)

Motion preserving C1 ring lateral mass screw fixation with transverse rod for isolated unstable Jefferson fracture of atlas vertebra: a multicentric study.

PURPOSE: Some cases of isolated unstable Jefferson's fracture of the Atlas verbebra require surgical fixation. Historical constructs with occipitocervical fixation or C1C2 posterior Harms-Goel fixation remain associated with functional impairment owing to limited postoperative cervical range of motion. Since 2004, motion-preserving C1 ring lateral mass screws with transverse rod (C1 LMF) has emerged as a possible technical alternative. METHODS: Retrospective multicentric study of patients operated on for isolated unstable Jefferson's fracture of the Atlas vertebra using C1 LMF in two tertiary

Decreased femoral fracture load after cephalomedullary nail removal : a biomechanical ex vivo study.

AIMS: Spontaneous neck fractures are feared complications of cephalomedullary nail removal after successful healing of per- and subtrochanteric fractures. To date, the initial postoperative stability as well as the correct weightbearing regimen remain unclear. The aim of this biomechanical ex vivo study was to evaluate the initial postoperative failure load after hardware removal of specimens, which received cephalomedullary nails during their lifetime. METHODS: A total of 20 specimens of voluntary body donors were included in this study. Group 1 (n = 10) consisted of specimens that received

Robotic navigation assistance for spinal fixation in Marfan syndrome: a case report addressing anatomical challenges.

BACKGROUND: Patients with Marfan syndrome (MFS) can experience spinal deformity characterized by thoracolumbar kyphoscoliosis, severe spondylolisthesis, dural ectasia, vertebral body scalloping, and hypoplastic pedicles. These unique anatomical features complicate conventional spinal instrumentation techniques, rendering robotic navigation assistance (RNA) a viable option in such cases. CASE DESCRIPTION: A 41-year-old woman diagnosed with MFS presented with progressive mid-thoracic and low back pain spanning over two decades. Imaging revealed characteristic findings of MFS, including severe l

Biomechanical study of rod stress in lumbopelvic fixation with lateral interbody fusion: an in vitro experimental study using synthetic bone models.

OBJECTIVE: Despite improvements in surgical techniques and instruments, high rates of rod fracture following a long spinal fusion in the treatment of adult spinal deformity (ASD) remain a concern. Thus, an improved understanding of rod fracture may be valuable for better surgical planning. The authors aimed to investigate mechanical stress on posterior rods in lumbopelvic fixation for the treatment of ASD. METHODS: Synthetic lumbopelvic bone models were instrumented with intervertebral cages, pedicle screws, S2-alar-iliac screws, and rods. The construct was then placed in a testing device, an

Retrospective study of radial dome osteotomy with volar plate fixation versus K-wires in Madelung's deformity: long-term follow-up.

PURPOSE: The emerging role of the locking plate improved the technique also in corrective osteotomies in Madelung's deformity, but there is a lack of analyses between the fixation techniques, as well as little information in functional outcomes and long-term follow-up. The current study compared the outcomes, pitfalls, and advantages of volar plate and screws fixation versus K-wires fixation in a long-term follow-up. METHODS: Twenty-eight children presenting the "distal radius" variant of Madelung's deformity underwent Vickers ligament release and distal radial dome osteotomy between 2009 and

Transverse Stress Fracture of the Proximal Patella: A Case Report.

Among stress fractures associated with sports activities, patellar stress fracture is rare. Regarding patella stress fractures, so far only distal transverse or lateral longitudinal fractures have been reported, but there are no reports of transverse fractures occurring in the proximal patella. We describe an extremely rare case of transverse stress fracture of proximal patella in a 9-year-old athlete.A 9-year old boy, who participated in sports (sprints and Kendo) presented with left knee pain without any external injury. In plain radiographs, a fracture line was observed in the proximal 1/3

Current treatment modalities of osteochondritis dissecans of the knee joint: results of a nation-wide German survey.

In a nation-wide survey 350 knee surgeons were asked for their current diagnostic and treatment modalities in patients with osteochondritis dissecans (OD) of the knee joint: 255 questionnaires were analyzed. With respect to the different imaging techniques, standard xray studies were performed by all surgeons. Other techniques were used in the following decreasing order of preference: xray tomography, MRI, CT-scan, ultrasound, bone scan, and arthrography. The indication for surgery was primarily influenced by the standard xray, and secondarily by xray tomography, then by MRI, and CT-scan. Rega

[Press-fit bone dowel arthrodesis of the ankle or the subtalar joint using a diamond bone cutting system. Surgical technique and initial results in 10 patients].

Arthrodesis of the ankle (AA) or the subtalar joint (AST) is still a necessary treatment in case of painful posttraumatic arthrosis or paresis of the muscles after compartment syndrome. Today the alloplastic ankle joint replacement does not satisfy. Many treatments of arthrodesis with minimal or extended resection of the joint surface with or without bone transplantation are described in literature. We present in detail a new developed technique of press-fit dowel arthrodesis (KDA) for the ankle and subtalar joint. After adjustment of the joint position and retention with Kirschner wires the s

Evaluation of screw density in Lenke type 1 adolescent idiopathic scoliosis: are low and high-density constructs truly comparable?

PURPOSE: Adolescent idiopathic scoliosis (AIS) affects 3% of adolescents, with Lenke 1 curves comprising 40% of cases. All-screw (AS) constructs provide adequate curve correction and maintenance in the coronal plane. Low-density (LD) screw constructs may offer comparable deformity correction, reduced implant density, and lower costs compared to high-density (HD) constructs. This study compares LD and HD screw constructs in Lenke 1 AIS, analyzing radiographic and surgical outcomes. METHODS: The study included 86 pediatric patients with Lenke 1 AIS curves (Cobb angle < 80°), treated between Jan
