Comparison of second-echelon treatments for Ménière's disease.

IMPORTANCE: To compare the efficacy of treatments commonly offered to patients with Ménière's disease who fail conservative medical therapy including diuretics and a sodium-restricted diet. OBJECTIVES: This study compared three second-echelon treatments: the Meniett device, endolymphatic sac decompression, and intratympanic gentamicin injections to determine their comparative effectiveness and capacity to mitigate against the necessity of a surgical labyrinthectomy. DESIGN, SETTING, AND PARTICIPANTS: Retrospective observational study at an academic tertiary care center. Patients with Ménière

Functional and patient-reported outcomes in symptomatic lumbar spinal stenosis following percutaneous decompression.

BACKGROUND: Neurogenic claudication due to symptomatic lumbar spinal stenosis (LSS) is a painful condition causing significant functional disability. While the cause of LSS is multifactorial, thickened ligamentum flavum (LF) accounts for up to 85% of spinal canal narrowing. mild percutaneous lumbar decompression allows debulking of the hypertrophic LF while avoiding the morbidities frequently associated with more invasive surgical procedures. METHODS: In this prospective case series study, consecutive LSS patients presenting with neurogenic claudication were treated with percutaneous lumbar d

Effects of tendon transfer to restore index finger abduction for severe cubital tunnel syndrome.

BACKGROUND: Severe cases of cubital tunnel syndrome do not always result in functional recovery after surgical decompression of the ulnar nerve. A combined operation of tendon transfer to restore index finger abduction and decompression of the ulnar nerve was performed for patients with severe cubital tunnel syndrome who required powerful pinch strength and whose preoperative compound muscle action potential of the abductor digiti minimi muscle was not recordable or almost non-recordable. The purpose of this study was to assess the efficacy of tendon transfer to restore index finger abduction

Single level lumbar disc herniations resulting in radicular pain: pain and functional outcomes after treatment with targeted disc decompression.

STUDY DESIGN: Single-arm prospective clinical trial. OBJECTIVE: To determine the treatment effect and MRI changes of targeted disc decompression using an intradiscal catheter for focal heating of symptomatic lumbar disc protrusions resulting in radicular pain. SUMMARY OF BACKGROUND DATA: Percutaneous methods for treatment of lumbar disc herniations are potentially attractive alternatives to open surgical discetomies. The effects of a newly designed, percutaneous electrothermal intradiscal catheter were evaluated. METHODS: Twenty-two subjects with MRI confirmed, single-level contained lumbar

Surgical experience in 130 pediatric patients with Chiari I malformations.

OBJECT: The aim of this study was to present the long-term findings of a surgical series of pediatric patients with Chiari I malformations. METHODS: One hundred thirty symptomatic pediatric patients with Chiari I malformations underwent posterior fossa decompression. The age in this group of patients ranged from 2 months to 20 years (mean 11 years). The length of the hospital stay extended from 2 to 7 days (mean 2.7 days), and follow up was from 3 months to 15 years (mean 4.2 years). Patients most often presented with headache/neck pain (38%) and scoliosis (18%). Examples of associated diagno

Neck Pain: Differential Diagnosis and Management.

Axial neck pain is a common and important problem in the outpatient setting. In isolation, neck pain tends to have a musculoskeletal etiology and responds best to medication and targeted physical therapy. Careful history and physical examination are required to ascertain if there is a neurologic component in addition to the patient's neck pain. For patients needing surgical intervention, there are a variety of approaches and operations that can decompress the appropriate nerve root or the spinal cord itself. These operations are generally well-tolerated and provide significant benefit for appr

Cor triatriatum sinister with an intact interatrial septum and a decompressing vein in a toddler.

Cor triatriatum sinister is a very rare cardiac anomaly that may lead to pulmonary hypertension, right ventricular dilation, and eventually right heart failure. We report a case of a toddler who presented with respiratory distress and cardiomegaly and was found to have cor triatriatum sinister with a restrictive communication, decompressing vertical vein, pulmonary hypertension, severe tricuspid regurgitation, and severe right ventricular dysfunction. She underwent a successful surgical repair, with normalisation of right ventricular function and pulmonary artery pressure.

[Complete brachial plexus paralysis caused by compartment syndrome in heroin intoxication].

We report the case of a young man with heroin intoxication. While deeply unconscious, he sustained a compartment syndrome of the arm and shoulder region leading to a lesion of the upper plexus. Immediate surgical decompression by fasciotomy incisions, intensive care treatment including hemofiltration to treat myoglobinemia, intense physical exercise, and mesh-grafting closure of the wounds soon led to unexpected recovery. The function of the arm was restored in such a way that the patient was able to intoxicate himself again. He needed intubation and ventilation but recovered uneventfully.

[Conversion of atrial fibrillation after mitral valve valvuloplasty].

As in the case described here, thromboembolism originating in the heart may indicate stenosis of the mitral valve. In addition to the risk of an embolism, increased by factor 17 in the case of rheumatic mitral valve disease, such patients have hemodynamic problems, in particular resulting from the loss of active atrial contraction in atrial fibrillation. Surgical correction of a mitral stenosis can be delayed for years by balloon valvuloplasty. Following decompression achieved in this way, cardioversion to sinus rhythm may be worth attempting, even when the atria are clearly dilated.

Decompression of the orbit using a lateral rhinotomy approach in patients with malignant endocrine exophthalmos.

Malignant endocrine exophthalmos is a complication seen in 1-2% of patients treated for thyrotoxicosis. A variety of surgical techniques have been used to prevent loss of vision due to compression of the optic nerve. In this study, nine patients with rapidly diminishing vision were operated upon using a lateral rhinotomy technique creating the largest possible space for expansion into the nose and decompressing the optic nerve far back in the orbit. In all patients the vision was improved 1 year after surgery and the proptosis was decreased.

Solitary histoplasmoma of the thalamus: case report and literature review.

Only nine cases of solitary thalamic abscess have been reported. All were of bacterial origin, and three were successfully treated by surgical drainage and antibiotic therapy. The authors present the history of a patient with a histoplasmoma of the thalamus. As is often the case with solitary thalamic lesions, this lesion was initially assumed to be a tumor and was first treated with radiotherapy. Despite the sensitive location of the lesion, tissue diagnosis and decompression was safely accomplished with the aid of ultrasonic localization. Appropriate antifungal treatment was given, and the p

Surgical treatment of plantar ulcers in leprosy.

Plantar ulcers present a serious problem in the management of leprosy. After studying the mechanism of ulceration and the causes of their indolence, it appears that the treatment of the disease by antileprotic drugs is not sufficient. The underlying causes, such as anaesthesia, bony deformities and paralysis, should usually be treated by surgical means. Anaesthesia can be reversed by decompression of the posterior tibial nerve if this is done early. Deformities should be corrected by procedures on the soft tissues or bones, and chronic infection eradicated to prevent recurrence of the ulcerati

Basal encephalocele presenting with monocular visual loss.

A 23-year-old male was seen with acute onset of decreased vision in the right eye. There was no associated facial abnormality, optic disc abnormality, or previous history of head trauma. A computerized tomography scan of the head revealed a soft tissue density in the right ethmoidal sinus. Surgical decompression of the ethmoid sinus for a possible pyocele or mucocele revealed the mass to be cerebral cortex. Successful neurosurgical repair of the defect in the cribiform plate and amputation of the abnormal brain tissue was accomplished with restoration of vision.

Applications and techniques of gastrointestinal intubation.

Gastrointestinal intubation has found increasing applications over the past several years. Although usually an uncomplicated procedure, passage of an intestinal tube may be impeded by ineffective peristalsis, obstructing lesions or surgical alteration of the normal anatomy. A variety of techniques are described for passing tubes into the stomach, duodenum and small bowel. These techniques are an outgrowth of those initially developed for manipulating tubes through the vascular, biliary and genitourinary systems. The characteristics of the various decompression and alimentation tubes as well as

Tetralogy of Fallot with acquired pulmonary atresia and hypoplasia of pulmonary arteries. Report of surgical management in infancy.

A perforated intracardiac prosthesis, and patch infundibuloplasty and annuloplasty were employed for the surgical management of an infant with tetralogy of Fallot, pulmonary atresia, and severe hypoplasia of the pulmonary arteries. This approach provides a means of decompression of the right ventricle in the immediate postoperative period. It allows sufficient systemic cardiac output while increasing pulmonary blood flow to relieve severe hypoxemia and cyanosis. In addition, the high risk of further compromise to the diminutive pulmonary arteries by a palliative shunt is avoided. A physiologic

Aorto-Pulmonary Shunt Creation in Congenital Heart Disease: Different Indications, Various Modifications, but Common Technical Challenges.

Percutaneous creation of an aorto-pulmonary (AP) shunt has been explored in preclinical models for over 15 years, yet its clinical application remains limited. Surgical AP shunts are widely used to augment pulmonary blood flow in cyanotic congenital heart disease, but repeated thoracic operations and general anesthesia carry substantial risks in critically ill or surgically complex patients. A less invasive transcatheter alternative could therefore represent a transformative option. We report a case series of two pediatric patients in whom a percutaneous connection between the aorta and pulmon

Subaxial cervical spine fractures with facet joint dislocation: Surgical management, technique, and results in a retrospective series of 36 patients.

OBJECTIVE: Subaxial cervical spine fractures with facet dislocation are burdened by a high risk of neurological impairment. The surgical management of this severe clinical condition is poorly standardized, concerning the role of closed reduction and the use of anterior, posterior, or circumferential approaches. We performed a retrospective evaluation of our series to assess the safety and efficacy of our surgical strategy. MATERIALS AND METHODS: Transcranial traction was progressively applied, followed by anterior decompression and arthrodesis. Whether adequate reduction was not obtained with

Applicability of RECIST1.1, MDACC and SPINO Tumor Response Criteria After Adjuvant Stereotactic Spinal Radiosurgery in Surgically Treated Spinal Metastases.

Background and Objectives: Up to 40% of patients with cancer develop spinal metastases, and stereotactic spinal radiosurgery (SSRS) achieves high local control rates as definitive or postoperative treatment. Multiple tumor response assessments have been used but their compared clinical performance in post-surgical patients remains unclear. We sought to compare the applicability of RECIST1.1, MDACC and SPINO criteria. Materials and Methods: This IRB-approved retrospective study included patients with high-grade epidural spinal cord compression treated with decompressive surgery followed by adju
