This article describes the complex orthodontic treatment of a 22-year-old patient with Duchenne muscular dystrophy and macroglossia. His orthodontic treatment hinged on providing proper informed consent and management of the malocclusion with glossectomy, extractions, fixed appliances, and elastics. Challenges to traditional treatment are outlined, and compromises to both process and outcome are discussed from an informed consent point of view because of the serious risks involved. The treatment objectives were met, and the outcome was considered a success.

Tapping mode-atomic force microscopy and force spectroscopy were used for studies of the topography of the cell surface and elastic properties of lymphocytes from healthy donors and patients with chronic lymphoblastic leukemia. It was demonstrated that the decrease in lymphocytes stiffness in patients with chronic lymphoblastic leukemia by 51.4% (p<0.05) was accompanied by spatial modification of the cell surface, in particular, increase in the number of globular protrusions and depressions by 247 and 122%, respectively (p<0.05), in comparison with normal lymphocytes.

Annular elastolytic giant cell granuloma (AEGCG) is a rare granulomatous skin disease of unclear pathogenesis which belongs to the group of disorders in the skin and elastic fibers with similar clinical features of granuloma annulare (GA). This case report is intended to describe a rare hybrid pattern in histopathology demonstrating coexistence of AEGCG and GA. An endocrine disease, such as diabetes mellitus (DM), could contribute to the coexistence of both lesions, and this possibility must be included in the medical investigation.

Anetoderma is an uncommon disease characterized by multiple circumscribed atrophic, herniated skin lesions on trunk, thighs and upper arms caused by loss of elastic fibers. Associations with autoimmune diseases or infections, especially spirochetal infections, have been described. We report a case of anetoderma with an increased serum Borrelia burgdorferi IgM-titers. After treatment with doxycycline 200 mg/day for three weeks, the progression of the disease stopped and no new lesions appeared.

Lacquer cracks are breaks in the Bruch membrane by alteration of the elastic layer. They are often associated with pseudoxanthoma elasticum. The authors report the first case in Burkina Faso involving a 51-year-old woman who was seen for visual acuity loss. She had pseudoxanthoma elasticum associated with lacquer cracks complicated by choroidal neovascularization in both eyes. She was treated with laser photocoagulation of the neovessels in the left eye, and no recurrence was reported after 6 months.

A 70-year-old male was admitted to our hospital with the chief complaint of a left intrascrotal mass. A painless, solid and elastic-hard mass was palpable in the left scrotum. The testis, epididymis and spermatic cord could not be palpated separately from the mass. Tumor resection was performed. The tumor was 13 x 7.5 x 3.0 cm in size and 285 g in weight. Histological diagnosis was schwannoma. To our knowledge, the case was considered as the sixth reported in Japan.

Acute infantile haemorrhagic oedema (AIHO) is characterised by purpura, ecchymosis and inflammatory oedema of the face and extremities. It is seen in children aged 4-24 months. The atiology is not known. We report a case of AIHO diagnosed by skin biopsy demonstrating leukocyte-elastic vasculitis. Laboratory studies showed positive hepatitis A IgM and IgG antibodies. The liver function tests were normal, indicating subclinical hepatitis. Cryoglobulinaemia was detected, suggesting that the disease was related to hepatitis A.

Pseudoxanthoma elasticum (PXE) is a rare inherited connective tissue disorder which is characterized by elastic tissue degeneration involving the skin, eyes and arteries. We describe the case of a hypercholesterolemic 25 year-old woman with typical genetic dominant form of PXE revealed by distal limb artery involvement after a pregnancy. In young adults with arterial disease, a non atheromatous cause should always be sought for, even if risk factors for atherosclerosis are present.

A histologically unique glioneuronal neoplasm occupying the lateral ventricle of a child was immunohistochemically and ultrastructurally characterized. Its principal component exhibited the characteristic features of ependymoma, whereas a minor population of neuroendocrine cells, occurring singly and in small clusters, lay scattered throughout the ependymoma component. Yet another unusual finding was the presence of numerous elastic fibers within the extracellular matrix. This tumor is considered to represent a true mixed neoplasm consisting of ependymal and neuroendocrine elements.

Acro-angiodermatitis is a very common disorder, with a close clinical, anatomical and morphological resemblance to Kaposi's sarcoma. Several types of this disorder can be found in different settings. However, these conditions are often misdiagnosed and therefore mistreated. A review of the literature and a classification of all types of acro-angiodermatitis are presented. We also describe a case of a patient with acro-angiodermatitis which completely regressed following a course of dapsone combined with leg elevation and elastic support stockings.

A case of cystic adventitial degeneration (CAD) of the popliteal artery is presented. Histologically the cyst appeared localised in the adventitia, outside the elastic lamina. A lectin-histochemical study evaluated the following peroxidase conjugated lectins: Peanut agglutinin, Concanavalin A, Ulex Europaeus (UEA I) and Wheat Germ Agglutinin. The lack of UEA I reactivity excluded an arterial origin of the cyst. Moreover, the lectin binding pattern of CAD appeared to be similar to the reactivity pattern of normal synovia.

Among 22 proven branchiogenic or thyreoglossal cysts, 15 presented with a characteristic pattern of homogeneous smooth reflections within a well-demarcated thin and elastic, deformable wall when using a high-resolution transducer. The other cases, showing an inhomogeneous texture and/or irregular walls, cannot be distinguished from inflammatory or neoplastic processes with central necrosis. Complete surgical removal is recommended especially in these latter cases. One case of malignant transformation was found.

Forty-four patients presenting with painful outlet constipation had internal rectal intussusception documented by cinedefecography. Rectal reservoir reduction by multiple elastic ligation or staple excision of redundant mucosa effectively improved bowel function and outlet symptoms in the majority of patients. Rectal reduction procedures were performed under intravenous sedation with antibiotic coverage, no postoperative sepsis, and a single case of significant postoperative bleeding. Associated abnormalities on defecography were unaffected.

Hereditary haemorrhagic telangiectasia is an autosomal dominant disorder characterized by the presence of mucocutaneous dilatation of capillaries and venules. The underlying defect, a deficiency of vascular elastic tissue and muscle, predisposes these vessels to injury and inadequate contractility. The ultrastructure of a myocardial tumor, referred to by a variety of names, which most currently is myocardial fibroma, has been studied. Although the tumor cells did not have the structure associated with protein synthesizing cells or active fibroblasts, there was evidence that they were participating in formation of the abundant stroma rich in collagen and elastic fibers in varying stages of maturity. The morphology of the stromal elements was similar to that in the surrounding myocardium and that described in the "normal" heart but differed from the elastica described in fibroelastosis.

We report the case of a patient with tricuspid and pulmonic valve atresia, with survival to the age of 21 years. Postmortem examination revealed an anomalous vessel connecting the ascending aorta with the main pulmonary artery. This fistula measured 20 mm by 4 mm, arose independently of the coronary arteries and had the histologic features of an elastic artery. The remarkable longevity was ascribed in part to the maintenance of pulmonary arterial flow by the left-to-right shunt through the anomalous arterial connection.
