Corrective osteotomy of a malunion of the distal radius using a spanning plate in an older patient.

INTRODUCTION: Distal radius malunions in older patients represent a therapeutic challenge due to osteoporotic bone quality and the need to preserve autonomy. Spanning plates, which are widely used in complex distal radius fractures, allow immediate weight-bearing, but their use in corrective osteotomies has not been reported. CASE REPORT: We report the case of a 77-year-old right-handed female patient, dependent on a cane, who presented with a distal radius malunion 3 months after conservative management of a displaced fracture. The deformity resulted in severe functional impairment and loss

Primary hypertrophic osteoarthropathy with anemia as assessed by 18F-FDG PET/CT.

Primary hypertrophic osteoarthropathy (PHO), also known as pachydermoperiostosis, is a rare genetic disorder with autosomal inheritance. We present the case of a 31-year-old male with a disease onset at 12 years of age, initially presenting as digital clubbing of the hands and feet and ankle joint hypertrophy. The clinical phenotype progressed significantly by age 17, with the development of marked facial skin thickening, deepened centripetal skin folds, a corrugated scalp, hypertrophic alae nasi, acne, ptosis, and palmoplantar hyperhidrosis. Anemia was identified at age 19, accompanied by per

TOWARD AN IMAGING-CENTRIC DEFINITION OF NONPARANEOPLASTIC AUTOIMMUNE RETINOPATHY.

PURPOSE: To explore characteristic imaging features of nonparaneoplastic autoimmune retinopathy (npAIR) to augment diagnostic criteria. METHODS: This is a retrospective cohort study of patients with npAIR evaluated at the Emory Eye Center between 2013 and 2019. Multimodal fundus images were evaluated to characterize the evolution of the disease. RESULTS: Twenty-one eyes of 12 patients were classified as having npAIR. Five patients (42%) were female, with median (range) age of 59 years (45-85 years). Median baseline visual acuity was 20/30 (20/20 to hand motions). Disease was asymmetric in 11

Methods of Assessing Skeletal Maturity When Planning Surgeries About the Knee.

Increased participation in youth sports has led to more knee injuries necessitating surgical intervention. Among the youngest athletes, such procedures typically involve physeal-respecting techniques for anterior cruciate ligament reconstruction, medial patellofemoral ligament reconstruction for patellar instability, osteochondritis dissecans fixation, and implant-mediated guided growth procedures. In each case, the choice of appropriate intervention is critically dependent on a patient's skeletal maturity. Compared with chronologic age, skeletal age accounts for individual maturation and is t

Pachydermoperiostosis: a case report of initial improvement with etoricoxib.

INTRODUCTION AND IMPORTANCE: Pachydermoperiostosis (PDP) is a syndrome characterised by the triad of pachydermia, digital clubbing and periostosis of long bones and its scarce incidence and similarity in clinical features with acromegaly makes the diagnosis challenging. The elevated PGE2 levels have been hypothesised as one of its mechanisms and therapies have been targeted to inhibit this prostaglandin. CASE PRESENTATION: A 25-year-old man with no comorbidities presented to OPD with a 10-year history of bilateral pain and swelling of the hands and feets associated with hyperhidrosis, grade I

Construction of predictive model for osteoporosis related factors among postmenopausal women on the basis of logistic regression and Bayesian network.

Osteoporosis is a prevalent chronic disease that often goes unnoticed in postmenopausal women. Early identification of risk factors for osteoporosis in postmenopausal women is essential. This study aimed to develop predictive models for osteoporosis-related factors among postmenopausal women in the U.S. and explore the influencing factors. In this cross-sectional study, we included 4417 postmenopausal women from the NHANES (2009-2010, 2013-2014, and 2017-2020). Through multiple regression analysis, we found that age, minutes of sedentary activity, prednisone or cortisone usage, arthritis, bone

Pronation-Type Galeazzi-Equivalent Fracture: A Case Report.

Fractures of the forearm are common among children and adolescents. Radial shaft fracture with dislocation of the distal radioulnar joint (DRUJ), called Galeazzi fracture, is unusual in pediatrics. The Galeazzi-equivalent fracture is a variant of the classic Galeazzi fracture that occurs in children and adolescents. It is a radius fracture associated with a distal ulnar displaced physeal injury without dislocation of the DRUJ. Our patient was a male, aged 15 years, who visited our emergency department after falling off a scooter onto his left hand. Left wrist X-rays showed a displaced Galeazzi

Acalvaria: the first case report from Nepal.

INTRODUCTION AND IMPORTANCE: Acalvaria is a rare congenital malformation in which the flat bones of the cranial vault, dura mater, and associated muscles are absent while the central nervous system usually remains unaffected. It is an extremely rare congenital anomaly with only a handful of cases being reported in literature. CASE PRESENTATION: The authors report a case of a 2-month-old male infant with acalvaria who was delivered at home and brought to our centre with the complaint of an abnormally soft skull. He was evaluated in a primary centre in rural Nepal. Parietal bones were missing b

Genetic, Cellular and Molecular Aspects involved in Apical Periodontitis.

The development, establishment and repair of apical periodontitis (AP) is dependent of several factors, which include host susceptibility, microbial infection, immune response, quality of root canal treatment and organism's ability to repair. The understanding of genetic contributions to the risk of developing AP and presenting persistent AP has been extensively explored in modern Endodontics. Thus, this article aims to provide a review of the literature regarding the biochemical mediators involved in immune response signaling, osteoclastogenesis and bone neoformation, as the genetic component

[Identification of epithelial-mesenchymal transition markers (EMT) by immunohistochemistry in pediatric osteosarcoma and association with clinical outcomes].

UNLABELLED: The epithelial-mesenchymal transition (EMT) is the ability of epithelial and mesenchymal cells to exchange phenotypes transiently. Its identification in carcinomatous cells has been associated with aggressive clinical phenotypes. In sarcomas, this ability is under study. OBJECTIVE: to evaluate the expression of two transcription factors involved in EMT by immunohistochemistry in pediatric osteosarcoma and its association with clinical outcomes. PATIENTS AND METHOD: A retrospective cohort study in children under 18 years of age with osteosarcoma diagnosis. Immunohistochemistry was

[Third molars in the orthodontic therapeutic decision].

INTRODUCTION: Despite a high prevalence of agenesis, third molars should frequently be considered in our orthodontic treatment plans. MATERIAL AND METHOD: The aim of this study was to describe, according to the literature, the therapeutic possibilities of third molar management. RESULTS: The avulsion isn't systematic. The French National Authority for Health has updated its guidance about it. Indeed, surgical procedures can present with potentially irreversible risks (nerve damage, bone necrosis). It is necessary to assess the benefit/risk balance and to inform the patient accordingly. Wheth

Editorial Commentary: Shoulder Biceps Tenodesis Versus Tenotomy: Both Show Good Results and Have Different Indications.

Shoulder long head biceps pathology is one of the most common causes of shoulder pain. The fact that there are many surgical techniques available has led to discussion of which should be the best treatment; although, in general terms, the two main options are tenotomy or tenodesis. Tenotomy is a simple technique, with a low rate of complications and a very good cost-benefit ratio, faster recovery, and less use of narcotic pain medications. Tenodesis has a lower risk of "Popeye deformity" and theoretically better biomechanics and strength, but few studies confirm superior outcomes in cases of b

Prevalence of 25(OH)D insufficiency and overweight/obesity in an adult population from the Central Italy.

BACKGROUND: Nowadays, excess weight is reaching epidemic proportions and it is necessary to identify patients at greater metabolic risk, in order to determine the most suitable pharmacological and dietetic options. On the other hand, vitamin D insufficiency (or hypovitaminosis D) is prevalent in obese patients. It most commonly occurs in people with inadequate sunlight exposure and nutritional intake of vitamin D, disorders limiting vitamin D absorption, and conditions impairing vitamin D conversion into active metabolite, including certain liver, kidney, and hereditary disorders. It can impai

Mechanical vibration as an adjunct to clear aligner treatment for accelerating tooth movement: A review.

Many patients, particularly adults, may prefer clear aligner treatment due to its esthetics and ease of use. Some studies have shown that mechanical vibration can affect the rate of tooth movement and other aspects of orthodontic treatment. The purpose of this systematic review was to substantiate the effects of vibration as an adjunct to clear aligner treatment. A comprehensive search of the PubMed, Embase, Cochrane library, and Scopus and also hand searching of reference lists was conducted for finding published studies up to March 2021. Two authors reviewed the titles and abstracts independ

De novo serine synthesis regulates chondrocyte proliferation during bone development and repair.

The majority of the mammalian skeleton is formed through endochondral ossification starting from a cartilaginous template. Cartilage cells, or chondrocytes, survive, proliferate and synthesize extracellular matrix in an avascular environment, but the metabolic requirements for these anabolic processes are not fully understood. Here, using metabolomics analysis and genetic in vivo models, we show that maintaining intracellular serine homeostasis is essential for chondrocyte function. De novo serine synthesis through phosphoglycerate dehydrogenase (PHGDH)-mediated glucose metabolism generates nu

Fibrous dysplasia in cardio-facio-cutaneous syndrome: A case report and review of literature.

Cardio-facio-cutaneous (CFC) syndrome (OMIM #:115150, 615278, 615279, 615280) is a rare genetic condition caused by variants in the RAS/mitogen-activated protein kinase (MAPK) signal transduction pathway. Up to 75% of cases are caused by mutations in the BRAF gene, whereas KRAS gene mutation has only been reported in <2% of cases. CFC syndrome is characterized by cardiac abnormalities, distinctive craniofacial dysmorphism, and various cutaneous abnormalities. Musculoskeletal and orthopedic manifestations are also prevalent in patients with CFC syndrome, among which the most common are skeletal

Reconstruction of Completely Circumferentially Degloved Thumb Using Omental Flap and Tubular Split-thickness Skin Graft.

Various types of thin flaps have been used for reconstruction in cases of complete circumferential degloving injuries of digits (Urbaniak classification class III). The omental flap has extensive, pliable, and well-vascularized tissue and helps prevent bone resorption of the distal phalange that can result from avascular necrosis. A 31-year-old right-handed man with class III left thumb injury and compartment syndrome of the left forearm was treated successfully with a free omental flap wrapped around the thumb and then covered with a tubular split-thickness skin graft. The postoperative cours

Surgical treatment of metastatic bone disease of the distal extremities.

Metastatic bone disease of the distal extremities, also known as acrometastasis, is very rare. Thus, there is very limited information regarding the clinical manifestations and methods of surgical treatment. The current available literature shows that acrometastases are often encountered in the context of advanced disease and are thus associated with poor patient survival. As metastatic bone disease is generally uncurable, the goal of surgical treatment is to provide the patient with good function with as few complications as possible. In this article, we discuss the clinical manifestation of

Calcific aortic valve stenosis - comparison of inflammatory lesions in the left, right, and non-coronary cusp.

BACKGROUND: Calcific aortic valve stenosis (CAVS) is the most frequent acquired heart valve disease in the developed world and the most common cause of heart valve replacement, particularly in older adults. It is considered a form of atherosclerosis and, like the latter, of inflammatory pathogenesis. METHODS: The incidence and severity of features of chronic inflammation (vascularization, cellular infiltration, bone metaplasia, calcification) in surgically resected semilunar cusps of a tricuspid aortic valve in 100 patients with CAVS were assessed. A novel method of placing metal clips during
