Management and outcome of fetomaternal Rh alloimmunization in twin pregnancies.

BACKGROUND: A study indicates that elevated aldosterone levels increase the risk of hypertension by 16%. This retrospective cohort study examines the relationship between blood pressure and renin-aldosterone levels in patients with essential hypertension (EH), aiming to elucidate the clinical features and risk factors of inappropriate hyperaldosteronism in Chinese individuals with essential hypertension. METHODS: Clinical data of 309 EH patients were analyzed. The participants were divided into three groups with a high (H)/medium (M)/low (L) blood pressure level: Group H (BP≥160 / (or) 100 mm…

Endocrine mechanisms in congestive cardiac failure. Renin, aldosterone and atrial natriuretic hormone.

Overactivity of the renin-angiotensin-aldosterone system occurs in the syndrome of congestive cardiac failure. Aldosterone overactivity is crucially involved in maintaining the oedematous state as evidenced by its often complete correction by adrenalectomy, or by aldosterone antagonists, in both experimental and clinical heart failure. The hyperaldosteronism of heart failure can also be attacked by angiotensin-converting enzyme (ACE) inhibition, which not only blocks the angiotensin drive to aldosterone, but also unloads the heart by blocking renin-angiotensin-mediated vasoconstriction. Accord…

Inverse association between the extracellular matrix protein Tenascin-X and concurrent abdominal aortic calcification in patients with primary aldosteronism.

BACKGROUND: Patients diagnosed with primary aldosteronism (PA) are at significantly increased risk of abdominal aortic calcification (AAC) and subsequent cardiovascular complications. Notably, abdominal aortic calcification is independently associated with adverse cardiovascular outcomes. However, to date, no serum proteomic profiling has been specially carried out in PA patients complicated with AAC, leaving an unaddressed research gap. This study was designed to identify serum differentially expressed proteins (DEPs) via DIA-based untargeted proteomics. METHODS: In this translational resear

From low renin to the primary aldosteronism spectrum: aldosterone synthase inhibitors and the endocrine management of hypertension.

BACKGROUND: Hypertension remains a major contributor to global cardiovascular morbidity despite decades of available therapy. AIM: This review advances an endocrinological framework in which chronic hypertension, including resistant hypertension, is understood as a consequence of dysregulated hormonal control of renal sodium handling, mediated by excess renin-angiotensin II activity, inappropriate aldosterone excess, or both. RESULTS: Within this framework, the traditional binary distinction between "essential" and "secondary" hypertension gives way to a mechanistic continuum characterized b

ACTH Stimulation and Oral Salt Loading Tests Detect Biochemical Outcome Early After Primary Aldosteronism Surgery.

BACKGROUND: In primary aldosteronism (PA), the biochemical outcomes of the Primary Aldosteronism Surgical Outcome study are used to assess aldosterone hypersecretion 6 to 12 months after surgery. However, few studies have investigated whether the outcomes can be predicted in the early postoperative period. In this retrospective study, we evaluated whether the adrenocorticotropin stimulation test (AST) and oral salt loading test (OST) performed immediately after surgery could predict biochemical outcomes 1 year after surgery. METHODS: We assessed 268 patients with PA who underwent adrenalectom

Adrenal venous sampling outcomes with and without mineralocorticoid receptor antagonists in primary aldosteronism.

BACKGROUND: Current guidelines recommend discontinuing mineralocorticoid receptor antagonists (MRA) at least 4 weeks before adrenal venous sampling (AVS) in primary aldosteronism (PA) to avoid interference with the lateralization index (LI). However, discontinuing MRA can cause hypertension and hypokalemia. This study aimed to evaluate the impact of continuing MRA use prior to AVS. METHODS: A 5-year retrospective cohort study of patients aged ≥ 18 with PA who underwent successful AVS was conducted. Patients were divided into two groups: those who continued MRA during AVS and those who discont

Development of a prediction model by combining tumor diameter and clinical parameters of adrenal incidentaloma.

When adrenal incidentalomas are detected, diagnostic procedures are complicated by the need for endocrine-stimulating tests and imaging using various modalities to evaluate whether the tumor is a hormone-producing adrenal tumor. This study aimed to develop a machine-learning-based clinical model that combines computed tomography (CT) imaging and clinical parameters for adrenal tumor classification. This was a retrospective cohort study involving 162 patients who underwent hormone testing for adrenal incidentalomas at our institution. Nominal logistic regression analysis was used to identify th

Clinical characteristics of primary aldosteronism screened from Chinese patients with hypertension: The China primary aldosteronism prospective study.

We investigated the clinical characteristics of primary aldosteronism (PA) screened from patients with hypertension in China. The participants were hypertensive patients who were suspected of PA and registered in the China Primary Aldosteronism Prospective Study. Plasma aldosterone-to-renin ratio (ARR) was used as the screening test. In patients screened positive for PA, that is, an ARR exceeding the thresholds and plasma aldosterone concentration (PAC) > 100 pg/mL, a confirmatory test was performed for diagnosis. Patients with PA underwent a CT scan and adrenal venous sampling for subtyping.

Double somatic mutations in CTNNB1 and GNA11 in an aldosterone-producing adenoma.

Double somatic mutations in CTNNB1 and GNA11/Q have recently been identified in a small subset of aldosterone-producing adenomas (APAs). As a possible pathogenesis of APA due to these mutations, an association with pregnancy, menopause, or puberty has been proposed. However, because of its rarity, characteristics of APA with these mutations have not been well characterized. A 46-year-old Japanese woman presented with hypertension and hypokalemia. She had two pregnancies in the past but had no history of pregnancy-induced hypertension. She had regular menstrual cycle at presentation and was dia

Primary Aldosteronism: Spatial Multiomics Mapping of Genotype-Dependent Heterogeneity and Tumor Expansion of Aldosterone-Producing Adenomas.

BACKGROUND: Primary aldosteronism is frequently caused by an adrenocortical aldosterone-producing adenoma (APA) carrying a somatic mutation that drives aldosterone overproduction. APAs with a mutation in KCNJ5 (APA-KCNJ5MUT) are characterized by heterogeneous CYP11B2 (aldosterone synthase) expression, a particular cellular composition and larger tumor diameter than those with wild-type KCNJ5 (APA-KCNJ5WT). We exploited these differences to decipher the roles of transcriptome and metabolome reprogramming in tumor pathogenesis. METHODS: Consecutive adrenal cryosections (7 APAs and 7 paired adja

Relationship between vascular ageing and left ventricular geometry in patients with newly diagnosed primary aldosteronism.

BACKGROUND: Changes in left ventricular (LV) geometry are early manifestations of cardiac damage. The relationship between vascular aging and LV geometry has been reported. However, in newly diagnosed primary aldosteronism (PA), with more severe target organ damage than essential hypertension, the relationship between vascular aging and LV geometry has never been described. METHODS: We conducted a retrospective study among newly diagnosed PA from 1 January 2017 to 30 September 2021 at the Third Xiangya Hospital. The data of vascular aging parameters were collected, including ankle-brachial in

CXCR4-directed [68Ga]Ga-PentixaFor PET/CT versus adrenal vein sampling performance: a study protocol for a randomised two-step controlled diagnoStic Trial Ultimately comparing hypertenSion outcome in primary aldosteronism (CASTUS).

INTRODUCTION: Primary aldosteronism (PA) is the most common form of secondary hypertension. It is caused by overproduction of aldosterone by either a unilateral aldosterone-producing adenoma (APA) or by bilateral adrenal hyperplasia (BAH). Distinction is crucial, because PA is cured by adrenalectomy in APA and is treated by mineralocorticoid receptor antagonists in BAH. The distinction is currently made by adrenal vein sampling (AVS). AVS is a costly, invasive and complex technical procedure with limited availability and is not superior in terms of outcomes to CT scan-based diagnosis. Thus, th

Changes of the CYP11B2 Expressing Zona Glomerulosa in Human Adrenals From Birth to 40 Years of Age.

BACKGROUND: Aldosterone synthase (CYP11B2) antibodies for immunohistochemistry, enables to visualize aldosterone-producing zona glomerulosa (ZG), aldosterone-producing micronodules, and aldosterone-producing adenomas. The architecture of the ZG differs in old versus young age but the evolution of the changes is not well known. The pathogenesis of aldosterone-producing micronodules and aldosterone-producing adenomas is still unclear and research on the ZG in young populations is limited. In this study, we elucidate changes in human ZG with age by quantifying the CYP11B2 expression. METHODS: We

Variability of Aldosterone Measurements During Adrenal Venous Sampling for Primary Aldosteronism.

BACKGROUND: Variability of aldosterone concentrations has been described in patients with primary aldosteronism. METHODS: We performed a retrospective cohort study of 340 patients with primary aldosteronism who underwent adrenal venous sampling (AVS) at a tertiary referral center, 116 of whom also had a peripheral venous aldosterone measured hours before the procedure. AVS was performed by the same interventional radiologist using bilateral, simultaneous sampling, under unstimulated and then stimulated conditions, and each sample was obtained in triplicate. Main outcome measures were: (i) cha

Adrenal Venous Sampling in Young Patients with Primary Aldosteronism. Extravagance or Irreplaceable?

CONTEXT: Current clinical guidelines suggest that adrenal venous sampling (AVS) may not be mandatory in young patients with primary aldosteronism (PA) and a solitary adrenal adenoma on imaging. OBJECTIVE: The aim of this study was to further elucidate whether conventional imaging alone is sufficient to distinguish unilateral from bilateral PA among patients aged 40 years or younger. METHODS: This was a retrospective study where data from 45 patients with PA, aged between 26 and 40 years, who underwent successful AVS between 2005 and 2019, were analyzed. Results concerning laterality on imagi
