Community-based conservation reduces sexual risk factors for HIV among men.

BACKGROUND: Despite numerous programs to combat the global HIV and AIDS pandemic, infection rates remain high, especially in sub-Saharan Africa, where two-thirds of all people living with HIV reside. Here, we describe how we used rigorous program evaluation methods to assess the effectiveness of a community-based natural resource management program that "mainstreamed" HIV awareness and prevention activities within rural communities in Namibia. FINDINGS: We used data from two rounds of the Namibia Demographic and Health Surveys (2000 and 2006/2007) and quasi-experimental statistical methods to

Methylenetetrahydrofolate reductase C677T polymorphism and susceptibility to cervical cancer and cervical intraepithelial neoplasia: a meta-analysis.

BACKGROUND: A number of studies have explored the association between methyl enetetrahydrofolate reductase (MTHFR) C677T polymorphism and susceptibility to cervical cancer and cervical intraepithelial neoplasia (CIN). However, results remained controversial. To address this gap, we decided to conduct a meta-analysis of all available published studies. METHODS: Electronic literature searches of the PubMed, EmBase and Medline databases were performed up to April 30, 2012. Fixed-effects or random-effects model was used to calculate the pooled ORs for different genetic models. RESULTS: A total o

Application of the Central Limit Theorem in microbial risk assessment: high number of servings reduces the Coefficient of Variation of food-borne burden-of-illness.

The Central Limit Theorem (CLT) is proposed as a means of understanding microbial risk in foods from a Public Health perspective. One variant of the CLT states that as the number of random variables, each with a finite mean and variance, increases (→∞), the distribution of the sum (or mean) of those variables approximates a normal distribution. On the basis of the CLT, the hypothesis introduced by this paper states that the Coefficient of Variation (CV) of the annual number of food-borne illness cases decreases as a result of a larger number of exposures (or servings) (n). Second-order Monte-C

The influence of atorvastatin on tendon healing: an experimental study on rabbits.

Hyperlipidemia is a major risk factor for coronary heart disease. The most commonly used antihyperlipidemic drugs are 3-hydroxy-3-methylglutaryl coenzyme A (HMG CoA) reductase inhibitors (statins), of which atorvastatin is one of the most widely used. Little is known about the relationship between tendinopathy and HMG CoA reductase inhibitors (statins) or the effects of atorvastatin use on tendon healing following surgical repair of tendon rupture. We hypothesized that atorvastatin negatively affects this healing process. The Achilles tendons of 16 New Zealand rabbits were ruptured surgically

Panoramic radiographic findings as predictors of mandibular nerve exposure following third molar extraction: digital versus conventional radiographic techniques.

OBJECTIVE: The aim was to compare digital and conventional panoramic imaging techniques for identifying high-risk radiographic markers associated with mandibular nerve (MN) injury after mandibular third molar (M3) removal. STUDY DESIGN: The study used a retrospective cohort model. The predictor variable was the presence or absence of radiographic signs associated with MN exposure during M3 removal. The outcome variable was MN exposure. Intraexaminer variability was estimated using a kappa statistic. Logistic regression modeling was used to measure the association between radiographic signs an

The ANTICIPATE-NASH Models Stratify Better the Risk of Clinical Events Than Histology in Metabolic Dysfunction-Associated Steatotic Liver Disease Patients With Advanced Chronic Liver Disease.

BACKGROUND & AIMS: The reference for risk stratification and clinical trial selection of metabolic dysfunction-associated steatotic liver disease (MASLD) patients is fibrosis degree by histology. The noninvasive ANTICIPATE-NASH models have been validated for risk prediction of clinically significant portal hypertension (CSPH) and liver-related events (LRE). We assessed whether these models provide better risk stratification of events than histology. METHODS: A multicenter cohort 1, including 699 biopsy specimen-proven F3-F4 patients with MASLD was evaluated. The end point was LRE (hepatic dec

Simple Prediction Model for Colorectal Serrated Polyps: Development and External Validation Study in U.S. Prospective Cohorts.

UNLABELLED: Serrated polyps (SP) are precursors for colorectal cancer and contribute disproportionately to postcolonoscopy cancers. Leveraging three U.S. cohorts (43,974 women and 5,322 men), we developed prediction models for high-risk SPs (sized ≥10 mm or ≥3) among individuals undergoing their first colonoscopy screening. We then validated the model in the Partners Colonoscopy Cohort (51,203 women and 39,077 men). We evaluated discrimination and calibration using the C-statistic and Hosmer-Lemeshow test, respectively. The age and family history model generated a C-statistic [95% confidence i

Fracton Self-Statistics.

Fracton order describes novel quantum phases of matter that host quasiparticles with restricted mobility and, thus, lies beyond the existing paradigm of topological order. In particular, excitations that cannot move without creating multiple excitations are called fractons. Here, we address a fundamental open question-can the notion of self-exchange statistics be naturally defined for fractons, given their complete immobility as isolated excitations? Surprisingly, we demonstrate how fractons can be exchanged and show that their self-statistics is a key part of the characterization of fracton o

Single patient learning for adaptive radiotherapy dose prediction.

BACKGROUND: Throughout a patient's course of radiation therapy, maintaining accuracy of their initial treatment plan over time is challenging due to anatomical changes-for example, stemming from patient weight loss or tumor shrinkage. Online adaptation of their RT plan to these changes is crucial, but hindered by manual and time-consuming processes. While deep learning (DL) based solutions have shown promise in streamlining adaptive radiation therapy (ART) workflows, they often require large and extensive datasets to train population-based models. PURPOSE: This study extends our prior researc

Are Common Sense Model constructs and self-efficacy simultaneously correlated with self-management behaviors and health outcomes: A systematic review.

This systematic review answered two questions among adults with chronic conditions: When included in the same statistical model, are Common Sense Model constructs and self-efficacy both associated with (1) self-management behaviors and (2) health outcomes? We followed the Preferred Reporting Items for Systematic Reviews and Meta-Analyses guidelines and included 29 articles. When included in the same statistical model, Common Sense Model constructs and self-efficacy were both correlated with outcomes. Self-efficacy was more consistently associated with self-management behaviors, and Common Sens

Comparison of Methods for Sensitivity Analysis of Heterogeneous Treatment Effects in Observational Studies and Application to Alzheimer's Disease and Cognitive Decline.

In Alzheimer's disease (AD) research, many observational studies have shown that the effect of sleeping quality, a modifiable risk factor, on cognitive decline is heterogeneous, where some adults experience faster rates of cognitive decline compared to others. However, these effects are likely confounded by unmeasured confounders, and the sensitivity of these effects to unmeasured confounders may be heterogeneous, where one subgroup's treatment effect is more sensitive than that of another subgroup. Unfortunately, compared to the overall treatment effect, there are limited investigations about

Randomization-Based Covariance Analysis for Confidence Intervals of Treatment Comparisons Based on Restricted Mean Survival Time With Categorized Time-to-Event Data.

This paper introduces a randomization-based method for covariate-adjusted comparisons of restricted mean survival time (RMST) between treatment arms in randomized controlled trials. Existing approaches for covariate-adjusted RMST analysis have model-based assumptions that may not be compatible with the complexity of survival data. We estimate the treatment difference in RMST using randomization-based analysis of covariance (RB-ANCOVA) for categorized time-to-event data by constraining the covariate mean differences between treatment groups to zero. Accordingly, we provide corresponding confide

Integrative learning of individualized treatment rules from multiple studies with partially overlapping treatments.

An individualized treatment rule (ITR) tailors treatments to a patient's specific characteristics. However, randomized controlled trials (RCTs) are often underpowered to detect the treatment effect heterogeneity needed for reliable ITR estimation. To address this limitation, there is growing interest in leveraging information from multiple studies to improve statistical power and support individualized decision-making. A key challenge in this context is that available RCTs may not evaluate the same set of treatments. In this paper, we propose an integrative learning framework that synthesizes

The DSM-5 Alternative Model of Personality Disorder's Coverage of Borderline Personality Concepts.

The clinical utility of the DSM-5's Alternative Model of Personality Disorders (AMPD) depends on how well it encodes diagnostic concepts recognized in clinical descriptions of PDs. The present study examined the AMPD's coverage of borderline personality features identified in the clinical literature. In three independent samples (total N = 1166), community adults and university students rated their borderline features, level of personality functioning (AMPD Criterion A), and maladaptive personality traits (AMPD Criterion B). We observed that, collectively, the AMPD dimensions accounted for 76%

Estimating intervention impacts when timing is unclear: an AR-LagDT model with distributed lags.

Interrupted Time Series (ITS) analysis is widely used to evaluate intervention impacts, but conventional methods like the classical segmented regression (CSR) and "prediction" models (ARIMA, ETS, etc.) have limitations. CSR assumes an immediate effect, while "prediction" models rely on counterfactual estimates that become unreliable over time. To address this, we propose the Autoregressive with Lag Distributed Term (AR-LagDT) model, which explicitly accounts for intervention effect ambiguity using distributed lag functions. By incorporating varying fuzzy statuses (the onset and duration ambigu

Clustering longitudinal data: comparison of model-based and distance-based approaches using simulated and real-world data in psychiatric research.

BACKGROUND: Clustering longitudinal symptom trajectories is increasingly used to characterise clinical heterogeneity in psychiatric disorders. However, the relative performance of distance-based and model-based approaches remains insufficiently studied in psychiatric settings, where limited follow-up waves, measurement variability, and heterogeneous patient responses may affect clustering reliability. We aimed to systematically compare distance-based and model-based (Latent Class Mixed Models, LCMMs) methods, evaluating their performance in recovering the number of groups, classification accur

Developing Dynamic Prediction Methods for Survival Time Lost in Chronic Kidney Disease Progression Under Competing Risks.

Patients with chronic kidney disease (CKD) may progress to end-stage renal disease (ESRD) or die from other causes during long-term follow-up, making it essential to properly account for competing risks in prognostic modeling. However, most existing CKD prediction models rely on hazard-based measures, which primarily reflect relative effects, lack clinical interpretability, and cannot directly quantify survival time lost due to disease progression or death. To address this limitation, we adopted the restricted mean time lost (RMTL), an absolute and intuitive measure of survival time loss, and

Sustained control of PM2.5 concentrations in low concentration areas can improve life expectancy for patients with specific cardiovascular diseases.

A growing number of studies suggest that exposure to particulate matters with aerodynamic diameters < 2.5 μm (PM2.5) below air quality standards still increases the risk of death from cardiovascular disease (CVD), and the risk of death from ischemic heart disease (IHD) and stroke, the largest cause of CVD death. The aim of this study was to investigate the effect of PM2.5 exposure at lower levels on years of life lost (YLL) and potential life expectancy benefits for IHD and stroke. We investigated the relationship between daily PM2.5 and YLL for IHD and stroke using a generalized additive mode

Bayesian hierarchical mixture modelling to derive probabilistic iELISA thresholds for bovine brucellosis in endemic dairy systems.

BACKGROUND: In endemic dairy systems, the interpretation of serological tests for bovine brucellosis is compromised using fixed diagnostic cut-offs, which fail to account for continuous antibody distributions and population heterogeneity. This study aimed to apply a Bayesian hierarchical Gaussian mixture model (BHGMM) to resolve diagnostic uncertainty by deriving probabilistic, biologically informed thresholds for indirect ELISA (iELISA). METHODS: A cross-sectional dataset comprising 2,696 milk samples from large-scale dairy herds was analysed. Log-transformed and standardised antibody values
