Restorative and regenerative therapies for vision loss.

Reversing blindness and restoring functional vision remains a central challenge in biomedical engineering. Advances in ocular prosthetics have enabled limited recovery of light perception, motion detection, and coarse object recognition through anterior segment devices such as keratoprostheses and through posterior segment interfaces including retinal, suprachoroidal, optic nerve, and cortical implants. Parallel progress in organ-on-a-chip systems and retinal organoids is providing increasingly sophisticated platforms for disease modelling and therapeutic development. Despite these advances, i

Light-induced remodeling of the Drosophila phosphoproteome regulates visual adaptation through the Trp channel.

Light is an important environmental stimulus that is necessary for vision and circadian entrainment. Here, we present a system-wide analysis of the kinase-mediated light response in Drosophila using optimized data-independent acquisition mass spectrometry (DIA-MS) for quantitative phosphoproteomics analysis. Exposure to standard white light or high-intensity blue light induced a widespread increase in phosphorylation relative to dark-adapted flies. The light-sensitive phosphoproteins were part of multiple pathways, including phototransduction, circadian control, lipid metabolism, and synapse o

Bleaching of mouse rods: microspectrophotometry and suction-electrode recording.

When a substantial fraction of rhodopsin in a rod photoreceptor is exposed to bright light, the rod is desensitized by a process known as bleaching adaptation. Experiments on isolated photoreceptors in amphibians have revealed many of the features of bleaching adaptation, but such experiments have not so far been possible in mammals. We now describe a method for making microspectrophotometric measurements of pigment concentration and suction-electrode recording of electrical responses over a wide range of bleaching exposures from isolated mouse rods or pieces of mouse retina. We show that if p

Role of ocular aberrations in dynamic accommodation control.

BACKGROUND: Accommodation control is mediated by a number of cues, including blur, chromatic aberration and target proximity. Data from wavefront measurements have shown clear shifts in ocular aberrations during increasing accommodative demand, most notably a negative shift in spherical aberration. Work in adaptive optics, where aberrations have been corrected, has suggested a role for aberrations in the control of accommodation for some individuals. This study aimed to determine the relative effects of aberration correction and inversion on closed-loop stepwise accommodation responses to small

Intermediate spatial frequency letter contrast sensitivity: its relation to visual resolution before and during amblyopia treatment.

We examined the loss of letter contrast sensitivity (LCS) measured using the Pelli-Robson chart, and the extent to which any such loss was modulated by spectacle wear and occlusion therapy in children participating in an amblyopia treatment trial. Their initial mean interocular difference in logMAR acuity was approximately three times that of their LCS (0.45 vs 0.14 log units). Log LCS was weakly though significantly correlated with logMAR visual acuity (VA) for all VAs better than 0.90 (r = -0.19, 95% CI: -0.28 to -0.10) whereas for all VAs of 0.90 or poorer, log LCS was markedly and signific

Pupil location under mesopic, photopic, and pharmacologically dilated conditions.

PURPOSE: To determine whether there are systematic changes in pupil location with changes in the state of pupil size and with other ocular variables. METHODS: High-resolution images of the pupil of the eyes of 70 subjects were taken using an infrared-sensitive camera. Images were obtained under mesopic, photopic, and pharmacologically dilated conditions. From the images, the center and diameter of the corneal limbus and the pupil were computed. In addition, the location of the first Purkinje image was calculated. RESULTS: The pupil center shifted consistently temporally as the pupil dilated.

Electrophysiological evaluation of the macular cone adaptation: VEP after photostress. A review.

In the present review, the methodologies and clinical applications of the visual evoked potentials (VEPs) after photostress, will be described. Photostress induces transient VEP changes consisting of an increase in response latency and a decrease in amplitude. When serial VEP recordings are obtained at discrete time intervals (i.e., every 20 s) after bleaching, the recovery of VEP waveform can be evaluated. The time needed for the VEP to recover to the pre-bleach, baseline status (recovery time after photostress) ranges in normal subjects between 68 and 78 s. Patients with different pathologie

A randomized comparison of prism adaptation and augmented surgery in the surgical management of esotropia associated with hypermetropia: one-year surgical outcomes.

OBJECTIVE: To compare motor and sensory 1-year surgical outcomes in patients with hypermetropic esotropia, managed with either augmented surgery based on the average of the near deviation with and without correction or preoperative prism adaptation. METHODS: Forty-three patients with hypermetropic esotropia without distance-near disparity entered a randomized prospective evaluation of augmented surgery (group A, 27 patients) versus prism adaptation (group P, 16 patients). The formula for augmenting the amount of the rectus muscle recession was based on the average of the near deviation with a

The visibility of 350 degrees C black-body radiation by the shrimp Rimicaris exoculata and man.

The eye of the 'eyeless' shrimp Rimicaris exoculata is unusual in having no image-forming optics and a high concentration of rhodopsin. The shrimps swarm around 350 degrees C hydrothermal 'black smoker' vents in the Mid-Atlantic Ridge. There is no other known source of visible light in the shrimp's environment. The spectral sensitivity of rhodopsin is well matched to typical spectra of bioluminescence of organisms found at lesser depths, but other animals detect such emissions without the unusual features of the R. exoculata eye. These two features are most easily understood as an adaptation f

A note on the action spectrum of human rod vision.

A template representing the spectral distribution of the absorption coefficients of human rhodopsin was fitted to each of 59 individual action spectra of human rod vision (from one of three populations) by an optimization routine. Curve-fitting parameters included peak wavenumber, optical density at this wavenumber and (for those from the population neither aphakic nor constrained to ages where the standard lens transmissivity curve is supposed valid), density of the latter at the wavenumber of peak lens absorption. The average peak wavenumber of each population differed significantly from tha

Effects of a visual-vestibular stimulus on the vestibulo-ocular reflex.

Exposure before flight to an environment that provides altered visual-vestibular relationships similar to those seen in weightlessness might facilitate adaptation to spaceflight. Fifteen normal subjects were exposed for 0, 5, and 20 min to an abnormal visual-vestibular environment in which the vestibular sensation of lateral tilt was associated with the visual impression of lateral translation. Lateral eye movement elicited by similar tilt in darkness was measured before and at intervals after the exposure. Of the 15 subjects, 12 showed increased VOR amplitude following the exposure, but this

Gyrate atrophy of the choroid and retina: improved visual function following reduction of plasma ornithine by diet.

In a patient with gyrate atrophy of the choroid and retina, an arginine-deficient diet has reduced plasma ornithine concentration fivefold during the past 20 months. Subjective improvement in her visual function was noted approximately 15 months after institution of her diet. This has been documented by improvements in the electroretinogram, dark-adaptation, and color vision. The improvement involves rod and, to a lesser extent, cone function. The results, although preliminary and limited to a single patient, suggest that reduction of plasma ornithine with a low arginine diet is beneficial in

Pathogenesis and Manifestations of Zika Virus-Associated Ocular Diseases.

Zika virus (ZIKV) is mosquito-borne flavivirus that caused a significant public health concern in French Polynesia and South America. The two major complications that gained the most media attention during the ZIKV outbreak were Guillain-Barré syndrome (GBS) and microcephaly in newborn infants. The two modes of ZIKV transmission are the vector-borne and non-vector borne modes of transmission. Aedes aegypti and Aedes albopictus are the most important vectors of ZIKV. ZIKV binds to surface receptors on permissive cells that support infection and replication, such as neural progenitor cells, dend

Random Copolyester-Based Delivery Systems for Tear Protein Therapeutics in Ocular Surface Disorders.

This study presents a polymeric drug delivery platform designed to address the limitations of conventional therapies for ocular surface and corneal disorders, such as dry eye disease (DED). Conventional treatments typically require frequent administration of eye drops, leading to poor bioavailability and patient adherence. We investigated the use of two random poly-(butylene succinate/diglycolate) copolymers, namely, P-(BS x BDG y ) at varying molar ratios (x, y) as vehicles for conjunctival drug delivery. Copolymers were synthesized and extensively characterized by their molecular structure,

One-stop strabismus digital diagnosis via AI-integrated skin-like and wearable "Eyelectronics".

Strabismus, affecting ~4% of children, impairs vision and psychosocial health. However, clinical diagnosis requires multiple instruments and stepwise examinations of ocular alignment, extraocular muscle function, and deviation angle. It is limited by low diagnostic objectivity, poor pediatric compliance, and high cost. Here, we propose a strategy for one-stop strabismus digital diagnosis via artificial intelligence (AI)-integrated, skin-like, and wearable "Eyelectronics." The ultralightweight, imperceptible eye-wearable system features an ultrathin (~60 micrometers in thickness), breathable, a

Characterization of the ocular inflammatory response to AAV reveals divergence by sex and age.

Progress for ocular adeno-associated virus (AAV) gene therapy has been hindered by AAV-induced inflammation, limiting dose escalation and long-term efficacy. Broadly, the extent of inflammatory responses alters with age and sex, yet these factors are poorly represented in pre-clinical development of ocular AAV gene therapies. Here, we combined clinical imaging, flow cytometry, and bulk sequencing of sorted microglia to interrogate the longitudinal inflammatory response following intravitreal delivery of AAV2 in young (3-month-old), middle aged (9-month-old), and old (18-month-old) Cx3cr1-creER

Harnessing engineered adaptive nanoparticles to modulate macrophages for treating corneal alkali burns.

Corneal alkali burns represent a severe ophthalmic emergency characterized by progressive inflammation, impaired epithelial regeneration, and pathological neovascularization, posing significant clinical challenges. Conventional topical therapies are hindered by rapid precorneal clearance and limited corneal penetration. To overcome these limitations, we designed adaptive nanoparticles (PDNPs) formed by the self-assembly of polysialic acid (PSA)-dexamethasone (Dex) via acid-sensitive hydrazone linkages. Upon topical administration, PSA-mediated Siglec-E recognition enabled selective anchoring o

Update on Ophthalmic Implications of Highly Pathogenic Avian Influenza A (H5N1) Virus.

INTRODUCTION: Highly Pathogenic Avian Influenza (HPAI) A(H5N1) represents a growing public health challenge, given broadening zoonotic vectors, with a previously reported human mortality rate of roughly 50%. Late March 2024 marked the start of a new outbreak of HPAI A(H5N1) in the United States. While offering unique public health challenges, this outbreak also provides insight into clinical presentation and ocular involvement implications, transmission vectors, and the implementation of successful surveillance strategies. OBJECTIVES: This review seeks to highlight current and historical outb

Pediatric blepharokeratoconjunctivitis: A challenging ocular surface disease.

Pediatric blepharokeratoconjunctivitis (PBKC) is a chronic and recurrent ocular surface inflammatory disorder affecting children in early life. It is frequently under- or late- diagnosed, representing a potential cause of severe visual morbidity worldwide. An expert panel consensus recently agreed on its definition and proposed diagnostic criteria for suspected and definitive PBKC to reduce confusion and avoid varied terminology previously used in the literature, improving early and precise diagnosis. Previous evidence has pointed to the role of the adaptive immune system in recognizing and ha
