[Detection of human papillomavirus in the upper respiratory tract in children without recurrent respiratory papillomatosis].

OBJECTIVE: The purpose of this prospective study was to investigate the presence of human papillomavirus (HPV) in tonsillectomy and adenoidectomy specimens from pediatric patients without juvenile-onset recurrent respiratory papillomatosis (JORRP), so as to understand the effect of HPV infection in the upper respiratory tract in children. METHODS: Two hundred and forty-one pediatric patients without known JORRP or other HPV-related diseases undergoing tonsillectomy and/or adenoidectomy for hypertrophy or chronic tonsillitis were enrolled in this prospective study. One hundred and seventy-seve…

Anesthetic Management of a Pediatric Patient With Carnitine Deficiency: A Case Report.

Carnitine deficiency syndromes are a group of rare metabolic disorders characterized by impaired β-oxidation and increased reliance on glucose during metabolic stress. Published literature on anesthetic management in patients with congenital carnitine deficiency is limited. We report the case of a 10-year-old boy with carnitine deficiency undergoing bronchoscopy, tonsillectomy, and adenoidectomy. A nontriggering anesthetic was used due to concern for malignant hyperthermia risk with carnitine palmitoyltransferase deficiency; a diagnosis used interchangeably with primary carnitine deficiency fo…

[Reduction of overgrown lymphatic system among children as a method of restoration of upper airways].

It was concluded, that airway stenosis in the pharyngeal area is the most frequent cause of sleep apnea syndrome among children. The basis of research were children treated at The Clinic from 1999-2002 and it was stressed that the best way to cure is surgical therapy. Partial tonsillectomy or adenoidectomy is a stable method of dilatation of the airway. Proper treatment depends on correct recognition made by the laryngologist based on interview, clinical examination and pulsoxymetry examination. The less extensive procedures were stressed specially among children because of role of pharyngeal …

[Pathological changes in the middle ear and delayed speech development. Preventable sequelae of disordered tube function and seromucotympanum].

Serous otitis media is a frequent disorder, as 80-90% of children up to the age of 8 years experience at least one episode of middle ear effusion. When deciding on a therapeutic strategy, it should be taken into consideration that there is a spontaneous recovery rate of about 95%. Initially, conservative treatment should be given (Valsalva maneuver), as the efficacy of drug therapy is questionable. In our opinion, in cases of persistent middle ear effusion an adenoidectomy plus myringotomy should be performed. In cases of recurrence, a ventilation tube should be inserted to reduce eardrum path…

Management of tracheal lesions in Hurler syndrome.

Hurler syndrome is a rare mucopolysaccharide storage disease that becomes clinically apparent during early childhood as mucopolysaccharide deposits form in skeletal and soft tissues. Progressive mucopolysaccharide deposition in the oropharynx and tracheal connective tissues leads to airway obstruction if untreated. Tonsillectomy, adenoidectomy, and tracheostomy have been utilized to provide symptomatic relief of upper airway obstruction. Treatment of tracheal lesions by laser excision is a recent innovative method for the management of airway obstruction in children with Hurler syndrome. We de…

Cardiopulmonary changes secondary to chronic adenotonsillitis.

Thirty children with lower respiratory tract infection of varying degrees secondary to chronic tonsillitis and/or enlarged adenoids were studied clinically and roentgenographically. Symptoms and signs of alveolar hypoventilation and signs of cor pulmonale were present in two of them. These symptoms and signs improved markedly after adenoidectomy and tonsillectomy. The relationship of the lung changes to the incidence of cor pulmonale is discussed. The development of recurrent attacks of severe lower respiratory tract infection secondary to diseased tonsils and adenoids is believed to play an i…

Middle ear effusions -- complications of disease and treatment.

The results of treatment of secretory otitis media with commonly found disease in Finland is described. Treatment includes adenoidectomy and/or insertion of tympanostomy tubes with mastoidectomy in selected cases. Tympanostomy tube treatment results in an increased amount of visible tympanic membrane scar formation and tympanosclerosis, but these are of no major functional importance. In about 10 per cent of the glue ears a mastoid disease process complicates the disease. Here thorough mastoidectomy at an early stage is the treatment of choice. Cholesteatoma formation appears in about two per …

[The present methods of diagnosing adenoidal hypertrophy in children].

The role of enlarged adenoids and tonsils in upper airway obstruction has become well-established earlier, thus, adenoidectomy with, or without, tonsillectomy is one of the most commonly performed surgical procedures in the paediatric population. Since the difficulty to objectively assess the adenoidal tissue and the nasopharyngeal airway in children, it is necessary to find out an accurate method of measuring these structures pre-operatively. In this work, the following methods are described: recording of obstructive symptoms, transoral posterior rhinoscopic examination, radiological evaluati…

Obstructive sleep apnea syndrome in Thai children diagnosed by polysomnography.

Overnight polysomnography was conducted in 39 Thai children with clinically suspected obstructive sleep apnea syndrome (OSAS) during the years 1994 to 1996. Eighty-five percent of these children met the polysomnographic criteria of pediatric OSAS, 42.4% among whom had severe OSAS. Male : female ratio of children with OSAS was 4.5:1. The peak age at the time of diagnosis was 3 to 4 years. The most common predisposing factor was adenoidal and tonsillar hypertrophy. Adenoidectomy and/or tonsillectomy was the most effective therapeutic option. Recovery of symptoms was observed following surgery an…

A role for Waldeyer's ring in immunological response to allergens.

OBJECTIVE: Adenoids, tubal tonsil, palatine tonsil, and lingual tonsil are immunological organs included in the Waldeyer's ring, the basic function of which is the antibody production to common environmental antigens. Adenoidal hypertrophy (AH) is a major medical issue in children, and adenoidectomy is still the most used treatment worldwide. The response of adenoids to allergens is a good model to evaluate their immunological function. This report assessed the immunological changes in adenoid tissues from children with allergic rhinitis (AR) undergoing sublingual immunotherapy (SLIT). METHOD…
