Clinical prognostic factors in pediatric adrenocortical tumors: A meta-analysis.

Pediatric adrenocortical tumors (ACT) are rare and sometimes aggressive malignancies, but there is no consensus on the outcome predictors in children. A systematic search of MEDLINE, SCOPUS, Web of Science, and the Cochrane Library for studies from 1994 to 2020 about pediatric ACT was performed. In 42 studies, 1006 patients, aged 0-18 years, were included. The meta-analyses resulted in the following predictors of better outcome: age <4 years (P < .00001), nonsecreting tumors (P = .004), complete surgical resection (P < .00001), tumor volume (P < .0001), tumor weight (P < .00001), tumor maximum

[The Impact of Corticotherapy on the Diagnostic Yield of Stereotactic Biopsy in Patients with Brain Lymphoma].

UNLABELLED: Backround: Because of the dismal prognosis of untreated brain lymphoma early histological verification using stereobiopsy is decisive for patient with this disease. The study analysed the diagnostic yield of stereobiopsy in brain lymphoma patients with respect to prebiopsy corticosteroid administration. PATIENTS AND METHODS: Patients with brain lymphomas were identified in a group of 162 stereotactic biopsies (108 frame-based and 54 frameless) of patients harboring suspected brain tumor. Non conclusive biopsies were reevaluated to exclude the possibility of missed lymphoma. RESUL

Peripheral Precocious Puberty due to Functioning Adrenocortical Tumor: Description of Two Cases.

BACKGROUND: Adrenocortical tumors (ACTs) represent less than 0.2% of all childhood neoplasms. Frequent clinical manifestations are virilization, hypercortisolism, and peripheral precocious puberty (PPP). CASES: We describe two cases in which ACTs were responsible for virilization (case 1) and PPP (case 2) in prepubertal girls. In both cases an ACT diagnosis was made after 5-6 months from the first appearance of clinical signs. Surgery was performed within 1 month of diagnosis, and the benign nature of tumors was histologically confirmed. Despite complete tumor resection, virilizing features p

[Medical appetite stimulation for patients with advanced cancer].

For patients with advanced cancer, the evidence of using medical appetite stimulants is sparse. A literature database search for randomized, controlled trials found data from 11 drugs. Only for corticosteroids and progestins there was enough evidence to support their use for improving appetite and weight gain. Several drugs are currently being tested. If medical appetite stimulation commences, it is recommended to be part of a multimodal therapy for patients with advanced cancer, weight loss and lack of appetite.

Nonfunctioning adrenocorticalcarcinoma.

Clinically inapparent adrenal masses detected through imaging for nonadrenal disease, often referred to as adrenal incidentalomas, were first described approximately 20 years ago. However, their impact on health outcomes is now better appreciated and gaining broader attention. Despite the rarity of primary endocrine cancers of the adrenal, adrenal masses are one of the most prevalent of all human tumors. The prevalence of adrenal incidentaloma approaches 3% in middle age and increases to as much as 10% in the elderly. This report describes the case of a 30-year-old man who presented primarily

Virilising adrenocortical carcinoma.

A 16-year-old, Hindu, female presented with rapidly growing abdominal lump for 6 months, primary amenorrhoea and non-development of secondary sex characters. Her BP was 180/120 mmHg. There was an excessive hirsutism involving face, neck, shoulders, abdomen and thighs. A lump was felt at left lumbar region extending on to left hypochondrium and part of umbilical region. Her serum testosterone level was 224 ng/dl and cortisol level was 15 microg/dl. Ultrasonography revealed a solid mass arising from the upper pole of left kidney. Exploratory laparotomy revealed a huge left adrenal tumour which w

Dermatomyositis.

Dermatomyositis is one of the idiopathic inflammatory myopathies with characteristic cutaneous manifestations including the heliotrope rash, Gottron's papules, cuticular changes including periungual telangiectasia, a photodistributed erythema or poikiloderma, and a scaly alopecia. Dermatomyositis has been linked to cancer, particularly ovarian cancer. Cancer-associated disease is more commonly found in older patients, and when present, is associated with a poor prognosis. A childhood form of the disease exists and is frequently complicated by the development of calcinosis. Dermatomyositis is a

Aldosterone-producing adrenocortical adenoma in childhood: a case report.

Aldosterone-producing adrenal tumor is an exceptional cause of hypertension in childhood. The authors describe an 11-year-old girl with hypertension and lower limb weakness who had hyperaldosteronism and left adrenocortical adenoma.

Bilateral adrenal cortical adenomas in primary hyperaldosteronism.

Bilateral adrenal cortical adenomas in the presence of primary hyperaldosteronism is an extremely rare condition. We present a case of primary hyperaldosteronism in which a unilateral hypersecreting aldosterone-producing adenoma coexisted with a large, contralateral adrenal mass ultimately found to be consistent with cortical adenoma. Management consisted of total adrenalectomy and enucleation of adenoma from the opposite adrenal. The patient is normotensive 3 years after surgery. Enucleation as a successful approach to hyperfunctioning cortical adenomas is proposed.

Cystic prostatic disease associated with adrenocortical lesions in the ferret (Mustela putorius furo).

A review of case materials at the Armed Forces Institute of Pathology identified six cases of prostatitis in domestic ferrets (Mustela putorius furo). Five of these ferrets (83%) had one or more cysts and the remaining ferret had a paraprostatic cyst. Three of the six ferrets (50%) exhibited various degrees of prostatic squamous metaplasia. Inflammation ranged from subacute to chronic-active or pyogranulomatous. In addition to the prostatic lesions, hyperplastic and/or neoplastic adrenocortical lesions were present in 4/6 (66%) ferrets; 1/6 (17%) ferrets had previously been clinically diagnose

A case of weak mineralocorticoid-producing benign adrenal tumor.

Tumors producing weak mineralocorticoids are exceedingly rare and most of them predominantly produce 11-deoxycorticosterone. We present a case of adrenocortical adenoma in which the overproduction of 18-hydroxydeoxycorticosterone exceeded that of 11-deoxycorticosterone.

Inter- and intratumoral heterogeneity of aldosterone-producing adenomas: recent advances and clinical implications.

Primary aldosteronism is the most common form of secondary hypertension, and aldosterone-producing adenoma (APA) is the most prevalent surgically curable subtype of primary aldosteronism. Over the past decade, the identification of recurrent somatic mutations, most notably in KCNJ5, has transformed the molecular understanding of APA and has revealed substantial heterogeneity across tumors. In parallel, advances in transcriptomic, epigenomic, and metabolomic profiling have further expanded insights into genotype-dependent differences in steroidogenic activity, cellular morphology, and clinical

Adrenocortical Carcinoma with Suspected Dual Cortisol and Aldosterone Secretion: A Case Report.

Adrenocortical carcinoma (ACC) is a rare malignancy, accounting for 0.05-2% of all malignant tumors. Most cases are sporadic and asymptomatic, often discovered incidentally via imaging. However, 50-60% present with autonomous hormonal secretion, with 45% producing excess cortisol, and only 1% exhibiting mineralocorticoid excess. We report a 43-year-old Filipino female with a 6-month history of abdominal bloating, proximal muscle weakness, edema, and hypertension. Laboratory tests showed hypokalemia (serum potassium 2.5-3.05 mmol/L; normal 3.3-5.0 mmol/L). Imaging revealed a neoplastic right su
