Is memory loss without anatomical damage tantamount to a psychogenic deficit? The case of pure retrograde amnesia.

Following a car accident, a patient remained unconscious for approximately 20 min and confused for a few hours. When he could be questioned, he was found to have lost all past memories. The retrograde amnesia covered his whole life and concerned autobiographic events as well as famous facts and encyclopaedic knowledge. It also partially involved the verbal and visual lexicon. Reading, writing and counting were no longer possible. The profound impairment of retrograde memory contrasted with the preservation of anterograde memory, which permitted the patient to reacquire some of the notions he h

[Marchiafava-Bignami disease: interhemispheric disconnection, Balint syndrome, spontaneously favourable outcome].

We report a new case of Marchiafava-Bignami disease with favourable outcome. This case is particular on several scores. The interhemispheric disconnection syndrome was associated with Balint's syndrome, and this had apparently never been reported in this context previously. Despite the intensity and diffusion of white matter lesions in both hemispheres, and in the absence of vitamin treatment, the clinical and neuroradiological (CT, MRI) improvement occurred early and rapidly. This course, similar to that of toxic drug withdrawal, suggested that this was not a process of vitamin deficiency. Da

Pure dysgraphia with relative sparing of lower-case writing.

We describe the cognitive analysis of a patient with acquired pure dysgraphia. She presented a peculiar dissociation between lower- and upper-case handwriting: lower-case writing was relatively spared and showed a significant superiority of words versus nonwords. Upper-case writing and oral spelling did not show lexical effects, but were affected by item length. In all modalities errors consisted mainly of single graphemic substitutions, deletions, insertions and transpositions, resulting in legal or illegal nonwords, and showed a similar distribution across letter positions. These findings we

A population-based longitudinal study of childhood neurodevelopmental disorders, IQ and subsequent risk of psychotic experiences in adolescence.

BACKGROUND: Schizophrenia has a neurodevelopmental component to its origin, and may share overlapping pathogenic mechanisms with childhood neurodevelopmental disorders (NDs). Nevertheless, longitudinal studies of psychotic outcomes among individuals with NDs are limited. We report a population-based prospective study of six common childhood NDs, subsequent neurocognitive performance and the risk of psychotic experiences (PEs) in early adolescence. METHOD: PEs were assessed by semi-structured interviews at age 13 years. IQ and working memory were measured between ages 9 and 11 years. The prese

Pathologically confirmed corticobasal degeneration presenting with visuospatial dysfunction.

Corticobasal degeneration (CBD) typically manifests as progressive asymmetric rigidity and apraxia, although other non-motor presentations have been reported. We report two patients with pathologically diagnosed CBD who presented with prominent visuospatial dysfunction. The pathological changes were maximal in the visual association cortices, but absent in 31 cases of pathologically proven CBD with more typical antemortem features. Underlying CBD should be considered in the differential diagnosis of patients with findings reflecting posterior cerebral dysfunction.

Aphasia disorders outcome after stroke.

INTRODUCTION: Aphasia is considered to be the most difficult disorders of speech-language communication, and is often companion by all forms … ia had 11 patients (78.6%) which remained unchanged in form, while 3 (21.4%) evolved into an Alexia agraphia. Full recovery was noted in two patients (3.84%). Type of stroke did not affect the outcome of aphasia disorders. Out of the 52 analyzed patients after hospitalization, unfortunately, only 11 (21.2%) had some kind of speech pathology treatment after …

[Associative visual agnosia. The less visible consequences of a cerebral infarction].

After a cerebral infarction, some patients acutely demonstrate contralateral hemiplegia, or aphasia. Those are the obvious symptoms of a cer … d global alexia (i.e., impairment in letter recognition and profound impairment of reading) without agraphia. Naming was impaired on visual presentation (20%-39% correct), but improved significantly after tactile presentation (87% correct) or verbal definition (89%). Pre-semantic visual processing was normal (correct matching of different views of the same …

Can a writing intervention using mainstream Assistive Technology software compensate for dysgraphia and support reading comprehension for people with aphasia?

BACKGROUND: Stroke profoundly affects quality of life (QOL), including loss of employment, reduced social activity, shrinking social networks and low mood. Dysgraphia (impaired writing) is a common symptom of aphasia yet is rarely targeted in rehabilitation. Recent technological advances might challenge this, since much communication is now conducted digitally through writing. The rehabilitation of writing may therefore help to address the wider consequences of stroke and aphasia. AIMS: Can assistive technology (AT) training for people with dysgraphia: (1) improve written output, and are gain

Neurorehabilitation applied to specific learning disability: Study of a single case.

BACKGROUND: Specific Learning Disorders (SLD) therefore represent chronic, not temporary disorders with varying degrees of expression throughout life. The beginning of imaging, anatomy and genetics studies have made it possible to investigate the brain organization of individuals suffering from SLD (Deheane, 2009). OBJECTIVES: The purpose of this paper is to describe a treatment method for reading and writing disorders through an intervention based on the integration of a sublexical method and a neuropsychological approach, with assistive technologies in the study of a single case. METHODS:

Writing treatment for aphasia: a texting approach.

PURPOSE: Treatment studies have documented the therapeutic and functional value of lexical writing treatment for individuals with severe aphasia. The purpose of this study was to determine whether such retraining could be accomplished using the typing feature of a cellular telephone, with the ultimate goal of using text messaging for communication. METHOD: A 31-year-old man with persistent Broca's aphasia, severe apraxia of speech, global dysgraphia, and right hemiparesis participated in this study. Using a multiple baseline design, relearning and maintenance of single-word spellings (and ora

Word-centred neglect dyslexia: evidence from a new case.

Neglect dyslexia resulting from damage to word-centred representations is extremely rare. We report on a new case. A left-handed subject, SVE, presented with aphasia and neglect dyslexia/dysgraphia following a right hemisphere stroke. In tachistoscopic reading tasks, some of his errors resulted from retina-centred neglect, as he responded more accurately to words flashed in the left visual field than to words flashed in the right visual field. However, the critical aspects of his reading performance indicated word-centred neglect. SVE incorrectly produced the initial elements of four-letter wo

From letters to words: procedures for word recognition in letter-by-letter reading.

There are two alternative interpretations of how word recognition is accomplished in letter-by-letter reading. One postulates that it relies on the spelling system operating "in reverse," whereas the other claims that it is mediated by the reading system. Because of the close association between patterns of reading and spelling in previously reported cases of letter-by-letter reading, both hypotheses may be considered equally plausible. We now report a patient with letter-by-letter reading who demonstrated a striking dissociation between reading and spelling. Our observations in this patient a

Evaluation and outcome of aphasia in patients with severe closed head trauma.

In this study long-term observation of 12 patients with aphasia secondary to severe closed head trauma took place. The most frequent symptoms were amnestic aphasia and verbal paraphasia. Only one patient with a constant slow wave EEG focus in the dominant hemisphere had severe receptive symptoms. In all other patients the aphasia recovered rather well, though not totally, but the presence and degree of concomitant neuropsychological disorders were most important for the final outcome.

Amplitude Manipulation Evokes Upper Limb Freezing during Handwriting in Patients with Parkinson's Disease with Freezing of Gait.

BACKGROUND: Recent studies show that besides freezing of gait (FOG), many people with Parkinson's disease (PD) also suffer from freezing in the upper limbs (FOUL). Up to now, it is unclear which task constraints provoke and explain upper limb freezing. OBJECTIVE: To investigate whether upper limb freezing and other kinematic abnormalities during writing are provoked by (i) gradual changes in amplitude or by (ii) sustained amplitude generation in patients with and without freezing of gait. METHODS: Thirty-four patients with PD, including 17 with and 17 without FOG, performed a writing task on

[Neuropsychological aspects of disorders of higher functions of the cerebral cortex in children with speech disorders].

A complex and multiaspects study of the high cerebral function--speech, gnosis, praxis, gesture and mimics has been coducted on 98 children with proved speech defects--alalia, dyslalia, dysgraphia, dyslexia and stammering. In high percentage of the children various focal neurological signs have been established (so called soft neurological symptoms). Very high was the percentage of the children with visual-space defects. Rhythm gnosis and praxis suffered mostly in children with motor alalia. The tests for gesture and mimics prevailed. The unified mechanism of the disturbed development of the s
