Alcohol abuse and alcohol dependence are frequent disorders in the elderly. The disorders often develop as a response to burdensome life events which have to be treated specifically. Old alcoholics often respond well to age-specific interventions, if co-existing depressive symptoms are also treated. Because of the non-specific nature of the initial symptoms, the majority of the elderly alcoholics remain unrecognised during the diagnostic process. This review, therefore, gives a synopsis of clinical clues to alcoholism and associated psychic complications in elderly patients. Psychic complicati…

OBJECTIVE: To examine the startle reflex as a possible vulnerability marker among offspring of parents with anxiety disorders and/or alcoholism.
METHOD: The subjects were 66 male and female offspring (aged 10 to 17 years) of proband who participated in a family study of comorbidity of alcoholism and anxiety disorders. Testing consisted of examining the startle reflex and its modulation by prepulse stimuli (prepulse facilitation and prepulse inhibition).
RESULTS: Different components of the startle discriminated among children of parents with anxiety disorders, children of alcoholics, and chi…

The clinical and experimental investigations of alcoholic liver disease, have proved that hepatocytes injury is associated with immunologic abnormalities, which can be one of the sources of these injuries. The acetaldehyde, the main ethanol metabolite modify liver cytosolic proteins and form new antigens. Thus acetaldehyde induces the synthesis of auto-antibodies. Some hypotheses maintain the autoimmune pathogenesis of these diseases. Research was carried out on some patient groups admitted in the alcohol addiction department of the Neurology and Psychiatry hospital "Professor Gh. Marinescu".

BACKGROUND: Despite recognition of the high prevalence of alcoholism among patients with head and neck cancer, the prognostic importance of alcoholism has not been evaluated adequately. Previous investigators have speculated that alcoholic patients may have a poorer prognosis than nonalcoholic patients because of more advanced stage of cancer, the immunosuppressive effects of alcohol, and an increased rate of death due to other alcohol-related diseases.
PURPOSE: The goal of this population-based study was to identify the features of alcoholism that are associated with survival for patients wi…

OBJECTIVE: The relationship between drinking relapse and family history for alcoholism to cognitive functioning was investigated in alcoholics who completed a 28-day inpatient treatment program.
METHOD: The current study examined 144 primary alcoholics who completed a battery of cognitive tests on admission to treatment and at a 3-4 month follow-up visit. Participants were classified as either family history positive (FH+) (n = 86) or family history negative (FH-) (n = 58) based on information on alcohol dependence in first degree relatives. The subjects were further classified as drinkers (n…

The scientific discussion on the interrelationship of "alcoholism and psychosis" has been dominated by nosological problems for years. Alcohol hallucinosis, a comparatively rare psychosis in chronic alcoholism, can mimic the symptomatology of paranoid schizophrenia. The clinical features (vivid acoustic hallucinations and fear) are often inconclusive, as, unlike other organic psychoses, alcohol hallucinosis does not entail clouding of sensorium and disorientation. On the one hand follow-up studies suggest that alcohol hallucinosis in some cases can take a chronic schizophrenia-like course. Esp…

OBJECTIVE: As part of a search for measurable attributes of an individual that might be related to the risk of alcoholism, the author's group previously compared 227 sons of alcoholics and 227 matched comparison subjects at the age of about 20 years. Forty percent of the men at high risk for alcoholism and less than 10% of the comparison subjects demonstrated a low intensity of response to alcohol challenge. This article reports the results of the follow-up of the first half of this study group almost a decade later.
METHOD: Of the men who had been tested at about age 20, 223 were about age 3…

The tremor of chronic alcoholism, although clinically similar to essential tremor, has been considered a distinct syndrome. Its underlying mechanism was analyzed in five patients (none in the acute stages of alcohol withdrawal) hospitalized in an alcohol detoxification program. All five patients performed tracking tasks in which they pursued a linearly moving "target" light with a response light that they controlled by flexion-extension activity of the wrist. Stationary and dynamic targets were used with both isometric and unconstrained wrist mechanical interfaces. Frequency, torque, and displ…

The association between parental history of alcoholism and the nature of alcoholism was assessed using a more reliable measure of family history (Family Tree Questionnaire) and a more comprehensive inventory of alcoholism (Alcohol Use Inventory) than used in earlier studies. Parental alcoholism was associated with more severe alcoholism on most parameters of alcohol use (age of onset, quantity, frequency, preoccupation, and sustained use) and alcohol-related problems (social, vocational, physical, cognitive, and loss of control). The association between parental history of alcoholism and more …

Alcoholics who start abusing alcohol early in life have been found to exhibit problems with mood and aggression control more frequently than patients with a later onset of alcoholism. Because alcohol preference and consumption, as well as mood and aggression regulation, are believed to be influenced by serotonin, relationships between tryptophan availability and mood and aggression regulation were explored in alcoholics. When studied in the entire population, the ratio of tryptophan over other amino acids competing for brain entry (which influences brain serotonin) was found to be lowest one d…

The article surveys empirical studies on the relation between alcohol addiction and depression. For a better understanding and interpretation of the results of these empirical since the end of the 'sixties, the author presents first of all a brief historical abstract of the development of classification of depressive disorders and of the definition of alcoholism. In this article, the author restricts his comments to studies conducted since the end of the 'sixties, as self-rating scales or observers-rating scales or standardised interviews have been employed from that time for characterising th…

Alcoholism and epileptic seizures frequently coincide. Two groups can be distinguished clinically and by EEG. In one group (group I), seizures occur only during a period of alcohol withdrawal or partial withdrawal and can only be explained because of alcoholism in the history of the patient. In the other group (group II), seizures occur spontaneously, as well as during alcohol withdrawal, and epileptogenic factors (residual brain damage, former epileptic seizures, and a family history of seizures) probably play an important role in the manifestation of the seizures. Paroxysms or focal abnormal…

Intestinal permeability was investigated with a chromium-51-EDTA (edetic acid) absorption test in 36 non-intoxicated alcoholic patients without liver cirrhosis or overt clinical evidence of malabsorption or malnutrition. Patients abstaining from alcohol for less than 4 days almost invariably had higher intestinal permeability than controls, and in many the abnormality persisted for up to 2 weeks after cessation of drinking. The presence of gastritis did not correlate with the presence of increased permeability. The site of altered intestinal permeability was shown by an in-vitro permeability t

Alcohol may lead to central nervous system damage through direct toxic action (intoxication and withdrawal). In addition fundamental dietary insufficiencies in the alcoholic may lead to nutritional disease of the CNS mainly through thiamin deficiency (Wernicke Korsakow syndrome). This syndrome may be aggravated by treatment with intravenous carbohydrates. It has been speculated that the rare syndrome of central pontine myelinolysis may be caused by intravenous administration of fluids in the presence of increased secretion of ADH during withdrawal. The differential diagnosis of changes in cons

While elevated serum GGTP levels in alcoholism may be partly related to structural liver damage, other mechanisms, such as hepatic microsomal enzyme induction or alcoholic pancreatic damage, may also play a role.
