Technetium-99m HM-PAO single photon emission computed tomography imaging in transient global amnesia.

Five right-handed patients with pure transient global amnesia were evaluated with technetium-99m hexamethylpropyleneamine oxime single photon emission computed tomography after the amnesic attack. Independent of the interval between the procedure and the amnesic episode, single photon emission computed tomography demonstrated a decreased cerebral blood flow in the left temporal lobe in one patient and in the left parietotemporal region in three patients, with a more marked reduction in the temporal lobe. The remaining three patients were normal. A control single photon emission computed tomogr

Changes in heart rate variability over time from symptom onset of transient global amnesia.

Transient global amnesia (TGA) often involves precipitating events associated with changes in autonomic nervous system (ANS), and heart rate variability (HRV) reflects the ANS state. This study aimed to investigate HRV changes after TGA. A retrospective analysis of HRV included patients diagnosed with TGA between January 2015 and May 2020. The time and frequency domains of HRV were compared among three groups: early (< 1 week after TGA, n = 19), late (1-4 weeks after TGA, n = 38), and healthy control (HC, n = 19). The Pearson's correlation between time and time-domain HRV was also examined. Th

Positive Magnetic Resonance Imaging Evidence of Transient Global Amnesia Following the Use of Sildenafil.

Transient global amnesia (TGA) has been proposed as a possible adverse effect of sildenafil. There are rare cases in the literature, but none had strong evidence to support. Our case is the first to demonstrate a focal punctate diffusion-weighted imaging lesion at the right hippocampus after the use of sildenafil. This finding, which is suggestive of cytotoxic edema and is typical for TGA, may provide us evidence for the implication of sildenafil in TGA. We speculate that sildenafil may precipitate TGA by altering blood flow, inducing venous congestion or cortical spreading depression at the h

Power Spectral Differences between Transient Epileptic and Global Amnesia: An eLORETA Quantitative EEG Study.

Transient epileptic amnesia (TEA) is a rare epileptic condition, often confused with transient global amnesia (TGA). In a real-life scenario, differential diagnosis between these two conditions can be hard. In this study we use power spectral analysis empowered by exact Low Resolution Brain Electromagnetic Tomography (eLORETA) to evidence the differences between TEA and TGA. Fifteen patients affected by TEA (64.2 ± 5.2 y.o.; 11 female/4 male; 10 left and 5 right temporal epileptic focus) and 15 patients affected by TGA (65.8 ± 7.2 y.o.; 11 females/4 males) were retrospectively identified in ou

[A case of recurrent transient global amnesia showing different symptom duration and MRI findings].

A 66-year-old man was admitted to our department with anterograde amnesia. He was diagnosed with transient global amnesia (TGA) because of the symptom lasting for several hours and no abnormal findings on MRI and EEG. About a year after the episode, he recurred amnesia lasting only for 20 minutes. MRI diffusion weighted image (DWI) revealed a small hyperintense signal in the right hippocampus, while there was no abnormality on EEG. We diagnosed him with recurrent TGA. This case may be interesting in that symptom duration and MRI-DWI finding are much different between two attacks of TGA.

A case of recurrent transient global amnesia: don't forget the hippocampal punctuate diffusion restriction.

Transient global amnesia (TGA) is a syndrome characterized by reversible impairment of short-term memory. TGA itself is a benign disease, however, it is reasonable to investigate and exclude sinister causes of global amnesia; such as stroke or a seizure activity. A case of TGA is presented here with special emphasis on the typical TGA lesion that was detected on the patient's magnetic resonance imaging (MRI) of Brain. In patients with TGA, the typical MRI Brain finding is a tiny focus of diffusion restriction in the mesiotemporal lobe. This finding can potentially aid in early diagnosis and ma

The Still Enigmatic Syndrome of Transient Global Amnesia: Interactions Between Neurological and Psychopathological Factors.

Transient global amnesia (TGA) is a neurological syndrome that usually occurs in middle-aged or older people. It is characterized by the abrupt onset of profound anterograde amnesia, associated with more variable retrograde amnesia and repetitive questioning. The whole episode lasts no more than 24 h. Almost 60 years after its first descriptions, the etiology of TGA remains unknown. Until now, TGA has been described exclusively as a memory disorder, but there is a growing body of evidence to show that emotional and psychological factors (as anxious and depressive symptoms) are present at diffe

Cardiac troponin T elevation associated with transient global amnesia: another differential diagnosis of 'troponosis'.

Elevated cardiac troponin (cTn) levels can be detected in a variety of diseases with or without signs of myocardial ischaemia. Acute neurological disorders such as subarachnoid haemorrhage, stroke, transient ischaemic attack, epileptic seizures, and traumatic head injury can cause cTn elevation with and without myocardial wall motion abnormalities. We report a case of transient global amnesia with elevation and dynamic rise of highly sensitive troponin T (hsTnT) without clinical symptoms or signs of ischaemia. Cardiac work up was otherwise normal, with no signs of atherosclerotic coronary arte

Awareness of disease state without explicit knowledge of memory failure in transient global amnesia.

INTRODUCTION: Transient global amnesia (TGA) is a syndrome characterised by the rapid onset of antero- and retrograde amnesia, accompanied by temporal disorientation and iterative questioning. It is now established that the acute phase is associated with a raised level of anxiety and a depressed mood. We conducted a thorough investigation of patients' perceptions of their disease state, focusing on the links between their lack of explicit knowledge of amnesia during the acute phase and their emotional experience. METHOD: Explicit knowledge of memory deficits was assessed during TGA by means o

Focal lesions of human hippocampal CA1 neurons in transient global amnesia impair place memory.

A critical role in place learning has been attributed to place cells within the cornu ammonis 1 (CA1) sector of the hippocampus in rodents. The role of CA1 cells in the human hippocampus with regard to place learning remains elusive. Using a virtual Morris water maze, we investigated patients with acute transient global amnesia (TGA), a rare self-limiting dysfunction of the hippocampal system. Fourteen individuals with selective and focal lesions in the CA1 sector of the hippocampus showed a profound impairment in place learning. The size of the lesions and the duration of the TGA correlated w

Selective disruption of hippocampus-mediated recognition memory processes after episodes of transient global amnesia.

Transient global amnesia (TGA) is characterized by the abrupt onset of severe amnesia without concomitant focal neurological symptoms. Recent studies revealed that small and punctate MR-signal diffusion-weighted imaging (DWI) lesions can be found within the hippocampus of TGA patients during the post-acute phase. On the basis of dual-process models of recognition memory, the present study examined the hypothesis that hippocampal dysfunction as suggested by these DWI lesions disrupts hippocampus-mediated recollection in patients with TGA, whereas familiarity-based recognition memory that is ass

Clinical features, risk factors, and prognosis in transient global amnesia: a follow-up study.

We previously observed a high frequency of psychopathological features in transient global amnesia (TGA). We aimed at assessing differences in risk factor profile and prognosis between TGA and transient ischemic attack (TIA) patients with a focus on aspects with possible psychopathological relevance. We studied 51 TGA patients (mean age +/- SD, 62.7 +/- 6.7 years; M/F = 24/27) and 51 control patients with TIA (mean age +/- SD, 63.8 +/- 6.7 years; M/F = 41/10) and followed them up for about 7 years. Compared with TIA controls, TGA patients more frequently had a history of psychiatric diseases (

Cerebral blood flow SPET in transient global amnesia with automated ROI analysis by 3DSRT.

The aim of this study was to determine the areas involved in episodes of transient global amnesia (TGA) by calculation of cerebral blood flow (CBF) using 3DSRT, fully automated ROI analysis software which we recently developed. Technetium-99m L, L-ethyl cysteinate dimer single-photon emission tomography ((99m)Tc-ECD SPET) was performed during and after TGA attacks on eight patients (four men and four women; mean study interval, 34 days). The SPET images were anatomically standardized using SPM99 followed by quantification of 318 constant ROIs, grouped into 12 segments (callosomarginal, precent

Time course of focal slow wave activity in transient ischemic attacks and transient global amnesia as measured by magnetoencephalography.

In this longitudinal study multichannel MEG was used to localize and to quantify focal pathological spontaneous neuromagnetic activity in six patients with transient ischemic attacks (TIA) and two patients with transient global amnesia (TGA). Slow (2-6 Hz) and beta (14-30 Hz) activity were monitored up to 10 weeks. Results were compared with normative data, and changes over time were statistically analyzed. MEG detected pathological activity that persisted clinical symptoms. Focal slow activity originating from sensorimotor (TIA) and mesiotemporal (TGA) cortices exceeded normal values up to 14
