Tolerability of nimesulide and paracetamol in patients with NSAID-induced urticaria/angioedema.

Previous studies evaluated the tolerance of nimesulide and paracetamol in subjects with cutaneous, respiratory and anaphylactoid reactions induced by nonsteroidal anti-inflammatory drugs (NSAIDs). In this study we investigated tolerability and reliability of nimesulide and paracetamol in a very large number of patients with an exclusive well-documented history of NSAID-induced urticaria/angioedema. Furthermore, we evaluated whether some factors have the potential to increase the risk of reaction to paracetamol and nimesulide. A single-placebo-controlled oral challenge procedure with nimesulide…

Angioedema of the small bowel due to an angiotensin-converting enzyme inhibitor.

We describe a case of a 72-year-old woman who presented with two episodes of abdominal pain, vomiting, and diarrhea. Abdominal computed tomographic scans done during each episode demonstrated edema of the small bowel. Review of the patient's history revealed that she had been started on a treatment of lisinopril for hypertension 1 month before the first episode and had her prescribed dose increased 24 hours before each presentation. Angiotensin-converting enzyme (ACE) inhibitor-associated angioedema was suspected and the medication was discontinued. The patient has remained symptom-free while …

Angiotensin-converting enzyme inhibitor induced angioedema of the head and neck.

Angiotensin-converting enzyme (ACE) inhibitors are now in widespread use for the treatment of essential and renovascular hypertension. Consequently, angioedema, an uncommon complication of ACE inhibitor therapy is being encountered with increasing frequency. Since ACE inhibitor angioedema predominantly involves the face, lips, oral cavity, and occasionally the larynx the otolaryngologist is often consulted to evaluate affected patients. Treatment ranges from simple cessation of the drug to emergent airway intervention. The pertinent pharmacologic properties of ACE inhibitors and the pathophysi…

[Long-term prophylactic therapy combined with on-demand therapy for the control of hereditary angioedema--Comment on the 2021 edition of the international WAO/EAACI guideline for the management of hereditary angioedema].

The 2021 edition of the international World Allergy Organization (WAO)/European Academy of Allergy and Clinical Immunology (EAACI) guideline for the management of hereditary angioedema (HAE) is mainly based on high-quality randomized controlled trials. It provides clinical classification for HAE and offers graded recommendations for on-demand therapy, short-term prophylactic therapy, and long-term prophylactic therapy. Additionally, it provides management strategies for people with different HAE types. This article focused on the interpretation of short-term, long-term prophylactic therapy and…

[Prophylactic use of icatibant before tracheal intubation of a patient with hereditary angioedema type III. (A literature review of perioperative management of patients with hereditary angioedema type III)].

Type III hereditary angioedema is a rare familial disorder that has recently been described as a separate condition. Triggers for episodes of angioedema include surgery, dental procedures, and tracheal intubation maneuvers. Since episodes affecting the upper airway are potentially life-threatening, prophylactic treatment is recommended in these situations. The use of icatibant (Firazyr(®)), for prevention of angioedema prior to tracheal intubation, is reported in a patient with type iii hereditary angioedema. A literature review on the anesthetic management of this condition was conducted.

Labial Angioedema and Bacterial and Viral Infection Following Hyaluronic Acid Injection.

Hyaluronic acid (HA) currently maintains a favorable safety profile in the field of aesthetic medicine. However, severe acute hypersensitivity reactions may occur, clinically defined as angioedema. This condition typically presents immediately after HA injection, with marked swelling, pain, and evident tissue distress secondary to edema and local vascular physiological alterations. These events are more frequently observed in the lips, and although reported cases are scarce, most involve this anatomical location. The etiology of this uncommon complication is not fully understood. Proposed mech…

Endothelial barrier disorder in hereditary angioedema: molecular mechanisms and therapeutic implications.

Hereditary angioedema (HAE) is a rare genetic disorder characterized by recurrent episodes of vascular leakage and tissue swelling. Although excessive bradykinin generation has long been considered the central pathogenic mechanism, increasing evidence indicates that endothelial cells play a decisive role in determining when and where vascular permeability occurs. This Review summarizes recent advances in endothelial biology relevant to HAE, highlighting how intercellular junctions, the endothelial glycocalyx, and dynamic endothelial activation states cooperatively regulate barrier integrity. N…

Severe angioedema requiring airway intervention following evolocumab administration: a case report.

Evolocumab is a human monoclonal antibody that effectively reduces low-density lipoprotein (LDL) cholesterol levels by inhibiting proprotein convertase subtilisin-kexin type 9 (PCSK9). Although generally well tolerated, evolocumab may rarely lead to severe hypersensitivity reactions, including angioedema. To the best of our knowledge, severe angioedema requiring airway intervention has not been reported previously in the US population. A 64-year-old woman with hypertension, hypothyroidism, hyperlipidemia, and active tobacco use presented with acute chest pain and was diagnosed with ST-segment …

Hereditary angioedema plasma proteomics following specific plasma kallikrein inhibition with lanadelumab.

INTRODUCTION: Plasma proteomics analyses were performed to identify novel disease state biomarkers of hereditary angioedema due to C1 inhibitor deficiency (HAE-C1INH) and investigate the biological consequences of specific plasma kallikrein inhibition with lanadelumab. METHODS: Affinity proteomic analyses were performed using plasma from healthy controls (n=30) and patients with HAE-C1INH before (baseline, n=125) and after 6 months of treatment with lanadelumab (300 mg every 2 weeks, n=112) using the SomaScan platform. RESULTS: Relative plasma levels for several proteins differed significant…

Superior Vena Cava Syndrome Masquerading as Angioedema: A Chemotherapy Port Complication.

Superior vena cava (SVC) syndrome is a constellation of symptoms that occur secondary to external compression of the SVC, most commonly by a mediastinal malignancy. With the increased use of implanted cardiac devices and indwelling central venous catheters, SVC syndrome from a benign cause has become quite common. This report follows a 62-year-old female who was initially admitted to the critical care unit for treatment of angioedema without a history of malignancy but was found to have a surgically placed port used to treat her rheumatoid arthritis. Despite treatment of what was presumed to b…

Case Report: Post-operative Angioedema After a Laryngeal Mask Airway Application.

Angioedema with macroglossia is a rare complication of anesthesia. We present a clinical case of post-operative development of angioedema presenting as macroglossia in a patient receiving chronic therapy with lisinopril, who developed symptoms in the early post-operative period following surgery in a lateral position, when a laryngeal mask airway was used. Possible mechanisms of angioedema and macroglossia development in our patient are discussed along with potential underlying predisposing mechanisms and available therapeutic approaches.

Simultaneous acute pancreatitis and angioedema associated with angiotensin-converting enzyme inhibitor.

Angiotensin-converting enzyme inhibitors (ACEI) are commonly prescribed drugs for blood pressure (BP) control and renal protection. The use of ACEI is not associated with an increased risk of acute pancreatitis and ACEI-induced angioedema is rare. A 36-year-old woman presented with vomiting, headache, and aphasia. Her BP was 220/100 mm Hg. urine analysis revealed proteinuria (2+), hematuria (3+). Serum creatinine level was at 1125 μmol/L. She had anemia with 6.1 g/dL of hemoglobin and thrombocytopenia (61,000/mm3). Renal histology revealed lesions of thrombotic microangiopathy. The diagnosis o…

Glucocorticoid receptor gene polymorphisms in hereditary angioedema with C1-inhibitor deficiency.

BACKGROUND: Hereditary angioedema caused by C1-inhibitor deficiency (C1-INH-HAE) is a rare, autosomal dominant disorder. C1-INH-HAE is characterized by edema-formation, which may occur in response to stress. The individual's response to stress stimuli is partly genetically determined. Activation of the hypothalamic-pituitary-adrenal axis results in the release of cortisol. In turn, the secreted gluco- and mineralocorticoids affect the metabolism, as well as the cardiovascular and immune systems. We hypothesized that changes in serum cortisol level and polymorphisms of the glucocorticoid recept…
