MRI of trauma to the foot and ankle.

Traumatic injuries involving the foot and ankle are very common. With the advent of magnetic resonance imaging (MRI) and its unsurpassed ability for soft tissue characterization, its utility in the investigation of these patients with foot and ankle trauma has rapidly expanded over the last decade. MRI has the unique capability to evaluate osseous, ligamentous, tendinous, and muscular injuries about the foot and ankle, with a single imaging study. MRI also allows for characterization of injury based on known biomechanical patterns. Injuries to specific soft tissue structures such as the Lisfra…

Foot and ankle injuries in dance.

Although dancers develop overuse injuries common in other athletes, they are also susceptible to unique injuries. This article reviews common foot and ankle problems seen in dancers and provides some basic diagnosis and treatment strategies.

Desmoplastic melanoma arising in the ankle.

Desmoplastic melanoma (DM) is an uncommon but potentially devastating malignancy that can be cured with early recognition and surgery. DM often occurs in elderly men and develops on sun-exposed areas with the head and neck being the most common site of origin followed by the extremities and trunk. We report a rare case of DM occurring in the ankle in an 85-year-old woman. Magnetic resonance imaging of the affected ankle showed a 4.3 x 4.3 x 2.5 cm sized mass which was located in the subcutaneous region, attached to lower leg muscles, the fibula, and the calcaneus. No metastasis to the lung, li…

Clinical examination of the foot and ankle.

The foot and ankle are critical components in our ability to ambulate. Injuries to either can significantly interfere with a patient's ability to carry out normal activities. In severe cases, they can be devastating to a patient's independence. Careful examination of the foot and ankle using established mechanical tests, along with understanding of the anatomy of the complex,is needed to confirm the history and to assist in the diagnosis and treatment of foot and ankle injuries. The following points are key to clinical examination of the foot and ankle: . The examination of the foot and ankle …

Trauma of the pediatric ankle and foot.

This article presents a brief overview of the injuries to the ankle and foot encountered in children and adolescents. Trauma to the ankle or foot may result from acute, chronic, or repetitive forces. The role of the different imaging modalities in the assessment of ankle and foot trauma in the growing patient is discussed. Plain radiographs remain the mainstay in the diagnosis of most acute traumas, whereas CT may be helpful to unravel the complex anatomy of certain fractures like the triplane or juvenile Tillaux fracture. In the evaluation of chronic injuries, including osteochondrosis dissec…

Pseudoaneurysm as a complication of ankle arthroscopy.

We describe a case of pseudoaneurysm of the anterior tibial artery as a complication after arthroscopic ankle synovectomy, in which standard anterolateral and anteromedial portals were used. Pseudoaneurysm has been previously reported as a complication in ankle arthroscopy with the use of the anterocentral portal. Previously described anatomic variations of the tibial artery and its close relationship with the anterior ankle capsule may complicate arthroscopic surgery, especially when aggressive synovectomy is performed. Anterior tibial artery aneurysm is a rare complication of ankle arthrosco…

Foot and ankle injuries in dance.

Acute traumatic injuries are common in ballet dancers. A careful history, thorough examination, and appropriate imaging should allow for the diagnosis of most problems. The clinician must have a high index of suspicion for occult bony injuries, especially if the patient fails to recover as expected. Aggressive treatment of the sprained ankle is essential to maintain foot and ankle mobility and prevent prolonged disability and subsequent overuse injuries. Kinetic chain dysfunctions are common in ballet dancers with overuse injuries and commonly follow ankle sprains. They may represent a seconda…

[Chronic diseases of the ankle joint].

The etiology of chronic diseases of the ankle joint comprises a wide spectrum including chronic inflammatory processes and chronic degenerative, tumorous and neuropathic processes, as well as some specific syndromes based on chronic changes of the ankle joint. Of the inflammatory processes, chronic juvenile arthritis (JVC) is the most common disease. However, also Reiter disease, psoriasis or chronic monoarthritid diseases such as gout, as well as granulomatous diseases (tuberculosis, sarcoidosis) and fungal infections, may affect the ankle joint in a chronic course. Chronic degenerative chang…

Foot and ankle injuries and disorders.

This chapter analyses the mechanics of walking and running gait and delineates the terminology used to describe foot and ankle injuries. A helpful anatomic review precedes explanations of the mechanisms of sports injuries in this area. The author documents the correct diagnoses and treatments for foot and ankle sprains, fractures, and stress injuries.

Arthroscopy of the foot and ankle in the athlete.

Ankle arthroscopy has evolved over the past decade to address many acute and chronic injuries in the athlete. Although similar outcomes are reported with established open procedures, arthroscopic intervention confers the advantages of decreased postoperative morbidity and earlier return to sports activity. Arthroscopic procedures for the foot are evolving; however, most are considered investigational at this time.

Malignant melanoma of the foot and ankle.

The records of sixty patients who had a malignant melanoma of the foot or ankle were reviewed retrospectively to determine the clinical features, prognostic factors, and distinguishing characteristics. Fifty-seven patients were white and three were black. There were forty-two women and eighteen men (a female-to-male ratio of 2.3 to 1). The mean age at the time of presentation was fifty-seven years (range, twenty-two to eighty-three years). The most common site of involvement was the plantar aspect of the foot. The mean duration of follow-up was forty-five months (range, three to 144 months). K…

Kinematics of the axially loaded ankle.

An apparatus that allowed the application of a 900 N axial load and the simultaneous measurement of rotation in the sagittal, coronal, and axial planes was used to study the normal kinematics of the ankle in 13 below-knee amputation specimens. Two testing routines were done on all specimens. In the first sequence, specimens were moved through a dorsiflexion (DF) and plantarflexion (PF) arc of 60 degrees (25 degrees DF and 35 degrees PF). DF was associated with an average of 2.5 degrees of external rotation, and PF was associated with an average of < 1 degree of internal rotation. In the corona…
