Ankle joint contact loads and displacement in syndesmosis injuries repaired with Tightropes compared to screw fixation in a static model.

BACKGROUND: The effect of syndesmotic fixation on restoration of pressure mechanics in the setting of a syndesmotic injury is largely unknown. The purpose of this study is to examine the contact mechanics of the tibiotalar joint following syndesmosis fixation with screws versus a flexible fixation device for complete syndesmotic injury. METHODS: Six matched pairs of cadaveric below knee specimens were dissected and motion capture trackers were fixed to the tibia, fibula, and talus and a pressure sensor was placed in the tibiotalar joint. Each specimen was first tested intact with axial compre

Ligamentous Injuries and the Risk of Associated Tissue Damage in Acute Ankle Sprains in Athletes: A Cross-sectional MRI Study.

BACKGROUND: Ankle joint injuries are extremely common sports injuries, with the anterior talofibular ligament involved in the majority of ankle sprains. There have been only a few large magnetic resonance imaging (MRI) studies on associated structural injuries after ankle sprains. PURPOSE: To describe the injury pattern in athletes who were referred to MRI for the assessment of an acute ankle sprain and to assess the risk of associated traumatic tissue damage including lateral and syndesmotic ligament involvement. STUDY DESIGN: Cross-sectional study; Level of evidence, 3. METHODS: A total o

A randomized, prospective study of operative and non-operative treatment of injuries of the fibular collateral ligaments of the ankle.

One hundred and forty-six adults who had an isolated injury of the fibular collateral ligaments of the ankle were randomized to be managed operatively or non-operatively. Disruption of the ligaments was diagnosed by means of a physical examination and on the basis of stress radiographs of the ankle made with use of a specially designed device to hold the leg. Operative treatment, performed in seventy-three patients, consisted of suture repair of the disrupted ligaments within seventy-two hours after the injury, followed by immobilization of the ankle in a below-the-knee plaster cast for six we

Functional Walker Boots are Preferred to Synthetic Casts by Patients and Carers in the Management of Pediatric Stable Ankle Injuries.

INTRODUCTION: Traditionally children who presented with a stable ankle injury have been managed in a below-knee synthetic cast. No previous study has investigated patient and parents/carer preference between synthetic casts and walker boots. METHODS: Children aged between 6 and 16 years who presented with stable ankle injuries (ankle sprains, small fragment avulsions, and fibular Salter-Harris 1 injuries) were randomized to receive treatment with either a synthetic cast or a walker boot. A tailored, study-specific questionnaire was designed and completed at 4 weeks from injury to evaluate bot…

Can Weightbearing Cone-beam CT Reliably Differentiate Between Stable and Unstable Syndesmotic Ankle Injuries? A Systematic Review and Meta-analysis.

BACKGROUND: Ankle injuries are common presentations to the emergency department and may lead to syndesmotic instability. These have a high socioeconomic burden due to prolonged rehabilitation, chronic pain, and posttraumatic arthritis. Early diagnosis is essential to minimize these complications, and the assessment of instability in the clinical setting is often limited by pain and clinician experience. Cross-sectional imaging of the distal syndesmosis accurately evaluates the syndesmosis through abnormal bony relationships, which in the presence of instability, worsens during physiological lo…

Getting the Starting Point Right: Prevention of Skiving and Fibular Cortical Breach During Suture Button Placement for Syndesmotic Ankle Injuries.

The use of suture button (SB) devices in the treatment of syndesmotic ankle injuries is increasing. These constructs have demonstrated better syndesmotic reduction, improved clinical outcomes, and lower rates of hardware removal compared with screw fixation. However, placing a SB device without a fibular plate can be technically challenging. In this technique tip, we use an illustrative case to demonstrate a technique tip that minimizes the risk of anterior or posterior cortical breach of the fibula and helps facilitate more accurate placement of a SB device.Levels of Evidence: Level V: Expert…

Beyond the bones and joints: a review of ligamentous injuries of the foot and ankle on 99mTc-MDP-SPECT/CT.

99mTc-MDP-single photon emission computed tomography (SPECT)/CT has established itself as a useful modality in the assessment of the painful foot and ankle, owing to its ability to depict anatomical and functional information simultaneously. Despite its growing popularity, much of the literature describing the utility of 99mTc-MDP-SPECT/CT of the foot and ankle is limited to osseous and articular pathology, with descriptions of ligamentous pathology limited to just a handful of cases. Though assessment of soft tissues using a combination of bone tracer and CT is limited, with a sound understan

The relationship between the height of the medial longitudinal arch (MLA) and the ankle and knee injuries in professional runners.

BACKGROUND: Usually the rate of lower extremity's injuries in sports such as running is known to be correlated with the height of foot arches. Foot pressure measurement studies have shown controversial results in this issue, mainly due to the complexity of the foot structure. This study aimed to investigate if any relationship exists between the MLA height and the ankle/knee injuries in professional runners. It was also aimed to find out any association between the foot pressure patterns and the clinical navicular drop test in the subjects. MATERIALS AND METHODS: Forty-seven professional runn

[Ligament injuries of the ankle--what happens later? Non-surgical treatment is effective in 80-90 per cent of cases].

Ligament injuries of the ankle joint are the most common sports-related injuries. In more than 90 per cent of all cases, it is a question of lateral ligament injury. The recommended acute treatment is always non-surgical, with range of motion training, followed by strengthening and co-ordination exercises. Approximately 80-90 per cent of all ligament injury patients regain normal ankle function after this form of treatment. Early active treatment can reduce time off from sports and the duration of sick leave. Residual problems, mainly pain or recurrent or chronic instability are seen in approx

Contribution of ankle motion pattern during landing to reduce the knee-related injury risk.

BACKGROUND: Single-leg landing (SL) is an essential technique in sports such as basketball, soccer, and volleyball, which is often associated with a high risk of knee-related injury. The ankle motion pattern plays a crucial role in absorbing the load shocks during SL, but the effect on the knee joint is not yet clear. This work aims to explore the effects of different ankle plantarflexion angles during SL on the risk of knee-related injury. METHODS: Thirty healthy male subjects were recruited to perform SL biomechanics tests, and one standard subject was selected to develop the finite element

Evaluation of Syndesmosis Reduction after Removal Syndesmosis Screw in Ankle Fracture with Syndesmosis Injury.

BACKGROUND: Ankle fracture-dislocation with a syndesmotic injury has been treated with syndesmotic screw fixation. There are little evidences about the safety and efficacy of syndesmotic screw removal on the syndesmotic malreduction. This study aimed to evaluate the effects of syndesmotic screw removal of distal attachment of the fibula and tibia bones on the syndesmotic reduction and also impact of syndesmotic screw removal on the final functional score of ankle joint. MATERIALS AND METHODS: Patients who underwent syndesmotic screw fixation for diagnosed syndesmosis injury during internal fi

Supination External Rotational Ankle Fracture Injury Pattern Correlation With Regional Bone Density.

BACKGROUND:: Rotational ankle fractures can present with an array of possible osseous and ligamentous injury combinations in reliable anatomic locations. What accounts for these different injury patterns and whether specific patient and injury factors underlie the different injury patterns is unclear. The purpose of this study was to determine whether causative factors exist that could account for the various injury patterns seen with rotational ankle fractures. METHODS:: A registry of operatively treated supination external rotation stage IV (SER IV) ankle fractures was used to identify pati
