An innovative intraocular lens (IOL), the CM-T Flex IOL, was used to correct surgical aphakia without disturbing a functioning filtering bleb. A 66-year-old man presented with aphakia in the left eye. Cataract extraction and trabeculectomy was performed in the left eye 2 years previously elsewhere. METHODS: Retrospective institutional medical record review of paediatric aphakic patients 12 years of age and younger with documented ultrasonic axial length and objective refraction (retinoscopy) within 3 months of each other. The level of glycosylated blood hemoglobin (HbA1c) as a clinical and prognostic criterion for selecting diabetics for implantation of intraocular lens (IOL) is assessed. Extracapsular cataract extraction with intraocular correction was carried out in 68 patients with noninsulin-dependent diabetes mellitus. A combined surgical procedure consisting of corneal relaxing incisions and epikeratophakia was used to correct high-degree postkeratoplasty astigmatism and aphakia in two patients. Interlamellar refraction keratoplasty with central keratectomy of superficial laminae of the cornea was used to correct aphakia in 15 patients and the results followed up for 2.5-7 years. This method permits a simultaneous refraction and optic transplantation of the cornea. Corneal refraction was enhanced by 7.2-14.8 diopters (the mean value 11.03 diopters). The technique is an extraocular one and therefore may be widely used for aphakia correction, particularly in cases with superficial opacities of the cornea.

More work is necessary to establish corneal dimensions and retinal structure and neural organisation in the equine eye. This paper reports a case of surgical management of bilateral cataracts in a pony and the results of a survey of refractive error in normal horses. Aspects of accommodation are discussed.
