Experimental chronic erysipelas polyarthritis in rat, induced by living erysipelas bacteria, histologically can be divided into four different phases. In the phase of population bacteria are distributed diffusely within the whole joint but accumulate in the transitional zones and entheses by multiplication within the ground substance of cartilage. In the phase of acute destruction a severe inflammation of all joint tissues predominates. Bacterial antigen is eliminated by a pannus tissue destroying the cartilaginous structures. In the following phase a diffuse dystrophy of articular cartilage d…

Many viruses can cause arthritis in children. Viral arthritis can mimic other rheumatic conditions as rheumatic fever, septic arthritis or juvenile rheumatoid arthritis. Usually viral arthritis is a self-limited disease; occasionally it may show a chronic course lasting several months. Rubella arthritis follows natural infection or vaccination; it looks the expression of a direct invasion of the synovial tissue by the virus. Hepatitis B arthritis is an immune-complex synovitis occurring during the prodromic stage of HBV infection.

Rubella virus and the rubella virus vaccine are relatively common causes of acute arthralgias and occasionally objective arthritis. Rarely, chronic or relapsing joint syndromes may follow exposure to this virus or vaccine. This article will focus on the virus and its clinical illness. Major emphasis then will be given to the resulting arthritis syndromes and to consideration of the possible pathogenetic mechanisms of the joint manifestations.

A 54-year old man had severe inflammatory polyarthritis 10 days after the onset of an acute febrile illness that was serologically documented to be psittacosis. The pattern and chronicity of the articular symptoms, the response to nonsteroidal anti-inflammatory agents, and the presence of HLA-B7-CREG strongly suggest that this was a reactive arthritis. The association of psittacosis and reactive arthritis has previously been reported in the British literature, but this may be the first reported North American case.

Rheumatoid arthritis often begins in the hand, and upper extremity function can be seriously impaired with progression of the disease. When numerous areas are involved, a multitude of deformities can arise. Before initiating treatment of an arthritic patient, the surgeon must understand the natural course of the disease process, the response to medical treatment, and the functional limitations the disease has imposed. This article describes the examination for and treatment of this disease.

Fasting hypergastrinemia was noted in 13-41% of 82 of outpatients with definite or classical rheumatoid arthritis. No differences were noted between the normogastrinemic and the hypergastrinemic subjects in their age, sex, duration of rheumatoid disease, serological status, or medications. The food-stimulated gastrin response was 117 mg/L, or 216% above the basal levels, and the integrated gastrin response was increased to the level previously reported in patients with duodenal or gastric ulcer disease. These results suggest that hypergastrinemia is a feature of patients with rheumatoid arthri…

The clinical and microbiological features of bacterial arthritis in 21 elderly patients presenting to hospitals in an English health district over the decade 1973-1982 are reviewed. Differences from bacterial arthritis in younger patients include the high prevalence (71%) of underlying joint disease, infections more commonly affecting the hip (38%), absence of constitutional features of toxaemia and delay in diagnosis. Outcome in elderly patients is markedly worse, with an appreciable mortality attributable to the condition.

What is currently classified as rheumatoid arthritis in the elderly (that is, late onset after age 60) may in fact include a number of separable distinct diseases. Identifiable subsets include seropositive or 'classic' rheumatoid arthritis, Sjögren's syndrome with a limited form of arthritis, and a seronegative benign synovitis akin to polymyalgia rheumatica.

Lyme arthritis appears not to be a rare manifestation of Borrelia burgdorferi infection in Germany. We report 20 cases of the illness occurring in South Germany. In eleven of our patients arthritis was the only clinical manifestation of the infection; the diagnosis in those cases has only been verified by the detection of specific antibodies to Borrelia burgdorferi with indirect immunofluorescence and IgG Western blot technique. In comparison with previous reports of Lyme arthritis the quota of chronic joint involvement in our patients was relatively high, 4 of the patients developed radiograp…

A study in 50 children suffering from acute brucellosis demonstrated that acute arthritis is a common and often predominant manifestation of the disease. The patients lived in an area where brucellosis is endemic: their ages ranged from six months to 12 years. All patients had a brucella agglutination titer of greater than or equal to 1:320 on admission. The knee and hip joints were most commonly affected with symptoms and findings from only one joint predominating. Associated findings of only moderately elevated ESR and normal leucocyte counts with relative lymphocytosis in the vast majority …

We report six patients with coeliac disease in whom arthritis was prominent at diagnosis and who improved with dietary therapy. Joint pain preceded diagnosis by up to three years in five patients and 15 years in one patient. Joints most commonly involved were lumbar spine, hips, and knees (four cases). In three cases there were no bowel symptoms. All were seronegative. X-rays were abnormal in two cases. HLA-type A1, B8, DR3 was present in five and B27 in two patients. Circulating immune complexes showed no consistent pattern before or after treatment. Coeliac disease was diagnosed in all patie…

The clinical and microbiologic features of septic arthritis in 23 elderly patients are reviewed. Fifteen patients had pre-existing joint diseases, predominantly osteoarthritis and rheumatoid arthritis. Eight patients had underlying systemic illnesses, and eight patients were receiving systemic corticosteroid therapy prior to the development of septic arthritis. The knee was the joint most commonly infected. Although Staphylococcus aureus was the major pathogen (52.2 per cent of patients), enteric gram-negative bacilli were found in seven of 23 patients (30.4 per cent). Five patients died (21.7…

Definite genetic associations with immunological cooperative HLA-D(R) antigens have been demonstrated for rheumatoid arthritis (RA). Microbial etiology has not been proven, but some hope for the supporters of this view is still given by small viruses, plasmids of enteric bacteria or perhaps oncogen-like DNA-sequences. Yet, electrophoretical analysis of membrane proteins or surface glycoproteins of RA synovial cells does not show any differences compared to reference cells. Autoimmunity to several tissue elements has been demonstrated, but most of it is of secondary nature. Antigenicities of ty…
