A model of practice for the management of velopharyngeal dysfunction.

This paper reviews normal speech production, the speech characteristics associated with velopharyngeal dysfunction, the investigations required and treatment options. A model of service delivery is proposed which has been in place over the past 4 years. This model highlights multidisciplinary management by a specialist team. An audit of the service has been undertaken. Of the 225 patients audited, some interesting findings are presented on aetiology, diagnosis and trends in management. Systematic and comprehensive collection of data on large numbers of patients permits meaningful audit of proc

Prevalence of articulatory disorders of different sounds in a group of Finnish first-graders.

Occurrence and type of articulatory speech disorders were studied in 157 7-year-old first-graders found to have articulatory disorders at school and referred for further treatment to the Department of Phoniatrics, University Central Hospital. The control group consisted of all 130 first-graders in two elementary schools in Kuopio. About one third of the control group had articulatory speech disorders, one fifth of them expressing need for speech therapy. In both groups distortions of the /s/-sound were the most common disorders, distortions of the /r/-sound being rather uncommon and those of t

Reversible slurred speech related to capecitabine and lapatinib combination in patients with breast cancer.

Capecitabine plus lapatinib combination is an effective chemotherapy regimen in patients with advanced breast cancer. Neurological adverse effects secondary to this regimen were reported rarely in literature. A woman with breast cancer presented with complaints of slurred speech while using the capecitabine and lapatinib combination. Her major complaint was slurred speech. No other radiologic or laboratory disorders were detected in the patient. Slurred speech improved one week after the capecitabine and lapatinib combination was discontinued without any further intervention.

Intervention for lateral /s/ using electropalatography (EPG) biofeedback and an intensive motor learning approach: a case report.

BACKGROUND: Visual biofeedback using electropalatography (EPG) has been beneficial in the treatment of some cases of lateral /s/ misarticulation. While EPG intervention is motorically based, studies have not commonly employed a motor learning approach to treatment. Furthermore, treatment success is measured primarily by change to EPG tongue-palate contact patterns and listener ratings conducted by speech-language therapists. Studies have not commonly measured articulatory change without the palate in-situ using acoustic analysis and non-professional listeners. AIMS: To determine if an intensi

Interventions for the management of submucous cleft palate.

BACKGROUND: Submucous cleft palate (SMCP) is a common congenital malformation of the soft palate which may present as velopharyngeal insufficiency (VPI), which can affect the quality and intelligibility of speech. Surgical techniques, which can be used to reconstruct these structural or anatomical defects and to correct velopharyngeal insufficiency, include palatal repair and procedures that rearrange the muscle attachments of the soft palate. OBJECTIVES: To provide reliable evidence regarding the effectiveness of surgical interventions to treat velopharyngeal insufficiency and improve speech

The genetic bases of speech sound disorders: evidence from spoken and written language.

The purpose of this article is to review recent findings suggesting a genetic susceptibility for speech sound disorders (SSD), the most prevalent communication disorder in early childhood. The importance of genetic studies of SSD and the hypothetical underpinnings of these genetic findings are reviewed, as well as genetic associations of SSD with other language and reading disabilities. The authors propose that many genes contribute to SSD. They further hypothesize that some genes contribute to SSD disorders alone, whereas other genes influence both SSD and other written and spoken language di

Developmental dyslexia in a regular orthography: a single case study.

This study of an adult case examined in detail with eye movement measures the reading speed problem which is characteristic for developmental dyslexia in regular orthographies. A dramatic length effect was found for low frequency words and for pseudowords, but not for high frequency words. However, even for high frequency words it was found that reading times were substantially prolonged although number of fixations did not differ. A neurocognitive assessment revealed no visual deficits (parallel processing, precedence detection, coherent motion detection) but speed impairments for certain ver

Uvular transposition: a new method of cleft palate repair.

This prospective study was done to determine whether a new cleft palate repair utilizing uvular transposition improved speech outcome as measured objectively by a speech pathologist. In the uvular transposition procedure, the palate was lengthened with tissue from the uvula by a double-opposing Z-plasty; an intravelar veloplasty was performed, and two-thirds of the mass of the uvula was transposed to the nasal surface of the soft palate. This procedure facilitates velopharyngeal closure by significantly lengthening the palate, anatomically reconstructing the muscles of the palate, and decreasi

Effects of treatment for sound errors in apraxia of speech and aphasia.

This investigation was designed to examine the acquisition, generalization, and maintenance effects of a treatment for sound errors in speakers with co-occurring apraxia of speech and aphasia. Three speakers with chronic apraxia of speech and aphasia were studied in the context of a multiple baseline design across speakers and behaviors. Treatment combined the use of minimal contrast pairs with traditional sound production training techniques such as integral stimulation and articulatory placement cueing and was applied sequentially to sounds that were determined to be consistently in error be

Correction of velopharyngeal insufficiency by pharyngeal augmentation using autologous cartilage: a preliminary report.

Twenty patients with velopharyngeal insufficiency (VPI) were treated with autologous costal bone or cartilage implants. Videofluoroscopy and videonasopharyngoscopy studies were used to identify candidates for the procedure. The specific size and shape of the gap as well as an appropriate location for the implant were determined in those patients for whom videonasopharyngoscopy was used. A piece of costal bone or cartilage was implanted into a preselected site in the posterior pharynx. Speech and voice evaluation were conducted preoperatively and 8 weeks postoperatively. Hypernasality and audib

CT of the heel and midfoot.

The authors stress the importance of CT examination in certain pathologic conditions of the heel and midfoot. In this region the bone structures form complicated articulations which can be affected by a number of disorders. This study (43 cases) includes the normal CT images of the heel and midfoot and an illustration of the various pathologic conditions encountered in this region: heel traumata (18 cases), bone tumors (4 cases), congenital coalitions (5 cases), and some inflammatory and soft tissue disorders (12 cases). Depending on the clinical aspect a coronal and/or axial incidence with fi

Rehabilitation of speech and swallowing after burns reconstructive surgery of the lips and nose.

Rehabilitation of speech and swallowing postburns reconstructive surgery has not been previously described in detail in the literature. Severe facial burn injury requiring subsequent reconstructive surgery may result in complications including circumoral contracture and aesthetic and functional irregularities. These complications may manifest as facial and labial sensation deficits, poor oral access for intubation and oral/dental hygiene, and inadequate oral competence causing chronic drooling and poor articulation. This report describes the physical rehabilitation of a patient with full-thick

Susceptibility to intraspeech spread of masking in listeners with sensorineural hearing loss.

Previous research with speechlike signals has suggested that upward spread of masking from the first formant (F 1) may interfere with the identification of place of articulation information signaled by changes in the upper formants. This suggestion was tested by presenting two-formant stop consonant--vowel syllables varying along a/ba--/da/--/ga/ continuum to hearing-impaired listeners grouped according to etiological basis of the disorder. The syllables were presented monaurally at 80 dB and 100 dB SPL when formant amplitudes were equal and when F 1 amplitude was reduced by 6, 12, and 18 dB.

Distal Radioulnar Joint: Normal Anatomy, Imaging of Common Disorders, and Injury Classification.

The distal radioulnar joint (DRUJ) is the distal articulation between the radius and ulna, acting as a major weight-bearing joint at the wrist and distributing forces across the forearm bones. The articulating surfaces are the radial sigmoid notch and ulnar head, while the ulnar fovea serves as a critical attachment site for multiple capsuloligamentous structures. The DRUJ is an inherently unstable joint, relying heavily on intrinsic and extrinsic soft-tissue stabilizers. The triangular fibrocartilage complex (TFCC) is the chief stabilizer, composed of the central disk, distal radioulnar ligam
