Multicystic Encephalomalacia in Early Infancy: A Neuroimaging Case Report.

Multicystic encephalomalacia (MCE) is an uncommon but severe consequence of perinatal hypoxic-ischemic injury in neonates. We report a 1.5-month-old male infant presenting with fever and focal seizures, with a prior history of birth asphyxia and prolonged neonatal intensive care unit stay. Neurosonography and magnetic resonance imaging revealed extensive bilateral cortical and subcortical cystic changes with cerebral atrophy consistent with MCE. This case highlights the importance of early neuroimaging to identify characteristic patterns of extensive cystic degeneration, enabling timely progno

The umbilical cord as a potential culprit in cases of perinatal asphyxia. A case-control study.

OBJECTIVE: To characterize the pathological features of placentas from neonates with perinatal asphyxia and to compare them with those from neonates without asphyxia. METHODS: An observational case-control study was conducted, including a group of patients diagnosed with perinatal asphyxia and a comparison group without this condition. SETTING: A high-complexity neonatal unit. PARTICIPANTS: 83 neonates were included: 28 with perinatal asphyxia and 55 without asphyxia, all of whom had placental examination available. RESULTS: Umbilical cord abnormalities were significantly associated with p

A Survey on Diagnosis and Prognosis of Prenatal Asphyxia based on Oxidant Antioxidant Balance: Evidence from a Systematic Review and Meta-analysis.

INTRODUCTION: The mechanism of occurrence and complications of asphyxia change in the treatment process and the future prognosis of newborns. One of the discussed mechanisms is the disruption of oxidant anti-oxidant balance. Therefore, the current study was conducted aiming to systematically review and meta-analysis in the diagnosis and prognosis of prenatal asphyxia based on oxidant-antioxidant balance. METHODS: A comprehensive electronic search was conducted with PubMed, Cochrane Library, Scopus, and Web of Science databases, up to February 2023 to identify relevant studies examining the as

[Association between serum cortisol levels and multiple organ dysfunction in neonates with perinatal asphyxia].

Introduction: Therapy-resistant hypotension is observed in 16% of neonates with perinatal asphyxia undergoing therapeutic hypothermia. Relative adrenal insufficiency, defined by an inadequate cortisol response to stress, is a potential etiological factor. Hydrocortisone supplementation may be the causal treatment; however, optimal dosing during hypothermia remains unknown. Serum cortisol levels may serve as a useful marker for monitoring treatment, although these levels are likely influenced by multiorgan dysfunction secondary to the hypoxic injury and by the effects of hypothermia on drug met

Trends and distribution of birth asphyxia, Uganda, 2017-2020: a retrospective analysis of public health surveillance data.

BACKGROUND: During 2018-2020, almost half of all neonatal deaths reviewed in Uganda were due to birth asphyxia. In 2015, Uganda adopted the Every Newborn Action Plan interventions to renew the focus on surveillance for birth asphyxia and other childhood-related illnesses. In 2016, the Ministry of Health implemented an evidence-based educational program for birth attendants about neonatal resuscitation techniques to improve the management of birth asphyxia. We described the trends and distribution of birth asphyxia in Uganda during 2017-2020 following these renewed efforts. METHODS: We analyse

Bridging gaps in recommendations for the management of newborns with severe perinatal asphyxia or presumed hypoxic-ischemic encephalopathy: a French Delphi consensus.

UNLABELLED: Neonatal hypoxic-ischemic encephalopathy (HIE) is a severe complication of perinatal asphyxia requiring timely and coordinated interventions. While international guidelines address early resuscitation and therapeutic hypothermia, key aspects of severe perinatal asphyxia and HIE management remain undefined, leading to practice variability in France. This study aimed to achieve national expert consensus on the first hour's perinatal care for infants with perinatal asphyxia or HIE using the Delphi method. A three-round Delphi survey was conducted between April and July 2021, involving

Auditory Impairment in Infants with Neonatal Hypoxic-Ischaemic Encephalopathy: A Systematic Review and Meta-Analysis.

INTRODUCTION: Hypoxic-ischaemic encephalopathy (HIE) due to perinatal asphyxia remains a significant cause of neonatal morbidity and mortality. Despite therapeutic hypothermia (TH), a considerable proportion of survivors experience a wide range of deficits, including auditory impairment (AI), which needs deeper knowledge. This review aimed to describe AI outcomes in infants with HIE. METHODS: A systematic literature review was performed using standard methods outlined by the Preferred Reporting Items for Systematic Reviews and Meta-Analyses protocol. A qualitative synthesis of all the include

Qualitative Insights Into Enhancing Neonatal Resuscitation in Post-Pandemic Vietnam: A Stakeholder Perspective on the Helping Babies Breathe Program.

BACKGROUND: The neonatal phase is vital for child survival, with a substantial portion of deaths occurring in the first month. Neonatal mortality rates differ significantly between Vietnam (10.52/1000 live births) and the United States (3.27/1000). In response to these challenges, interventions such as the Helping Babies Breathe (HBB) program have emerged, aiming to enhance the quality of care provided during childbirth, and the postpartum period in low-resource settings. PURPOSE: The purpose of this study was to explore stakeholder perceptions of the HBB program in Vietnam postpandemic, aimi

EUROPEAN ASSOCIATION OF PERINATAL MEDICINE (EAPM) Position statement: Use of appropriate terminology for situations related to inadequate fetal oxygenation in labor.

In high-resource countries, adverse perinatal outcomes are currently rare in term, non-malformed fetuses, undergoing labor, but they remain a leading cause of medico-legal dispute. Precise terminology is important to describe situations related to inadequate fetal oxygenation in labor, to ensure appropriate communication between healthcare professionals and adequate transmission of information to parents. This position statement provides consensus definitions from European perinatologists and midwives regarding the most appropriate terminology to describe situations related to inadequate fetal

Protocol to optimize the Rice-Vannucci rat pup model of perinatal asphyxia to ensure predictable hypoxic-ischemic cerebral lesions.

The Rice-Vannucci model in rodent pups is subject to substantial loss of animals, result inconsistency, and high lab-to-lab variability in extent and composition of induced injury. This protocol allows for highly predictable and reproducible hypoxic-ischemic cerebral injury lesions in post-natal day 10 Wistar rat pups with no mortality. We describe steps for common carotid artery ligation, brief post-operative normothermia, exposure to hypoxia, and post-hypoxic normothermia. Precise timing and temperature control in each step are crucial for a successful procedure. For complete details on the

Uncovering the Role of Inflammation with Asphyxia in the Newborn.

The etiology of perinatal brain injury is multifactorial, but exposure to perinatal hypoxiaischemia (HI) is a major underlying factor. This review discusses the role of exposure to infection/inflammation in the evolution of HI brain injury, changes in immune responsiveness to subsequent inflammatory challenges after HI and modulation of neural outcomes with interaction between perinatal HI and inflammatory insults. The authors critically assess the clinical and preclinical evidence for the neuroprotective efficacy of therapeutic hypothermia and other anti-inflammatory treatments for inflammati

Timing of neonatal mortality and severe morbidity during the postnatal period: a systematic review.

OBJECTIVE: The objective of this review was to determine the timing of overall and cause-specific neonatal mortality and severe morbidity during the postnatal period (1-28 days). INTRODUCTION: Despite significant focus on improving neonatal outcomes, many newborns continue to die or experience adverse health outcomes. While evidence on neonatal mortality and severe morbidity rates and causes are regularly updated, less is known on the specific timing of when they occur in the neonatal period. INCLUSION CRITERIA: This review considered studies that reported on neonatal mortality daily in the
