Minimally invasive transtracheal cardiac plexus block for sympathetic neuromodulation.

BACKGROUND: Bilateral thoracoscopic stellectomy has antiarrhythmic effects, but the procedure is invasive with associated morbidity. Sympathetic nerves from both stellate ganglia form the deep cardiac plexus (CP) in the aortopulmonary window, anterior to the trachea. OBJECTIVE: The purpose of this study was to demonstrate a novel and minimally invasive transtracheal approach to block the CP in porcine models. METHODS: In 12 Yorkshire pigs, right (RSG) and left (LSG) stellate ganglia were electrically stimulated and sympathetic baseline response recorded (hemodynamic parameters and T-wave pat

Catheter-based distal sciatic nerve block in patients with Charcot-Marie-Tooth disease.

BACKGROUND: The use of peripheral nerve blocks in patients with Charcot-Marie-Tooth (CMT) disease is scarcely reported; however, when performed it has proven to be effective for postoperative pain control. METHODS: A distal catheter-based sciatic nerve block for postoperative pain control was offered to 27 consecutive CMT patients scheduled for elective foot surgery. 18 of the 27 CMT patients consented to the offered sciatic nerve block. Localization of the sciatic nerve was guided by a nerve stimulator. The threshold current required to generate a motor response was assessed and a catheter i

Therapeutic spinal corticosteroid injections for the management of radiculopathies.

Current literature and a recent meta-analysis suggest a favorable role for corticosteroid injections in the nonoperative treatment of radiculopathy [70]. The superior results reported in recent literature may be attributable to precise fluoroscopically guided transforaminal placement of injectate close to the disc-nerve root interface and near the dorsal root ganglia, maximizing the therapeutic effect. The favorable results of corticosteroid injections in the treatment of radiculopathy caused by a focal disc herniation are consistent with the biochemical construct of radicular pain. The prelim

Non-adrenergic, non-cholinergic salivary secretion in the ferret.

Secretion of parotid and submandibular saliva evoked by electrical stimulation of the parasympathetic nerves occurred in the presence of adrenergic and cholinergic blocking agents. The 'atropine-resistant' response amounted to about 30% in the submandibular gland and to about 5% in the parotid gland as compared to the response before administration of antimuscarinic agents. The 'atropine-resistant' secretion was found at relatively high frequencies, and the latency between the start of the stimulation and the onset of secretion was long. Upon long-lasting stimulation the 'atropine-resistant' s

Cluster headache pathophysiology - insights from current and emerging treatments.

Cluster headache is a debilitating primary headache disorder that affects approximately 0.1% of the population worldwide. Cluster headache attacks involve severe unilateral pain in the trigeminal distribution together with ipsilateral cranial autonomic features and a sense of agitation. Acute treatments are available and are effective in just over half of the patients. Until recently, preventive medications were borrowed from non-headache indications, so management of cluster headache is challenging. However, as our understanding of cluster headache pathophysiology has evolved on the basis of

Role of aortic and carotid sinus baroreceptors on Valsalva-like vasoconstrictor and heart rate reflexes in the conscious rabbit.

Graded 30 s manoeuvers were used to elicit cardiovascular responses in conscious instrumented rabbits by applying pressures from 2.5 to 20 mm Hg (Valsalva Pressure, VP) to the animal's respiratory valve and to a sphygmomanometer cuff wrapped around its thorax and abdomen. In normal rabbits the Valsalva-like manoeuvers elicited VP-related rises in total peripheral resistance (TPR), heart rate and in right atrial pressure (RAP), VP-related fall in cardiac output (CO) and no change in mean arterial pressure (MAP). The rises in TPR and heart rate and the maintenance of MAP were reflexly mediated,

Use of Temporary Peripheral Nerve Stimulation for Management of Recalcitrant Pain in Patients With Complex Regional Pain Syndrome: Case Series and Literature Review.

BACKGROUND: Complex regional pain syndrome (CRPS) is a chronic pain syndrome involving autonomic, sensory, motor, and sudomotor dysfunction. Its pathophysiology involves both central and peripheral nervous systems, with a constant interplay between multiple mechanisms. Treatment for CRPS varies from pharmacological choices, physical therapy, and sympathetic blocks to neuromodulation techniques, namely use of spinal cord stimulation, dorsal root ganglion stimulation, and peripheral nerve stimulation (PNS). CASE REPORT: We present a case series detailing the use and efficacy of temporary PNS fo

Peripheral nerve blocks for primary and secondary headache disorders: review of current evidence and a practical approach.

Headache is prevalent, disabling, and a frequent neurological referral in the healthcare system. Clinic-based procedures have evolved in recent years to play an important role in headache medicine, with growing evidence on the safety, tolerability and efficacy of peripheral nerve blocks (PNBs). Despite novel headache therapies, PNBs are still widely used in headache services to treat primary and secondary headache disorders, including cluster headache and other trigemino-autonomic cephalalgias, migraine, occipital neuralgia, and other less frequent headache disorders. We aim to provide an upda

Careful Dissection of the Distal Ureter Is Highly Important in Nerve-sparing Radical Pelvic Surgery: A 3D Reconstruction and Immunohistochemical Characterization of the Vesical Plexus.

OBJECTIVE: Radical hysterectomy with pelvic lymphadenectomy (RHL) is the preferred treatment for early-stage cervical cancer. Although oncological outcome is good with regard to recurrence and survival rates, it is well known that RHL might result in postoperative bladder impairments due to autonomic nerve disruption. The pelvic autonomic network has been extensively studied, but the anatomy of nerve fibers branching off the inferior hypogastric plexus to innervate the bladder is less known. Besides, the pathogenesis of bladder dysfunction after RHL is multifactorial but remains unclear. We st

Diagnostic value pharmacological autonomic blockade in patients with suspected sick sinus syndrome.

The study comprised 67 subjects, mean age 43.7 years, with suspected sick sinus syndrome, in whom rapid atrial pacing before and after combined atropine and propranolol was performed by Narula's method. Three groups were formed: group I--with normal sinus node recovery time (SNRT) and corrected sinus node recovery time (CNRT) before and after the autonomic blockade; group II--with functional disorders of the sinus node and group III--with intrinsic sinus node dysfunction. After autonomic blockade in groups I and II mean SNRT, CNRT, post-stimulation cycle lengths (except No. 2 and Nos. 5, 6, 10

Peripheral nerve blocks in primary headaches and neuralgias: a six-country multicenter real-world study.

Peripheral nerve blocks (PNBs) represent an accessible, minimally invasive therapeutic approach for headache disorders; however, real-world data on procedural patterns and outcomes in different headache subtypes remain limited. This multicenter study evaluated 2,219 patients (receiving 2250 PNBs) with migraine, tension-type headache (TTH), trigeminal autonomic cephalalgias (TACs), and cranial neuralgias. Demographic features, preventive and acute medicine, target nerves, anaesthetic agents, injection volumes, application intervals, number of sessions, clinical outcomes, and adverse events were

T3T4 endoscopic sympathetic blockade versus T3T4 video thoracoscopic sympathectomy in the treatment of axillary hyperhidrosis.

BACKGROUND: The use of endoscopic sympathetic blockade (ESB) in the treatment of axillary hyperhidrosis has grown because of its potential reversibility. But it is still not clear whether the rates of success, compensatory sweating, and satisfaction are better than those accomplished with video thoracoscopic sympathectomy (VTS). METHODS: Eighty-four patients were studied to compare the rates of success, satisfaction, and compensatory sweating in patients undergoing either ESB or VTS of the T3T4 ganglion after 2 years' follow-up. RESULTS: Thirty-five patients (83.3%) undergoing ESB and 39 pat

Sacral nerve stimulation with appropriate parameters improves constipation in rats by enhancing colon motility mediated via the autonomic-cholinergic mechanisms.

Although sacral nerve stimulation (SNS) has been applied for treating constipation, its parameters were adopted from SNS for fecal incontinence, its effects are limited, and mechanisms are largely unknown. We investigated the effects and mechanism of SNS with appropriate parameters on constipation in rats treated with loperamide. First, using rectal compliance as an outcome measure, an experiment was performed to derive effective SNS parameters. Then, a 7-day SNS was performed in rats with constipation induced by loperamide. Autonomic functions were assessed by spectral analysis of heart rate

A Double-Blind Randomized Comparison of Continuous Interscalene, Supraclavicular, and Suprascapular Blocks for Total Shoulder Arthroplasty.

BACKGROUND AND OBJECTIVES: Continuous brachial plexus blocks at the interscalene level are associated with known diaphragm dysfunction from phrenic nerve paresis. More distal blocks along the brachial plexus may provide postsurgical analgesia while potentially having less effect on diaphragm function. Continuous interscalene, continuous supraclavicular, and continuous suprascapular nerve blocks were evaluated for respiratory function and analgesia after total shoulder arthroplasty. METHODS: After ethics board approval, subjects presenting for total shoulder arthroplasty were planned for rando

Effects of Intraneural Injection of Dexmedetomidine in Combination With Ropivacaine in Rat Sciatic Nerve Block.

BACKGROUND AND OBJECTIVES: Dexmedetomidine is known to have neural protection effect via attenuation of inflammatory responses induced by local anesthetics. We investigated whether intraneural dexmedetomidine is effective for attenuating or preventing neural injury resulting from inadvertent intraneural injection of local anesthetic. METHODS: Rats were randomly divided, and left sciatic nerve was surgically exposed. The rats received no injection (control group) or intraneural injections of 0.2 mL of normal saline (saline group), 0.2 mL of 0.5% ropivacaine (ropivacaine group), or 0.2 mL of 0.

Risks and Benefits of Ultrasound, Nerve Stimulation, and Their Combination for Guiding Peripheral Nerve Blocks: A Retrospective Registry Analysis.

BACKGROUND: Ultrasound, nerve stimulation, and their combination are all considered acceptable ways to guide peripheral nerve blocks. Which approach is most effective and associated with the fewest complications is unknown. We therefore used a large registry to analyze whether there are differences in vascular punctures, multiple skin punctures, and unintended paresthesia. METHODS: Twenty-six thousand seven hundred and thirty-three cases were extracted from the 25-center German Network for Regional Anesthesia registry between 2007 and 2016 and grouped into ultrasound-guided puncture (n = 10,3

Concomitant horner and harlequin syndromes after inferior alveolar nerve block anesthesia.

INTRODUCTION: Intraoral anesthesia is probably the most commonly used procedure in dentistry. METHODS: Although inferior alveolar nerve block (IANB) anesthesia is one of the safest procedures to anesthetize the mandibular teeth, side effects of IANB anesthesia can still give rise to potential risks for patients. Fortunately, most observed alterations are transient and self-limited. RESULTS: The complications of IANB anesthesia are varied in nature and could be specifically categorized into systemic, localized, and distant complications. When the complications occur around the orofacial struc

Lidocaine block of the recurrent laryngeal nerve in adductor spasmodic dysphonia: a multidimensional assessment.

OBJECTIVES/HYPOTHESIS: Lidocaine block of the recurrent laryngeal nerve (RLN) has been reported as a procedure for surgical selection of patients with adductor spasmodic dysphonia (ADSD). However, its effects on phonation have not been rigorously assessed in a prospective fashion using strict entry criteria and multiple measures of phonatory function. This investigation assessed the phonatory effects of RLN lidocaine block in ADSD to explore its potential as a diagnostic tool. STUDY DESIGN: Single group, pre/postexperimental trial. METHODS: Twenty-one consecutive patients with suspected ADSD

Treating the Broken Heart: Role of Stellate Ganglion Blockade in Takotsubo Syndrome.

Takotsubo syndrome (TTS) is an acute stress-induced cardiomyopathy characterized by transient left ventricular dysfunction, often mimicking acute myocardial infarction but without obstructive coronary artery disease. Although typically reversible, TTS can be complicated by serious adverse events, including life-threatening ventricular arrhythmias. The pathophysiology is complex but strongly linked to sympathetic nervous system hyperactivity and a surge in catecholamines, often termed "neurogenic stunned myocardium." This review discusses the role of arrhythmias in TTS, the influence of the aut
