The lack of a correct diagnostic and therapeutic planning of vestibular diseases is still often observed, and some difficulties are still to deal with in clinical practice, even when treating acute hearing problems, although the needed competence is more easily identified as otologic. A review of the international literature confirms the existence of such a problem, and permits to underline the scarcity of connections between otology and neurotology, on one hand, and principles of basic sciences and general and internal medicine, on the other hand: this can explain some therapeutic contradicti

PURPOSE OF REVIEW: This review considers recent observations on vestibular syncope in terms of clinical features, laboratory findings, and potential mechanisms.
RECENT FINDINGS: Vestibular syncope, potentially associated with severe fall-related injuries, may develop multiple times in about one-third of patients. Meniere's disease and benign paroxysmal positional vertigo are the most common causes of vestibular syncope, but the underlying disorders remain elusive in 62% of cases with vestibular syncope. The postictal orthostatic blood pressure test exhibits a lower diagnostic yield. Vestibula…

PURPOSE: To describe transient post-head-shaking nystagmus associated with horizontal canal benign paroxysmal positional vertigo (BPPV) and its resolution following canalith repositioning.
METHODS: A 78-year-old woman presented with positional vertigo. Video-oculography revealed left-beating pseudo-spontaneous nystagmus and a robust left-beating pHSN, initially suggestive of uncompensated unilateral vestibulopathy. Further bedside assessment, including the Head Pitch test and Supine Roll test, demonstrated direction-changing horizontal nystagmus consistent with left horizontal canalolithiasis…

Vertigo or dizziness is primarily caused by peripheral vestibular disorders, such as benign paroxysmal positional vertigo (BPPV) and vestibular neuritis. BPPV can be diagnosed from associated positional torsional or direction-changing horizontal nystagmus and can be treated with canalith repositioning procedures. In contrast, vestibular neuritis and other acute peripheral vestibulopathies can be diagnosed from associated unidirectional horizontal nystagmus. Evaluation of nystagmus is essential for the diagnosis of peripheral vestibular disorders. Vertigo/dizziness caused by disorders in the br…

OBJECT: The aim of this study was to evaluate the effectiveness of the Bow and Lean Test (BLT) based on nystagmus intensity and direction evaluation (NID-BLT) in the diagnosis of benign paroxysmal positional vertigo of the lateral semicircular canal (LSC-BPPV). We hypothesised that the presence of a pseudo-spontaneous nystagmus increases the diagnostic accuracy of the test.
METHODS: The BLT was performed in 32 subjects affected by LSC-BPPV.
RESULTS: BLT was positive in 27 cases with a sensitivity of 84.3% and an accuracy of 70.4%. In other words, it was possible to carry out a BPPV root cana…

Benign paroxysmal positional vertigo has typically been reported to be the most common cause of post-traumatic dizziness. There is however paucity in the literature about other peripheral vestibular disorders post-head injury. This article provides an overview of other causes of non-positional dizziness post-head trauma from our large institutional experience. The UHN WSIB Neurotology database (n = 4291) between 1998 and 2018 was retrospectively studied for those head-injured workers presenting with non-positional peripheral vestibular disorders. All subjects had a detailed neurotological hist…

This review of vestibular neurology for the general neurologist delves into the multifaceted realm of vestibular neurology where we address the diagnostic and therapeutic challenges associated with dizziness, vertigo and balance disorders. We outline the standard vestibular assessments that can be understood and incorporated by the generalist, discussing their use in common vestibular disorders. Key disorders covered include acute and chronic syndromes, benign paroxysmal positional vertigo, Meniere disease, vestibular migraine and persistent postural-perceptual dizziness. We also touch on emer…

We are reporting the case of a 52 years old male with a peripheral vertiginous syndrome which could have been diagnosed as a benign paroxysmal positional vertigo. In a craneocervical CT appeared an osteolytical lesion on C3 vertebral body. With this article we pretend to emphasize the importance of a complete exploration and the differential diagnosis in the peripheral vertiginous pathology.

Disorders affecting the peripheral vestibular system commonly involve the peripheral auditory system causing hearing loss. There are a number of disorders, however, that selectively involve the peripheral vestibular system causing dizziness without hearing loss. These disorders include benign paroxysmal positional vertigo, vestibular neuritis, recurrent vestibulopathy, familial vestibulopathy, and bilateral idiopathic vestibulopathy. This article reviews these disorders and their diagnosis and management.

This case series highlights the critical role of radiological imaging in patients presenting with positional vertigo initially suggestive of benign paroxysmal positional vertigo (BPPV) but ultimately attributable to central nervous system pathology. The objective is to raise awareness among clinicians and paramedical professionals regarding the importance of early imaging when atypical clinical features are present. We report 6 cases of central positional vertigo in patients aged 18-85 years (3 men and 3 women) initially referred for physiotherapy with a presumed diagnosis of BPPV. Detailed cl…

Balance disorders in the elderly often have several contributing causes. The search for these causes focuses on vision, proprioception, coor … es of vestibular dysfunction resulting in imbalance without vertigo: a bilateral vestibulopathy and benign paroxysmal positional vertigo. Symptoms, examination and treatment are discussed. All patients with imbalance should undergo a Head Impulse Test and Dix-Hallpike maneuver.

The vestibular disorders, which generally manifest themselves as nausea, vomiting, and dizziness or vertigo, have troubled many people. Increasing evidence suggests that certain vestibular disorders are hereditary, and gene variants contribute to the onset and susceptibility of these disorders. This review summarizes the latest research progress, especially the molecular and genetic aspects of several common vestibular disorders, including Meniere's disease, vestibular migraine, benign paroxysmal positional vertigo, and motion sickness, with the aim of encouraging more fundamental research and…
