One hundred and nineteen patients with gastrointestinal fistulas.

One hundred and nineteen patients with gastrointestinal fistulas were treated in the Massachusetts General Hospital, Boston, in the period from January 1960 to January 1970. None of these patients was hyperalimented. The mortality in this seris amounted to 15%; 78.2% of the patients had their fistulas closed. These results are correlated with primary disease, etiology, fistula output, fistula location, type of treatment, malnutrition, electrolyte disturbances and sepsis. In the discussion it is concluded that treatment based on sound surgical principles acquired in the past decades, with the s

Percutaneous management of bile duct injuries after cholecystectomy.

Intraoperative biliary tract injuries are relatively uncommon, but are a cause of significant morbidity and mortality. We have repaired open biliary tract injuries found postoperatively in three patients without reoperation by percutaneous intervention. In two cases, biliary stents were placed percutaneously across the injured portion of the bile duct. For this procedure, a unique coaxial guidewire technique was used. The bile leaks resolved soon after insertion of the biliary stents. In one patient, an expandable metallic biliary endoprosthesis was inserted, and the temporary stent was remove

Gallstone Ileus. What therapeutic options are there?

UNLABELLED: Gallstone ileus is a rare disorder in emergency surgical practice with diagnosis usually difficult and only achieved at surgery. The current approaches are: enterolithotomy, cholecystectomy and fistula repair (one-stage surgery), enterolithotomy with cholecystectomy performed later (two-stage surgery) and only enterolithotomy (most reported surgical procedure). METHODS: The clinical, operative and follow-up data on 14 consecutive patients treated in our clinic for gallstone ileus was retrospectively reviewed. RESULTS: Gallstone ileus was recorded in 0.06% of all operations for bi

[Congenital tracheobiliary fistula].

A case of congenital bronchobiliary fistula in a three-year old girl is reported. The patient was born after 40 weeks of gestation and referred due to a long history of respiratory tract problems. Bronchoscopy disclosed a third opening at the level of the carina. The opening was cannulated with an X-ray positive catheter, and a fistula crossing the diaphragm and communicating with the left hepatic duct was found. Cholescintigraphic examination (HIDA-scanning) confirmed the presence of the fistula, but found no evidence of abnormalities in the rest of the biliary system. The thoracic portion of…

The curious case of biliary emesis and bowel obstruction from Bouveret syndrome.

Bouveret syndrome is a rare complication of biliary lithiasis. This sequela is caused by the passage of the gallstone via a bilioenteric fistula, resulting in an impacted gallstone in the duodenum or stomach. The common presentation of non-specific symptoms contributes to the diagnostic uncertainty and delay, which is strongly associated with adverse outcomes. We report an uncomplicated stone extraction via open gastrotomy in an elderly man afflicted with bowel obstruction and biliary vomit secondary to Bouveret syndrome.

Nonoperative management of grade IV liver laceration following blunt abdominal trauma complicated with delayed bilioplueral fistula.

The incidence of bile leaks following blunt liver trauma ranges from 0.5% to 21%. Bile leaks could give rise to biliopleural fistula, which can end up causing cholethorax, where a bilious effusion is seen in the pleural cavity. Early recognition of this condition is essential for favorable outcome. We report an unusual case of delayed biliopleural fistula causing cholethorax in a 30-year-old male, presented following blunt abdominal trauma following a motor vehicle collision. Abdominal examination revealed tender right upper quadrant. He was haemodynamically stable following initial resuscitat

Secondary Choledocholithiasis in Obstructive Jaundice Patient due to Choledochoduodenal-fistula Stricture.

Choledochoduodenal fistula (CDF) is a rare condition marked by an abnormal connection between the biliary duct and duodenum. The common etiology of secondary CDF are cholecystolithiasis, tumor, and duodenal ulcer. CDF may also caused by prior inflammatory condition or as a complication of radiation therapy. Management for this case is based on the patient condition. Herein we aimed to present a case of secondary choledocholithiasis due to stricture in the CDF which presented with cholangitis treated by self-expanding metal stent (SEMS) for biliary drainage. Patient admitted with jaundice, feve

Gallstone ileus in a ninety-two years old colecistectomized patient after endoscopic biliary sphincterotomy: a case report.

INTRODUCTION: Gallstone ileus is an uncommon condition of mechanical bowel obstruction caused by the passage of a gallstone into the bowel. It occurs more frequently in female patients older than 65 years and often for a biliary-enteric fistula. The pathognomonic features of gallstone ileus - the Rigler's triad - are pneumobilia, ectopic gallstone and bowel obstruction. Less commonly, a gallstone may enter the intestinal lumen through the common bile duct, after endoscopic retrograde cholangiopancreatography, and very rarely in colecistectomized patient. CASE REPORT: A 92-year old colecistect

Implantation of migrated biliary stents in the digestive tract.

BACKGROUND: Biliary stents constitute an alternative for the palliative treatment of benign or malignant biliary obstruction, biliary strictures, choledocholithiasis, biliary fistulas from lateral lesions of the biliary duct or cystic duct leaks due to slippage of clip closure. Obstruction resulting in cholangitis is common. Proximal migration to the biliary duct or distal migration to the duodenum with subsequent passage per rectum are relatively frequent, but impaction and perforation of the bowel are rare. CASE OUTLINES: Two cases are reported. In one patient a migrated stent impacted in t…

Management of Complex Gallstone Disease With Bouveret's and Mirizzi Syndrome: A Case Report With Literature Review.

Gallstone disease affects up to 20% of adults, but rare complications include Bouveret's syndrome, occurring in approximately 0.05% of patients with gallstones, and Mirizzi syndrome, with Type Va involving concurrent biliary and enteric fistulae. We report the case of a 64-year-old man with concurrent Bouveret's syndrome and Csendes Type Va Mirizzi syndrome associated with a cholecystoduodenal fistula. An initial laparoscopic cholecystectomy was abandoned due to concern for malignancy. During the same admission, the patient developed gastric outlet obstruction, and imaging confirmed Bouveret's

[Massive hemobilia caused by necrotic hemorrhagic cholecystitis. Report of a case].

The authors report a case of massive hematobilia due to hemorrhagic cholecystitis. Hematobilia is a rare pathology which affects the biliary tract and gallbladder. The first authors to describe hematobilia defined it as a hemorrhage of the gastroenteric tract due to the communication of blood vessels with the intra and extra-hepatic biliary tract and in some rare cases to the communication of the branches of the cystic artery within the gallbladder wall. Sandblom, in particular, specified that bleeding must be within the biliary tract and not secondary to an enterobiliary fistula. In 55% of ca…

Toxic effect of streptozotocin on the biliary secretion of nicotinamide-treated rats.

The effect of streptozotocin (SZ) on bile flow (BF) and on protein and lipid biliary outputs were studied in rats with bile fistula. SZ was given i.v. as a single dose (50 mg/kg body wt.). Nicotinamide was administrated (500 mg/kg body wt., i.p.) 10 min prior to SZ. Decreases in BF and in biliary outputs of bile acids, proteins and acid phosphatase were observed in SZ-treated rats; conversely, the biliary excretion of cholesterol and phospholipids was increased. Nicotinamide pretreatment prevented the hyperglycemia induced by SZ and also suppressed the SZ-mediated increase of cholesterol and p
