Electromyographic biofeedback use in the treatment of voluntary posterior dislocation of the shoulder: a case study.

Voluntary posterior dislocation of the shoulder has been difficult to treat successfully. Electromyographic biofeedback is a nonoperative treatment measure that has been successfully used for this condition in three previously reported cases. The purpose of this article is to present a case history in which electromyographic biofeedback was used successfully as part of a nonoperative treatment program for voluntary posterior dislocation of the shoulder.

[The biological action of physical factors in the critical periods of embryogenesis].

The critical period in a chicken embryonic development (the 10-13-th days of incubation) is revealed under total electromagnetic radiation. This external factor is a physiologically active irritant which can influence functional state of the brain. The increased absorption of electromagnetic energy takes place in this incubation period. Its dynamics within 20 days of embryonic development has phasic, up and down character. Electromagnetic exposure (4 hours a day daily) in the above mentioned period evokes a delay in embryo adaptive motor behavior (biofeedback learning). Morphological investiga

A comparison of effectiveness of biofeedback and pelvic muscle exercise treatment of stress incontinence in older community-dwelling women.

BACKGROUND: Research using biofeedback as a treatment for sphincteric incontinence began with Kegel's early studies using a perineometer and pelvic muscle exercises demonstrating a 90% improvement in urine loss symptoms. More recent studies using varying combinations of biofeedback and pelvic muscle exercises found symptom reduction rates of 78% to 90%, but these studies lacked the rigor of a "phase three," or randomized controlled clinical trial. METHODS: A randomized controlled trial assessed the efficacy of biofeedback for older women for treatment of sphincteric incompetence. One hundred

Efficacy of feedback training in long-standing facial nerve paresis.

An efficacious treatment has not been available to patients with aberrant regeneration of the facial nerve as a result of Bell's palsy or after acoustic neuroma excision. This prospective controlled trial examines the efficacy of electromyographic feedback versus mirror feedback as treatment strategies for patients suffering from long-standing (18 months minimum) facial nerve paresis. Twenty-five patients were randomly assigned to electromyography with mirror feedback or mirror feedback alone. Seven rural patients who did not undergo treatment served as controls. At 0, 6, and 12 months, facial

[Correction of posture disorders and scoliosis in schoolchildren using functional biofeedback].

A novel technique is offered for correction of postural defects and early manifestations of scoliosis in schoolchildren using methods of functional biofeedback training. The technique allowed the patient to use the biofeedback signals informing on the degree of asymmetry in bioelectric activity of paravertebral muscles in static active straightening of the vertebral column. The data revealed a steady decrease in asymmetric activity indices in muscles erecting the column. This provided a physiological basis for a high clinical efficiency of the technique.

An experimental test of the muscle tension hypothesis of tension-type headache.

The present study employed an experimental design to provide a direct test of the classic etiological account of tension-type headaches, that these stem from elevated levels of muscle tension. Twenty-eight female subjects with relatively frequent headaches were divided into 4 groups, according to a 2 x 2 design. The independent variables were (1) Target response (to maintain elevated levels of either frontalis EMG or digital temperature for 40 min), and (2) Expectation (either specifically of a headache or of some unspecified discomfort). Dependent variables included headache as well as a numb

The gonadotrophin surge in humans: its mechanism and role in ovulatory function--a review.

The presence of the positive feedback by oestrogen resulting in the mid-cycle gonadotrophin surge in women distinguishes the female's feedback control of gonadotrophin secretion from that in males. The gonadotrophin surge is an event crucial for final oocyte maturation, ovulation and subsequently for corpus luteum function. In this article, the roles of oestrogen, progesterone and testosterone in the regulation of the gonadotrophin surge are reviewed. Evidences appear to support the suggestion that an oestradiol pulse of sufficient potency and lasting for an adequately long duration alone is s

Bell's palsy: management of sequelae using EMG rehabilitation, botulinum toxin, and surgery.

Fifteen percent of patients who have had an acute episode of Bell's palsy will be left with debilitating facial dysfunction. This chapter describes our approach to managing a variety of hypo- and hyperkinetic disorders caused by injury and faulty regeneration of the facial nerve, using electromyographic rehabilitation (EMGR) (13 patients), Oculinum toxin injection (14 patients), or surgical reanimation (72 patients). Improvement was noted after EMGR in 12 of 13 patients (92%), all 14 patients treated with Oculinum experienced temporary improvement, and improvement was noted in 66 of 72 patient

The efficacy of biobehavioral and compliance interventions in the adjunctive treatment of mild pregnancy-induced hypertension.

This investigation assessed the efficacy of a biobehavioral intervention in the adjunctive treatment of mild pregnancy-induced hypertension (PIH), a potentially serious complication of pregnancy in which normotensive women develop hypertension, proteinuria, and edema of unknown etiology late in gestation. Forty-five women with symptoms of PIH were randomly assigned to one of three treatment conditions: bed rest alone (the most common obstetrical treatment), bed rest with individualized compliance enhancement training, or a four-session biobehavioral treatment consisting of bed rest, compliance

Psychophysiologic response patterns in migraine before and after temperature biofeedback. Prediction of treatment outcome.

Psychophysiologic response patterns were studied in female migraine patients during headache-free periods (n = 16) and in controls (n = 8). Cardiovascular variables studied were heart rate (HR) and temporal artery pulse wave amplitudes (PWA). Phasic responses to 80-dB, 1000-Hz tones, each lasting 2 sec and sounded at irregular intervals, were studied within a habituation laboratory design. In addition, electromyographic (EMG) levels from the frontal muscles and hand temperature were measured. HR responses were less pronounced and PWA reduction more pronounced in migraine patients than in contr

Perceptual-physiologic approach to evaluation and treatment of dysphonia.

The value of a perceptual-physiologic approach to both the evaluation and treatment of voice disorders is described and illustrated with an example of a patient with ventricular fold phonation. Objective measures are obtained to facilitate the understanding of the underlying pathophysiology and to document change over time. Evaluation data are recorded and interpreted collectively by a speech-otolaryngology team and may be valuable for medicolegal cases. Finally, the evaluation methods, particularly videolaryngoscopy, may be used for biofeedback purposes to facilitate voice therapy.

Rehabilitation of conversion paralysis.

Three patients referred for rehabilitation of brachial plexus lesions and two referred with leg weakness associated with sciatica were found to have conversion paralysis. The diagnosis was made by demonstrating normal motor nerve conduction to the clinically weak muscles. The weakness was treated by intensive physical rehabilitation with complete and sustained recovery in all cases.

Biofeedback therapy for spastic dysphonia.

Spastic dysphonia is a disorder of phonation which is characterized by a strained, creaking, and choked vocal attack, a tense and squeezed voice sound. Spastic dysphonia in a functional voice disorder can be classified into two types from the viewpoint of activities of the extrinsic and intrinsic laryngeal muscle groups. A functional voice disorder pertaining to abnormal activities of the intrinsic and extrinsic laryngeal muscles results in spastic dysphonia. The adductor spastic dysphonia may be due to abnormal actions of the intrinsic laryngeal muscles as such is relieved by sectioning of th

Learned voluntary control of heart rate and rhythm in two subjects with premature ventricular contractions.

Two people with frequent premature ventricular contractions and bigeminal rhythm not associated with other signs of heart disease, learned to suppress the arrhythmia using a biofeedback technique. In both cases, physical exercise and atropine suppressed the arrhythmia, and in both the learned control was related to the development of tachycardia. In Case 1, it appeared to depend on something more than speeding alone, for sinus rhythm could be maintained during voluntary control at a lower heart rate than during physical manoeuvres such as exercise. In Case 2, the premature ventricular contract

Psychosocial treatments in pain management of sickle cell disease.

The principal symptom of sickle cell disease (SCD) is pain. Many studies have been conducted on pain management strategies for this illness. There is recognition that psychosocial factors influence clinical disease outcomes; therefore, more attention is being provided to behavioral interventions that address psychosocial problems. This review examines the psychosocial interventions that have been researched for children and adults with SCD, the limitations of these studies, and barriers to implementing the treatments. The intervention receiving the most empirical support was cognitive-behavior

Change mechanisms associated with combined relaxation/EMG biofeedback training for chronic tension headache.

Therapeutic mechanisms hypothesized to underlie improvements in tension headache activity achieved with combined relaxation and electromyographic (EMG) biofeedback therapy were examined. These therapeutic mechanisms included (1) changes in EMG activity in frontal and trapezii muscles, (2) changes in central pain modulation as indexed by the duration of the second exteroceptive silent period (ES2), and (3) changes in headache locus of control and self-efficacy. Forty-four young adults with chronic tension-type headaches were assigned either to six sessions of relaxation and EMG biofeedback trai
