Acute cortical blindness caused by neuropsychiatric systemic lupus erythematosus.

A man in his 30s, who presented with fevers and a diffuse purpuric rash, developed sudden-onset visual loss on day 2. He was unable to perceive light in either eye. Examination by a neurologist confirmed cortical blindness, and the MRI showed subtle juxtacortical infarcts and leptomeningeal enhancement in the occipital region. Further history taken in the patient's native language revealed a history of untreated systemic lupus erythematosus. A diagnosis of central nervous system lupus was made and he was treated promptly with pulse methylprednisolone and cyclophosphamide. His vision gradually …

Bilateral blindness after uneventful brolucizumab injection for macular degeneration.

BACKGROUND: We report a very severe case of bilateral panuveitis and ischemic vasculitis with possible perineural inflammation, which followed bilateral intravitreal brolucizumab administration in a patient with neovascular age-related macular degeneration (nAMD). CASE PRESENTATION: On December 11, 2020, a 81-year-old woman presented with severe bilateral loss of vision. Eight days earlier, she had received uneventful bilateral injection of brolucizumab, a novel anti-vascular endothelial growth factor (VEGF) single-chain variable region (scFv) recombinant protein drug, for treatment of neovas

tACS Stimulation at Alpha Frequency Selectively Induces Inattentional Blindness.

Contemporary neurocognitive models implicate alpha oscillations as a top-down mechanism of cortical inhibition, instrumental in the suppression of information that fails to reach conscious visual awareness. This suggests that alpha-band activity may play a key role in the phenomenon of inattentional blindness, however this has not yet been empirically examined. The current study employed transcranial alternating current stimulation (tACS) over occipital cortex at alpha, theta, and sham frequencies within an inattentional blindness task to delineate whether an exogenous manipulation of alpha os…

Blindness associated with platelet-rich plasma temporomandibular joint injections.

This case report highlights an ocular complication associated with platelet-rich plasma temporomandibular joint injections. This pioneering treatment can risk irreversible visual loss. This case highlights the importance of an experienced technique, in depth understanding of facial anatomy, and promptly recognising and referring the patient to a specialist to manage the complication should it arise.

Anton's syndrome: a rare and unusual form of blindness.

Anton syndrome is characterised by visual anosognosia. It results from damage to both occipital lobes, while the anterior visual pathways remain intact. We describe four cases of Anton's syndrome. First case is that of a 73-year-old woman, who presented with two separate events of intraparenchymal brain haemorrhage, 4 years apart. Her first stroke affected the left and second affected the right occipital lobe. Bilateral occipital lobe damage resulted in cortical blindness. Second case is an 88-year-old man, who suffered from two ischaemic strokes, 2 days apart. Each stroke involved one posteri…

Reversible Acute Blindness in Suspected Metformin-Associated Lactic Acidosis.

BACKGROUND: Metformin is commonly used for the treatment of type 2 diabetes mellitus. Renal insufficiency is one of the contraindications for its use. Inadvertent prescription in patients with renal insufficiency may lead to metformin-associated lactic acidosis (MALA), which is associated with a high risk of mortality. Consequently, the early recognition and management of MALA is essential. CASE REPORT: We present the case of a 68-year-old man who had reversible blindness resulting from severe lactic acidosis. On presentation, he was alert, oriented, and had no complaints except mild abdomina…

Rapid Assessment of Avoidable Blindness: looking back, looking forward.

The Rapid Assessment of Avoidable Blindness, or RAAB, is a relatively simple and low-cost survey methodology to provide data on the prevalence and causes of visual loss. The aim of this article is to reflect on the achievements and challenges of RAAB, and to describe the future developments that are needed to ensure that it remains a relevant and widely used tool. To date, at least 331 RAABs have been undertaken in 79 countries, and these surveys provide an important source of information on visual loss at both the local and global level. A RAAB repository has been developed which includes the…

Reversible Blindness as Presenting Manifestation of Severe Diabetic Ketoacidosis.

The presenting symptoms of diabetic ketoacidosis (DKA) include abdominal pain, polyuria and nausea. Diabetes has well known chronic ocular complications like glaucoma, cataracts and retinopathy. We report a case of reversible blindness as a presenting manifestation of DKA that has been reported in literature only 3 times previously. Our objective is to highlight a rare manifestation of a common disease. A 59-year-old male presented with painless vision loss for 3 days and was found to have DKA. The blindness was completely reversed with insulin and bicarbonate treatment. The dramatic presentat…

Macular sensitivity in patients with congenital stationary night-blindness.

AIM: To evaluate and correlate mean light sensitivity thresholds (MLST) in patients with congenital stationary night-blindness (CSNB) in comparison with healthy subjects using microperimetry (MP1). METHODS: Eleven patients with CSNB and 13 healthy subjects were compared. In all subjects, static threshold perimetry was performed using MP1 evaluating the central 6 mm of the retina. This central retinal area was divided into three rings through using the ETDRS grid algorithm with an innermost (1 mm), inner (3 mm) and outer ring (6 mm). The MLSTs were acquired in nine sectors of the ETDRS grid. A…

Charles Dickens, trachoma, and blindness in pre-Victorian England.

In the early 1820s, a Yorkshire boarding school was devastated by an outbreak of blinding ophthalmia. The cause of the epidemic was-in all likelihood-trachoma, then known as Egyptian ophthalmia. The headmaster of the Yorkshire school, William Shaw, was sued for gross negligence by 2 families whose sons went blind during the outbreak. The epidemic and trial would play a role in creating one of the literature's most notorious fictional characters. Eighteen years after the trial, Charles Dickens modeled the vile schoolmaster Wackford Squeers in Nicholas Nickleby after Shaw, whose reputation and c

Hyperbaric Oxygen Therapy to Avoid Blindness From Filler Injection.

The most serious complication of filler or fat injection is blindness. According to a recent systematic review of 98 patients of blindness provoked by filler or fat injection, only 2 patients had the outcome of a complete recovery of vision. In the literature, only 2 papers were found in which hyperbaric oxygen therapy (HBOT) was used in ophthalmic artery obstruction. However, no improvement of vision was obtained in either patient. Recently, the authors treated a patient who had central retinal vein occlusion and cilioretinal artery occlusion with HBOT (daily 2-hour sessions at 253 kPa for 14 …

Transient cortical blindness following vertebral angiography: a case report.

Transient cortical blindness (TCB) is a rare but well-known complication of cerebral angiography. Its pathophysiology remains uncertain. We would like to report a case of TCB in a patient during a follow up vertebral angiogram for post-coil embolization of left posterior inferior cerebellar artery aneurysm. Patient's vision was resumed spontaneously within 24 hours after angiography, with no residual neurological deficit in subsequent clinical follow up. Multi-modality imaging evaluation including vertebral angiography, brain CT and MRI performed on same day are presented.

A Patient With Word Blindness in the Seventeenth Century.

Johann Jakob Wepfer (1620-1695), city physician in Schaffhausen, Switzerland, published two books on "apoplexy." He proposed new ideas about the events in the brain during such attacks, based on Harvey's theory of the circulation of the blood. Wepfer postulated extravasation of whole blood or serum in the brain, in opposition to the Galenic notion of blocked ventricles. His case histories are remarkably precise and untainted by interpretation. This allows the recognition of a patient with word blindness, who was also unable to read words written by himself. Unlike patients with pure "alexia wi…

Unilateral blindness: a unique complication of choanal atresia surgery.

OBJECTIVE: To report a unique case of unilateral blindness secondary to transnasal endoscopic surgery and stenting for right choanal atresia. CASE REPORT: A 24-year-old man was referred with right eye blindness and acute headache, occurring immediately after transnasal endoscopic surgical repair of unilateral right choanal atresia with placement of an endonasal stent. Maxillo-facial computed tomography with three-dimensional reconstruction showed the endonasal stent entering the right nostril, passing through the lamina papyracea into the orbit and running anterior to the optic foramen toward…

Recurrent transient monocular blindness from stenotic central retinal artery.

Transient monocular blindness (TMB) is caused by a temporary reduction of blood flow to the retina or optic nerve. Even though embolism from the ipsilateral carotid artery has been considered the main mechanism of TMB, the vascular pathology remains unknown in many patients. A 42-year-old man presented with recurrent transient visual loss in the left eye for 2 months. The attacks tended to develop more frequently when his head was bent down. Fluorescence angiography during an attack revealed hypoperfusion of the left central retinal artery (CRA) and cerebral angiography documented a focal isol…
