Home blood pressure monitoring.

Blood pressure measurements made in the physician's office have been the standard for diagnosing hypertension and determining the efficacy of antihypertensive drug therapy. However, these measurements do not necessarily reflect a patient's usual blood pressures in other circumstances, such as at work or in the home. Accurate blood pressure measurements performed by the patient or a family member in an out-of-office setting can be integrated with office readings to assist in the management of hypertension.

Ambulatory blood pressure monitoring.

Non-invasive measurement of blood pressure in ambulatory humans began in the 1960s. Ambulatory devices have been modified over the years and are now pocket-sized, with almost noiseless pumps. Their accuracy must be validated by independent laboratories using a standardised protocol. Twenty-four-hour ambulatory monitoring offers considerable advantages over conventional measurement, including avoidance of error associated with conventional measurement, elimination of white-coat effect and the provision of a series of blood pressure readings over the time period rather than a one-off measurement…

Pretransplantation blood transfusion revisited.

BACKGROUND: Blood transfusion before organ transplantation has a beneficial effect on allograft survival; the mechanism of this effect has remained a mystery. In murine models, the presence of common histocompatibility antigens in the blood donor and the recipient favors the induction of allograft tolerance. METHODS: To investigate the effect of HLA compatibility between blood donor and recipient on the induction of allograft tolerance, we determined the relative frequency of cytotoxic T-lymphocyte precursors specific for donor cells before and at several times after blood transfusion in 23 p…

ABO incompatible blood transfusion.

Four cases of ABO incompatible blood transfusion are presented which occurred over a 3-year period and resulted in marked clinical variation. In each instance the error leading to the transfusion resulted from the incorrect identification of either the patient, the crossmatch sample or the donations. These cases not only highlight the problems in the clinical recognition of ABO incompatibility but indicate a need to shift emphasis from refining crossmatch methodology to the strict following of standard established protocols for transfusion. It is of paramount importance to guarantee the failsa…

Percutaneous umbilical blood sampling.

This report describes a technique for the performance of a sonographically guided percutaneous umbilical blood sampling and its potential uses in the management of diagnostic problems in the second and third trimester of pregnancy. The method has been employed in the prenatal assessment of 13 fetuses at risk for hemostatic failure (hemophilia and idiopathic thrombocytopenic purpura), chromosomal disorders, isoimmunization, and fetal hypoxia. This simple and rapid procedure offers access to the fetal circulation for diagnostic and therapeutic purposes.

Drug-induced blood disorders.

A case-history study of drug-induced blood disorders (pancytopenia, hemolytic anemia, thrombocytopenia, and granulocytopenia) was carried out at the Group Health Cooperative of Puget Sound, a health maintenance organization with more than 200,000 members, for the ten-year period from 1972 through 1981. During this time, only 26 instances of hospitalization for such disorders were thought to be definitely, probably, or possibly due to drugs other than antitumor agents. The rate was of the order of one per 100,000 person-years at risk. Only quinidine/quinine and sulfa-containing drugs were impli…

Blood induced secondary glaucomas.

Hyphema may lead to a secondary glaucoma in some patients. Six different mechanisms by which anterior chamber blood may cause elevated intraocular pressure are described and the treatment of these different conditions is discussed. It should be noted that more than 1 mechanism may be present simultaneously and that other mechanisms unrelated to the blood may also contribute to the problem.

Why measure blood barbiturates?

Phenobarbitone levels in the blood do not always provide a reliable guide in the management of the poisoned patient. They may rise in response to active treatment though the patient may be improving clinically. Some reasons for this are considered, with illustrations from three cases. It is concluded that the poor correlation of phenobarbitone levels with a patient's condition is due not to inadequate analytical techniques but to the poor representation of the "whole body" situation by a single blood specimen. The usefulness of analysing such specimens is questioned.
