Arterial pressure variations as parameters of brain perfusion in response to central blood volume depletion and repletion.

RATIONALE: A critical reduction in central blood volume (CBV) is often characterized by hemodynamic instability. Restoration of a volume deficit may be established by goal-directed fluid therapy guided by respiration-related variation in systolic- and pulse pressure (SPV and PPV). Stroke volume index (SVI) serves as a surrogate end-point of a fluid challenge but tissue perfusion itself has not been addressed. OBJECTIVE: To delineate the relationship between arterial pressure variations, SVI and regional brain perfusion during CBV depletion and repletion in spontaneously breathing volunteers. …

Prediction of intradialytic morbid events in hemodialysis patients by monitoring the second derivative of relative blood volume.

Intradialytic morbid events (IME) are a major issue affecting hemodialysis (HD) patients. Change in relative blood volume (RBV) reflects change in plasma refilling rate (PRR) and variability of ultrafiltration rate (UFR). However, degree of fluid overload influences this relationship. We propose a method using a second derivative of RBV (SDRBV) model to detect decrease in PRR during HD which is a major factor associated with IME. Forty-five HD patients (age 55.7±14 years, height 167.7±10 cm, Pre HD weight 79.3±16.6 kg) were studied with a total of 245 treatments, with IME occurring in 124 trea…

Dual action of NO synthases on blood flow and infarct volume consecutive to neonatal focal cerebral ischemia.

Research into neonatal ischemic brain damage is impeded by the lack of a complete understanding of the initial hemodynamic mechanisms resulting in a lesion, particularly that of NO-mediated vascular mechanisms. In a neonatal stroke rat model, we recently show that collateral recruitment contributes to infarct size variability. Non-specific and selective NO synthase (NOS) inhibition was evaluated on cerebral blood-flow changes and outcome in a P7 rat model of arterial occlusion (left middle cerebral artery electrocoagulation with 50 min occlusion of both common carotid arteries). Blood-flow cha

Blood cell counting in neonates: a comparison between a low volume micromethod and the standard laboratory method.

BACKGROUND: Iatrogenic anaemia caused by repeated blood sampling to monitor laboratory parameters can contribute, particularly in neonates, to the need for transfusion. "Point of care" laboratory equipment uses smaller amounts of blood for analytic determinations and could, therefore, help to prevent secondary anaemia. In this study we compared the results of haematological parameters measured using a standard laboratory method and using a "point of care" micromethod, with the aim of validating the use of this latter method in clinical practice in neonatology. MATERIALS AND METHODS: One hundr

Repetitive low-volume blood sampling method as a feasible monitoring tool in a mouse model of sepsis.

Blood-based monitoring of immunoinflammatory and organ function fluctuations is essential in models of critical illness. This is challenging in diseased mice as repetitive blood collection may be harmful and/or affect end points. We studied the influence of daily sampling in acutely septic (days 1-5) mice upon survival and selected hematologic and organ function parameters. In addition, we tested the reliabilty of complete blood cell (CBC) count using resuspended blood cells. Female OF-1 and CD-1 mice underwent cecal ligation and puncture (CLP) and were subdivided into Daily and Day 5 groups.

Intrathoracic blood volume as an end point in resuscitation of the severely burned: an observational study of 24 patients.

BACKGROUND: Treatment of burn shock according to empirical resuscitation formulas is still considered the gold standard, and the burn commun … hnique was used for semi-invasive hemodynamic monitoring. The clinical utility of the intrathoracic blood volume (ITBV) as an end-point variable for fluid resuscitation was evaluated, comparing correlation of filling pressure obtained by a pulmonary artery catheter and intrathoracic blood volume to cardiac index and oxygen delivery. In addition fluid volume pr …

[Possibilities of the use of MR tomography-based cerebral blood volume maps in the diagnosis of brain tumors].

PURPOSE: Today contrast enhanced MR imaging is a reliable method for detecting mostly distinguishing between different histological types of tumours. In this study we use a MR-based method to measure the regional cerebral blood volume (rCBV). Using this technique we try to judge the grading and vitality of the tumours. METHODS: 26 patients with various types of brain tumours were examined. To calculate rCBV-maps of one slice, low-dosed Gd-DTPA was injected as a bolus. Using the relaxation effect the obtained signal intensity-time curves were converted pixel-wise into rCBV images. For the tumo…

Effects of ventilation and pleural effusion on measurements of airway thermal volume and blood flow in dog lungs.

We studied the effects of ventilation and pleural effusion on measurements of airway thermal volume (ATV) and pulmonary blood flow (PBF) by using the airway gas thermometry method of V. B. Serikov, M. S. Rumm, K. Kambara, M. I. Bootomo, A. R. Osmack, and N. C. Staub (J. Appl. Physiol. 72: 944-953, 1992) in 39 anesthetized dogs with or without lung edema or pleural effusion. To examine the differential effects of increased-pressure and increased-permeability lung edema on accuracy and sensitivity of ATV and PBF, two models of lung edema were induced by intravenous infusion of a Dextran 70 solut

[A study on postoperative management and pre-and postoperative changes of blood volume in orthopedic surgery (author's transl)].

In order to apply adequate postoperative management of patients in the orthopedic surgery, the in vivo effects of the operations were investigated in the present study, from various viewpoints such as circulatory functions, metabolism of proteins, and hepatic and renal functions. In addition, the circulating blood volumes were determined in order to know the variations in the amount of blood loss from the operated regions during and after operations so as to improve the operative and postoperative management. On the basis of the results, the countermeasures against the adverse variations in bl…

Change in the renin dependency of blood pressure induced by volume depletion and/or propranolol therapy in hypertensive patients.

1. Plasma renin activity (PRA) and renin dependency of the blood pressure was analysed in ten patients with various forms of hypertension before and during treatment with volume depletion and/or propranolol. Renin dependency was tested by infusion of the specific competitive angiotensin II antagonist Sar1-Ala8-angiotensin II (P113). 2. The P113-induced fall of the blood pressure did correlate with the log PRA (r=0-888, P less than 0-001). This correlation was found irrespective of different types of hypertension and treatment schedules. 3. During volume depletion, PRA was stimulated and renin

Determination of the Minimum Discard Volume in the Extraction of Blood Samples Through Arterial Catheters in Critical Patients.

BACKGROUND: The critically ill patient requires multiple extractions through catheters, implying a substantial blood loss. The discard method continues to be a commonly used method in Intensive Care Units. However, there is no homogeneity in practice. AIM: Determine the minimum blood discard volume to obtain samples through radial arterial catheter. STUDY DESIGN: Quasi-experimental, prospective, cross-sectional study employing the STOBE checklist. Four consecutive arterial blood gases were performed considering the flush volume (sample A with 1 dead space discarding 1 mL, sample B with 2 dis

Survival and Early Ultra-massive Transfusion: High Red Blood Cell Volumes Are Not Uniformly Futile.

INTRODUCTION: Massive transfusion protocols for hemorrhagic shock in trauma are instrumental in improving patient outcomes. However, blood products are limited resources. We hypothesized that adult trauma patients receiving early ultra-massive transfusion (UMT) would have higher mortality compared to those receiving non-UMT, but there would be no strict transfusion volume threshold indicating uniform futility. METHODS: Using the Trauma Quality Improvement Program database from 2017 to 2021, we compared mortality and outcomes between adult (≥ 18 y) trauma patients who underwent early UMT (≥ 20
