Craniofacial abnormalities induced by the ectopic expression of homeobox genes.

In this paper I have tried to bring together work that highlights the role of homeobox genes in generating craniofacial form. I review both normal and disrupted embryogenesis and ask whether mis-expression of the homeobox genes outside their normal domains could be contributing to congenital facial abnormalities arising from either genetic or teratogenic actions. Experimentally generated transgenic mice carrying loss- or gain-of-function mutations in homeobox genes, in combination with their normal expression patterns, have allowed us to compile and test models of embryonic specification based

Gene trap insertion into a novel gene expressed during mouse limb development.

Gene trapping is a useful method to identify new genes involved in development. Here we describe the spatiotemporal expression of a gene identified in a gene-trap screen. This gene is first expressed at 9.5 days postcoitum (E9.5) in the forelimbs and in the branchial arches region. At E11.5, expression was detected in the stomach, genital bud, and pharyngeal epithelium. At later stages, expression includes the hair follicles, whereas the expression in the stomach and pharynx disappears. We performed 5'-rapid amplification of cDNA ends (RACE) to amplify and clone a partial cDNA of the endogenou

[Accessory external ear in the oropharynx].

This report describes the case of a one-day old girl with an accessorial ear lobe in the region of the left soft palate. The girl was referred to us by the children's clinic because of shortness of breath and difficulties in swallowing. On inspecting the cavity of the mouth and the pharynx a pedunculated, ball-shaped, whitish, membranous tumour was identified. The size of the tumour was comparable to a cherry, hanging down from the left velum into the pharynx. It was possible to remove the tumour from its base by using the sling technique. The operation itself did not present any complications

Multicentricity of Type II First Branchial Cleft Anomalies.

OBJECTIVES: First branchial cleft anomalies (FBCAs) are the result of misguided embryologic development of the external auditory canal, resulting in a partial duplication of the structure. A recent consensus statement by the International Pediatric Otolaryngology Group (IPOG) reclassified lesions based on anatomic location rather than the classic germ cell layer classification. Type I FBCAs are external and superficial to the parotid fascia, and type II FBCAs are juxtaposed to, or involving, the parotid gland. The consensus statement supports extensive resection to prevent recurrence. We hypot

Imaging features and therapeutic approaches for congenital branchial anomalies in 1668 pediatric cases.

PURPOSE: To summarize the imaging features of congenital branchial anomalies (CBA) in children based on a large single‑center cohort, propose cohort-based stratified diagnostic criteria and individualized surgical strategies, and provide a reference for the precise clinical management of pediatric CBA. METHODS: A retrospective study analyzed 1,668 pediatric CBA at the Children's Hospital of Zhejiang University between January 2017 and December 2024. Data regarding gender, age, lesion laterality, infection history, lesion type, imaging features, and treatment regimens were summarized and evalu

[Transoral resection of partial fistula wall to treat incomplete second branchial fistula: a case report].

We describe a case of a 55-year-old man who presented with sore throat for two days, while neck swelling for one day, and was found to have a fistula in his left tonsil and an abscess in his left lateral pharyngeal wall with the lower bound to the upper border of the cricoid cartilage by ultrasonography and enhanced CT. The fistula from tonsillar fossa to hypopharynx was detected followed by left tonsillectomy, and then anterior wall of the fistula and mucosa covering it was resected. He was eventually diagnosed with incomplete second branchial fistula with infection, and was followed up for f

Endoderm-specific deletion of Tbx1 reveals an FGF-independent role for Tbx1 in pharyngeal apparatus morphogenesis.

BACKGROUND: The T-box transcription factor Tbx1, is essential for the normal development of multiple organ systems in the embryo. One of the most striking phenotypes in Tbx1-/- embryos is the failure of the caudal pharyngeal pouches to evaginate from the foregut endoderm. Despite considerable interest in the role of Tbx1 in development, the mechanisms whereby Tbx1 controls caudal pouch formation have remained elusive. In particular, the question as to how Tbx1 expression in the pharyngeal endoderm regulates pharyngeal pouch morphogenesis in the mouse embryo is not known. RESULTS: To address t

The zebrafish van gogh mutation disrupts tbx1, which is involved in the DiGeorge deletion syndrome in humans.

The van gogh (vgo) mutant in zebrafish is characterized by defects in the ear, pharyngeal arches and associated structures such as the thymus. We show that vgo is caused by a mutation in tbx1, a member of the large family of T-box genes. tbx1 has been recently suggested to be a major contributor to the cardiovascular defects in DiGeorge deletion syndrome (DGS) in humans, a syndrome in which several neural crest derivatives are affected in the pharyngeal arches. Using cell transplantation studies, we demonstrate that vgo/tbx1 acts cell autonomously in the pharyngeal mesendoderm and influences t

Molecular dissection of craniofacial development using zebrafish.

The zebrafish, Danio rerio, is a small, freshwater teleost that only began to be used as a vertebrate genetic model by the late George Streisinger in the early 1980s. The strengths of the zebrafish complement genetic studies in mice and embryological studies in avians. Its advantages include high fecundity, externally fertilized eggs and transparent embryos that can be easily manipulated, inexpensive maintenance, and the fact that large-scale mutagenesis screens can be performed. Here we review studies that have used the zebrafish as a model for craniofacial development. Lineage studies in zeb

[Airway obstruction after general anesthesia in a patient with the first and second branchial arch syndrome].

A 20-year-old female with the first and second branchial arch syndrome underwent plastic surgery. Due to her mandibular hypoplasia and trismus, tracheal intubation was difficult even with a broncho-fiberscope. Fentanyl 100 micrograms was used for nasal intubation. Anesthesia was maintained with nitrous oxide (4 l.min-1), oxygen (2 l.min-1) and sevoflurane (0.5-3%). There were no troubles during surgery. The trachea was extubated after recovery of consciousness and spontaneous respiration. Five minutes after extubation, airway was obstructed suddenly after massive nasal bleeding. Sudden massive

Epicardial outgrowth inhibition leads to compensatory mesothelial outflow tract collar and abnormal cardiac septation and coronary formation.

In the present study, we investigated the modulatory role of the epicardium in myocardial and coronary development. Epicardial cell tracing experiments have shown that epicardium-derived cells are the source of interstitial myocardial fibroblasts, cushion mesenchyme, and smooth muscle cells. Epicardial outgrowth inhibition studies show abnormalities of the compact myocardial layer, myocardialization of cushion tissue, looping, septation, and coronary vascular formation. Lack of epicardial spreading is partly compensated by mesothelial outgrowth over the conotruncal region. Heterospecific epica

A new in utero model for lateral facial clefts.

The etiopathogenesis behind the formation of atypical craniofacial facial clefts remains unknown. To test the hypothesis that physical restricting forces such as amniotic bands can lead to the formation of these unusual clefts in the postorganogenesis period, we have modified a previously reported fetal lamb model of amniotic band syndrome to examine the effects of these bands on craniofacial development. Five 70-day gestation fetal lambs (term, 140 days) were exposed via a maternal hysterotomy. In each animal, an attempt was made to create a lateral craniofacial cleft by applying a 2-0 nylon

Persistence of the stapedial artery in a first arch anomaly: a case report.

This is a case report of a three month old girl with a unilateral facial dysplasia in whom the rudiments of external auditory organs were displaced towards the zygomatic region. The carotid arteriography demonstrated a persistent stapedial artery and absence of some branches of the external carotid artery (the middle meningeal artery and the superficial temporal artery). The autopsy study, undertaken at six months of age, confirmed the presence of the vascular anomaly and severe defects in the auditory organ. There was nerve tissue in the facial mass. This is the first reported case in which a

Clinical Characteristics of Branchial Cleft Anomalies.

BACKGROUND AND PURPOSE: Branchial cleft anomalies (BCA) are a group of relatively common congenital pediatric neck masses, which exhibit wide variability in clinical presentation and treatment approaches. This study attempts to fill this gap by evaluating BCA clinical, radiographic, and treatment features in a large cross-sectional cohort. MATERIALS AND METHODS: We performed a retrospective cross-sectional study of patients with BCA presenting to a single pediatric quaternary care center between 2017 and 2023. The radiology information system was queried for the term "branchial," with diagnos

A case report of bilateral cervical chondrocutaneous remnants with review of the literature.

Cervical chondrocutaneous remnants are very rare entities. They are thought to originate either from the second branchial arch or from auricular tissues. To date, less than 40 cases have been reported in the medical literature, and only 7 cases were bilateral. We report the case of a 1-month-old girl presenting with bilateral neck lesions since birth, with no other anomalies. Complete surgical excision was performed, the pathology of which confirmed the diagnosis of chondrocutaneous remnants. Follow-up after 9 months showed no clinical evidence of complications or recurrence.
