Bronchiolitis obliterans organizing pneumonia in common variable immunodeficiency syndrome.

A 26-year-old woman with common variable immunodeficiency syndrome (CVID) associated with frequent episodes of pneumonia underwent an open lung biopsy showing bronchiolitis obliterans organizing pneumonia (BOOP). Following corticosteroid therapy, there were no further episodes of pneumonia. In addition, there was roentgenographic and gas exchange improvement. This is the first case of BOOP in association with CVID. An immunologic basis of BOOP is postulated.

[A case of follicular bronchiolitis with broncho-pleural fistula in rheumatoid arthritis].

A 67-year-old male diagnosed clinically as having rheumatoid pleuritis and bronchiolitis was treated with adrenocorticosteroid. His clinical findings improved, but following the tapering of the steroid dose, exacerbation occurred. After the steroid dose was increased, serological findings improved, but chest X-ray findings revealed no improvement. To re-evaluate the etiology of the bronchiolar lesion, open lung biopsy was performed. The biopsy specimen showed lymphocytic infiltration and formation of lymphoid follicles in and around the bronchioles. The pulmonary lesion was diagnosed as follic…

Effect of ipratropium bromide on respiratory mechanics in infants with acute bronchiolitis.

The effect of nebulized ipratropium bromide in 14 infants (mean age: 20 weeks, range: 4-41) with acute respiratory syncytial virus bronchiolitis was examined. A modified rapid chest compression technique was used to obtain partial expiratory flow-volume curves and maximum flow at functional residual capacity. Passive respiratory mechanics were assessed by brief occlusion at end inspiration. Thoracic gas volume was measured in a body plethysmograph. No significant difference was found in forced and passive respiratory mechanics pre- and post-ipratropium bromide. No subgroups could be identified

Respiratory problems 2 years after acute bronchiolitis in infancy.

We assessed the clinical progress of 55 children 2 years after admission to hospital with acute bronchiolitis and performed lung function tests on 40. During the 2 year follow up period 75% of the children had wheezed, 36% had 2 or more lower respiratory symptoms lasting more than 2 weeks, 33% had more than 100 days of lower respiratory symptoms, and 13% were readmitted to hospital with acute respiratory disease. In addition 60% of the children were hyperinflated on lung function tests. Many of the children with hyperinflation at the 2 year assessment had not been hyperinflated 1 year earlier,

Bronchiolitis interstitial pneumonitis: a pathologic study of 31 lung biopsies with features intermediate between bronchiolitis obliterans organizing pneumonia and usual interstitial pneumonitis, with clinical correlation.

Bronchiolitis combined with interstitial pneumonitis generally has been equated with bronchiolitis obliterans organizing pneumonia (BOOP). We describe our experience with lung biopsies that had both bronchiolar and interstitial diseases. We studied 31 patients who had respiratory difficulty leading to open lung biopsy, which showed a combination of both prominent bronchiolitis and prominent interstitial pneumonitis. We compared these cases clinically and pathologically with 6 other pulmonary diseases, namely, bronchiolitis obliterans, BOOP, nonspecific interstitial pneumonitis, usual interstit

Electrolyte Testing Variability Among Critically Ill Children With Viral Bronchiolitis.

Background Viral bronchiolitis is the most common indication for pediatric hospitalization for children under 12 months, and admissions for critical bronchiolitis and associated hospital charges continue to rise. Despite this, electrolyte testing practices among children admitted to pediatric intensive care units with bronchiolitis are not well described. This study evaluates the frequency of electrolyte testing and the extent of variation in testing practices across U.S. pediatric ICUs (PICUs). Methods We conducted a multicenter, retrospective cohort study of children admitted to PICUs with v

Bronchiolitis Post Introduction of RSV Immunoprophylaxis: Shifting Patterns in Pediatric Hospitalizations.

Bronchiolitis is the leading cause of hospitalization in infants under 12 months of age, most often due to respiratory syncytial virus (RSV). Following pandemic-related seasonal disruptions and the introduction of RSV immunoprophylaxis, monitoring bronchiolitis trends is essential for public health planning and resource allocation. We conducted an institutional review board approved retrospective cohort study of children aged ≤ 24 months who presented to a tertiary children's hospital with bronchiolitis during RSV seasons (October-April) from 2018-2019 through 2024-2025, excluding 2020-2022 du

Examining Disparities in Pediatric Asthma, Pneumonia, and Bronchiolitis Care in General Hospitals.

BACKGROUND/OBJECTIVE: Prior disparity research has focused on evaluating patient outcomes in freestanding children's hospitals, rather than identifying potential drivers of these disparities. Our objective was to evaluate drivers of disparities in respiratory illness care quality and outcomes in a national sample of general hospitals. METHODS: This retrospective study used pretrial data from hospitals in the SIP trial (Simultaneously Implementing Pathways for Improving Asthma, Pneumonia, and Bronchiolitis Care for Hospitalized Children, NCT05206695). Patients were eligible if they had a prima

Utility of Lung Ultrasound Assessment in Infants with Acute Bronchiolitis: An Observational Study.

OBJECTIVES: To assess the utility of Lung ultrasound (LUS) in predicting clinical outcomes and disease severity in infants with acute bronchiolitis. METHODS: Infants with acute bronchiolitis underwent clinical assessment using the ReSVinet score and LUS to assess B-lines, intercostal space involvement, and sub-pleural consolidations. Nasopharyngeal swabs identified the viral etiology. Outcomes including ICU admission, length of hospital stay, and oxygen/advanced respiratory support were recorded and analyzed. RESULTS: One hundred (100) infants were enrolled with a mean age of 8.04±6.5 mo. On

Association of Serum Eosinophilic Cationic Protein With Disease Severity in Infant Bronchiolitis.

OBJECTIVES: This study aimed to evaluate serum eosinophilic cationic protein (ECP) levels in young children with acute bronchiolitis compared with healthy controls and to investigate their association with clinical severity and tobacco smoke exposure. We hypothesized that ECP would not function as a diagnostic biomarker but might reflect inflammatory phenotypes associated with severe disease. METHODS: In this prospective cross-sectional study, 96 children aged 1 to 24 months with acute bronchiolitis and 96 age-matched and sex-matched healthy controls were enrolled. Serum ECP levels and routin

Standardized Respiratory Weaning Protocol to Improve Bronchiolitis Length of Stay and Costs.

Background and objectives High-flow nasal cannula (HFNC) is widely used in the management of bronchiolitis. However, variation in weaning practices may contribute to prolonged hospitalization. At our institution, the average length of stay (LOS) for bronchiolitis admissions was 3.54 days, exceeding the national average. We hypothesized that implementation of a standardized weaning protocol would reduce LOS and associated hospital costs. Our primary aim was to decrease hospital LOS by 20% among patients <24 months of age admitted with bronchiolitis over the course of one year. A secondary objec

Respiratory support in paediatric patients with bronchiolitis: a systematic review and meta-analysis.

BACKGROUND: Recent evidence suggests that high-flow nasal cannula (HFNC) may confer better clinical outcomes compared with other respiratory support modalities in paediatric patients with bronchiolitis. METHODS: We performed a literature search up to 21 January 2024 on PubMed, Scopus, Cochrane Library, Embase, Cumulative Index to Nursing and Allied Health Literature, World Health Organization International Clinical Trials Registry Platform and Clinicaltrials.gov. Randomised controlled studies including paediatric patients aged ≤24 months with bronchiolitis comparing the use of HFNC versus low

Prone Positioning in Infants With Acute Bronchiolitis: The PROPOSITIS Randomized Clinical Trial.

IMPORTANCE: Prone positioning has been shown to improve respiratory mechanics and oxygenation, but its clinical benefit in infants with acute viral bronchiolitis receiving high-flow nasal cannula (HFNC) support remains unknown. OBJECTIVE: To investigate whether prone positioning in infants with moderate to severe acute bronchiolitis and HFNC support reduces escalation to noninvasive or invasive ventilation. DESIGN, SETTING, AND PARTICIPANTS: Multicenter, randomized, open-label trial conducted in 15 pediatric intermediate or intensive care units in France. Infants aged 6 months or younger adm

The Persistent Respiratory Impact of Respiratory Syncytial Virus (RSV) Bronchiolitis in Infants.

INTRODUCTION: Respiratory syncytial virus (RSV) bronchiolitis during infancy has the potential to progress to recurrent wheezing and asthma. However, studies evaluating lung function using tidal breath analysis in this age group are limited. OBJECTIVE: This study aimed to assess lung function in infants hospitalized during their first episode of acute RSV bronchiolitis using tidal breath analysis, to compare tidal breathing parameters between the acute phase and 2 months after clinical recovery, and to evaluate the impact of bronchiolitis severity on lung function. SUBJECTS AND METHODS: A to

Effects of implementing a clinical scoring system on the management of infants with RSV bronchiolitis.

INTRODUCTION: Respiratory syncytial virus (RSV) is a common pathogen of lower respiratory tract infections in infants 1 month to 1 year of age. Infections with RSV are the most common cause for hospitalization for respiratory infections and present with a high rate of complications. Different scores for severity assessment currently exist but only few of them are well validated. The "Inpatient clinical pathway for evaluation/treatment of children with bronchiolitis" from the Children's Hospital of Philadelphia (CHOP) provides clear and concise guidance for severity grading and inpatient manage
