Studies have shown that a permanent left bundle branch pacing (LBBP) is feasible and may be clinically beneficial. The paced QRS morphology in a permanent LBBP typically manifests as a right bundle branch block pattern (RBBB) because the left ventricle is activated earlier than the right ventricle. We present one such case in which a small change in the pacing rate strongly influenced the LBBP QRS morphology. A 71-year-old man diagnosed with sick sinus syndrome had a dual-chamber pacemaker (LBBP) implanted. During the transition from non-selective LBBP to selective LBBP, a shrinking R' wave of

A 57-year-old male suffering from cardiogenic syncope was found to have preexcited QRS on surface ECG at admission. A dual-chamber ICD was implanted after discovering intermittent high degree A-V block and ventricular tachycardia during hospitalization. An EP study was performed 2 days later. Fasciculoventricular accessory pathway was diagnosed based on the fixed H-V interval with different A-H interval when atrial activation conducted to ventricle. However, the H-V interval was normal, which can be explained by intra-His block based on the findings of two split His potentials, the second of w

BACKGROUND: The evaluation of the QT interval in the presence of left bundle branch block (LBBB) is associated with the challenge to discriminate native QT interval from the prolongation due to the increase in QRS duration. The newest formula to evaluate QT interval in the presence of LBBB suggests: modified QT during LBBB = measured QT interval minus 50% of LBBB duration. The purpose of this study is therefore to validate the abovementioned formula in the clinical setting.
METHODS: Validation in two separate groups of patients: Patients who alternated between narrow QRS and intermittent LBBB

Electrocardiograms were taken from an 84-year-old man with right bundle branch block in whom atypical atrioventricular Wenckebach periodicity was frequently occurred. The electrocardiographic findings as mentioned below suggested that the atypical periodicity was caused by conduction through triple atrioventricular junctional pathways as a probable mechanism. When a P wave was blocked after a markedly prolonged PR interval of 0.64 s, the RP interval containing this blocked P wave ranged between 0.84 s and 0.86 s, and the next P wave was followed by a QRS complex of the same configuration, with

The electrocardiographic criteria for isolated left anterior fascicular block are reviewed and illustrated. Left anterior fascicular block decreases the voltage in the chest leads and increases the voltage in the limb leads. The usual voltage criteria of left ventricular hypertrophy must be modified appropriately. Changes in repolarization include a decrease in the T wave of leads I and AVL and an increase in leads II, III, AVF, V5 and V6. Small Q waves in V2 may simulate an anteroseptal myocardial infarction. Three criteria for the possible diagnosis of superimposed inferior myocardial infarc

Complete heart block was found shortly after birth in a brother and sister (not twins). Both were treated by electronic pacing because of symptoms attributable to inadequate cardiac output and electrical instability of heart. The boy has done well with his artificial pacemaker and is now six years old. His sister died of complications due in part to the large size of her pacemaker and small size of her body. At necropsy special studies of her heart included the centers for normal impulse formation and concuction. The primary abnormalities were at the junction of atrial septum with atrioventric

INTRODUCTION: Recent studies have shown that His-bundle pacing could be an alternative in patients requiring cardiac resynchronization therapy as it is comparable or better in terms of amelioration of ventricular activation, narrowing of the QRS complex, or clinical outcomes. However, in case of high threshold at the level of His-bundle or inability to correct conduction through a diseased His-Purkinje system other option should be searched like left bundle pacing.
PATIENT CONCERNS: A 77-year-old man presented to the Emergency Department for dizziness and dizziness and lightheadedness due to

His bundle pacing was developed while seeking a physiological alternative to biventricular cardiac resynchronization therapy. However, His bundle pacing may not be adequate in all patients. In this scenario, left bundle branch pacing has arisen as a new cardiac resynchronization therapy modality to correct left bundle branch block and restore ventricular synchrony. (Level of Difficulty: Intermediate.).

Correlations of the His to ventricular (H-V) conduction time were made with the surface electrocardiogram during normal intraventricular conduction, unifascicular block (right bundle branch block), bifascicular block (left bundle branch block) and trifascicular block (right and left bundle branch block) in a patient with rate-dependent left bundle branch block who had transient right bundle branch block during recording of the His bundle electrogram. The results provide a functional confirmation of the theory that a prolonged H-V time is a manifestation of trifascicular disease.

OBJECTIVE: To illustrate the automatic modulation of refractoriness of His-Purkinje system during atrioventricular nodal reentrant tachycardia (AVNRT) and to discuss the possible mechanisms.
METHODS: Programmed electrical stimulations were performed in high right atrium (HRA) in 8 patients with AVNRT before ablation to induce tachycardia and electrocardiagraphic recordings were done synchronically when AVNRT appeared.
RESULTS: All the patients had 2:1 atrioventricular (A-V) conduction when AVNRT began, 2 of whom were blocked below His bundle, 5 above His bundle and 1 unclear. After a duratio

Paroxysmal tachycardia with widened QRS complexes was recorded in a 21-year-old man. In sinus rhythm there was no evidence of pre-excitation. His bundle studies revealed an abnormally short HV interval of 30 ms. Premature atrial stimuli produced an increased PR interval. At short coupling intervals the His bundle activity became incorporated within the QRS complex. Concurrently, a left bundle-branch block pattern appeared identical to that seen during tachycardia. Closely coupled ventricular extrastimuli initiated a tachycardia identical to the initial episode. A re-entry mechanism via anterog

BACKGROUND: Ablation of para-Hisian accessory pathway (AP) poses high risks of atrioventricular block. We developed a pacing technique to differentiate the near-field (NF) from far-field His activations to avoid the complication.
METHODS AND RESULTS: Three-dimensional mapping of the right ventricle was performed in 15 mongrel dogs and 23 patients with para-Hisian AP. Using different pacing outputs, the NF- and far-field His activation was identified on the ventricular aspect. Radiofrequency application was delivered at the NF His site in 8 (group 1) and the far-field His site in 7 dogs (group

In this report, we have outlined our experiences with the direct surgical treatment of 16 patients with supraventricular tachyarrhythmias (SVTA). Re-entry supraventricular tachycardia (SVT) was demonstrated by the 4 patients who had retrograde-conducting Kent pathways. The SVT was corrected by the successful division of the Kent bundle in 3 and a partially successful His bundle division in one patient. The remaining 9 patients with re-entry, which included 6 with Kent pathways and WPW, all had His bundle section. The remaining 3 with His interruption had focal tachycardias associated with dysf

A 70-year-old man with a history of prior ablation for atrial fibrillation presented with atrial tachycardia (AT). Electrophysiological studies revealed roof-dependent and localized-reentrant ATs. During each AT, widespread breakthrough activation was observed just anterior to the low-voltage area on the anterior wall, suggesting that Bachmann's bundle served as an epicardial bypass within the tachycardia circuits. This case highlights the potential role of Bachmann's bundle in the development of ATs in the remodeled left atrium.

Idiopathic left ventricular tachycardia is macro-reentrant tachycardia involving the fascicles in the left ventricle as a part of its reentrant circuit. The detailed circuit mechanisms somewhat remain unclear. We reported QRS and cycle length alternans confirmed after the first application of radiofrequency delivery for the distal site of left posterior fascicle potential (P2) in a patient with idiopathic left ventricular tachycardia.
