Risk factors of mortality among pediatric burn patients: a systematic review and meta-analysis.

BACKGROUND: Pediatric burn injuries remain a significant cause of morbidity and mortality worldwide. Identifying risk factors associated with mortality in this vulnerable population is critical for improving clinical outcomes and guiding preventive strategies. This systematic review and meta-analysis aimed to synthesize existing evidence on the risk factors of mortality among pediatric burn patients. METHODS: A comprehensive search was conducted across PubMed, Embase, Web of Science, and Cochrane Library databases from inception to December 2024. Two reviewers independently screened studies,

Does High-Frequency Chest Wall Oscillation Have an Impact on Improving Pulmonary Function in Patients With Smoke Inhalation Injury?

Smoke inhalation results in bronchospasm of the trachea, increasing secretion of mucus, casts formation, and improvement of blood flow of the airway. High frequency chest wall oscillation is a common modality used for clearing mucus secretion in patients suffering from hypersecretion of thick mucus and used also to help cough clearance. This study aimed to detect the effect of high frequency chest wall oscillation in improving pulmonary function in burn patients suffering from smoke inhalation. Sixty smoke inhalation injury patients were randomly distributed into two groups of equal size. Grou

Impact of Heparin and Enoxaparin Anticoagulant Prophylaxis on Improving Acute Mortality in Burn Patients.

Burn patients have a higher chance of developing thromboembolic complications, leading to worsened mortality rates, so prophylactic anticoagulation is important. Anticoagulants such as enoxaparin, a low-molecular-weight heparin, and unfractionated heparin (UFH) have been frequently used as chemical prophylactic treatments of thromboembolism. Enoxaparin has been shown to have lower mortality and higher efficacy in surgical patients and coronary artery disease patients. The aim of the study is to assess mortality and compare the safety of enoxaparin and heparin in acute burn patients. A retrospe

Clinical outcomes of bromelain-based enzymatic debridement (NexoBrid®): evidence from the Italian National Burn Database.

BACKGROUND: NexoBrid® (bromelain-based enzymatic debridement) has emerged as an alternative to traditional surgical excision in burn management, yet its overall impact on clinical outcomes in large unselected populations remains under discussion. METHODS: This retrospective study analyzed data from the Italian National Burn Database, including 2238 patients with complete core data treated between 2018 and 2024, to assess the association between NexoBrid® use and major outcomes: survival, escharotomy, skin grafting, number of surgical procedures, and hospital length of stay (LOS). Multivariabl

Uncommon suicide methods in the detention regime in Milan (1993-2019): Forensic contribution on autopsy cases.

We analysed the recorded cases of suicides committed other than by hanging in prison in Milan. A retrospective analysis was conducted on 25,512 autopsies performed from 1993 to 2019, selecting all the suicides in prison but our attention was focused solely on cases where an alternative mode to hanging was used. From a total of 97 suicide events in prison, 15 were consistent with the established search criteria: 6 victims died from plastic bag suffocation, 4 by direct inhalation of butane gas, 2 associated plastic bag suffocation to inhalation of butane gas, one committed suicide with an edged

Development, characterisation and pharmacoscintigraphic evaluation of nano-fluticasone propionate dry powder inhalation as potential antidote against inhaled toxic gases.

Acute lung injuries caused due to inhalation of toxic irritant gases such as ammonia, chlorine, hot smoke and burning plastic fumes predominantly affect the airways, causing tracheitis, bronchitis, and other inflammatory responses. The purpose was to develop and characterise nanoparticle based fluticasone propionate (FP) DPI formulation and assess its in vitro and in vivo pulmonary deposition using pharmacoscintigraphy. FP nanoparticles were prepared by nanoprecipitation method. Optimisation was carried out with the help of Box-Behnken statistical design. Nanoparticles were characterised with

Clinical outcomes in 'diabese' burn patients: A systematic review and meta-analysis.

OBJECTIVE: The objective of this study was to update the current status of clinical outcomes in diabetic (type II) and obese (BMI: 30-39.9 kg/m2) burn patients. METHODS: We adhered to Preferred Reporting Items for Systematic Reviews and Meta-Analysis (PRISMA) guidelines. We searched MEDLINE (PubMed), Google Scholar, Scopus, and Embase for studies related to a number of comorbidities and burn outcomes. Search terms for each of these databases are listed in the Appendix. From this search, we screened 6923 articles. Through our selection criteria, 12 articles focusing on either diabetes or obesi

Spectral Reflectance as a Unique Tissue Identifier in Healthy Humans and Inhalation Injury Subjects.

Tracheal intubation is the preferred method of airway management, a common emergency trauma medicine problem. Currently, methods for confirming tracheal tube placement are lacking, and we propose a novel technology, spectral reflectance, which may be incorporated into the tracheal tube for verification of placement. Previous work demonstrated a unique spectral profile in the trachea, which allowed differentiation from esophageal tissue in ex vivo swine, in vivo swine, and human cadavers. The goal of this study is to determine if spectral reflectance can differentiate between trachea and other

Choking agents and chlorine gas - History, pathophysiology, clinical effects and treatment.

INTRODUCTION: Choking agent exposure, among them chlorine gas, occurs in household or industrial accidents, chemical warfare and terrorist attacks. AIMS: Review of published animal and human data regarding the history, pathophysiology, clinical effects and management of chlorine exposure. PATHOPHYSIOLOGY: Highly soluble agents cause quick upper respiratory tract symptoms. Chlorine gas has a medium solubility, also causing delayed lower airway symptoms, mainly due to its oxidizing potential by releasing hypochlorous and hydrochloric acid, but also by interacting with Transient Receptor Potent

Intraosseous administration of hydroxocobalamin after enclosed structure fire cardiac arrest.

Smoke inhalation is the most common cause of acute cyanide poisoning in the developed world. Hydroxocobalamin is an antidote for cyanide poisoning. There is little published about human intraosseous antidote administration. We present a case of intraosseous hydroxocobalamin administration in an adult smoke inhalation victim, found in cardiac arrest inside her burning manufactured home. Return of spontaneous circulation was achieved after 20 min of cardiopulmonary resuscitation. Five grams of hydroxocobalamin were subsequently given intraosseously. On hospital arrival, patient was found to have

Hydrogen cyanide and carboxyhemoglobin assessment in an open space fire-related fatality.

Hydrogen cyanide (HCN) can be a major contributory factor in death from fire-related inhalation injury. Although carbon monoxide (CO) is considered the lethal agent of smoke in fires, its liability as a cause of death is sometimes debatable. The purpose of this report is to present the case of an 80-year-old man with locomotor disabilities who died due to an open space fire of vegetation debris and household waste in his yard. We evaluated here the concentrations of HCN and carboxyhemoglobin (COHb) and their contribution to the mechanism of death. In addition, the risk factors and the contribu

Cohort analysis of 50% lethal area (LA50) and associating factors in burn patients based on quality improvements and health policies.

Burn injuries are among the common traumatic injuries, which can be accompanied with lifelong morbidity and mortality. The Lethal Area Fifty Percent (LA50) index is another reliable outcome measurement tool that assesses the standard of medical care at burn centers. It is widely used as a benchmark for assessing the quality of burn care and is considered the percentage at which 50% of burn patients are expected to die because of burn-related injuries. We aimed to determine and compare the LA50 in burn patients admitted to Shiraz Burn Referral Centers in 2018-2021 and 2011-2018 with regard to i

Cost impact of hydroxocobalamin in the treatment of patients with known or suspected cyanide poisoning due to smoke inhalation from closed-space fires.

BACKGROUND & OBJECTIVES: Cyanide poisoning can occur due to exposure to smoke in closed-space fires. With no point of care cyanide test at the scene of a fire, first responders and clinicians base decisions to treat with cyanide antidote on patient history, clinical signs, and other indirect data points that have not been proven to correspond with actual systemic levels of cyanide. The aim of this exploratory study was to determine the economic implications of treating patients with known or suspected cyanide poisoning due to smoke inhalation with hydroxocobalamin. METHODS: A decision analysi

[Frostbite injuries causing compromised airway after inhalation of propane].

We describe a case report of a 23-year-old man with acute pharyngeal injuries due to frostbite subsequent to inhalation of propane. He was fiber-optically intubated on admission to hospital since his airways were considered acutely compromised. He was subsequently kept intubated for 11 days due to persistent pharyngeal oedema and frostbite injuries. The latter is caused by low temperature of propane upon release from a pressurized container. Injuries caused by frostbite often gradually progress and thus caution should be exerted in regards to airway management.
