In vitro synergy of azole antifungals and methotrexate against Candida albicans.

OBJECTIVE: This study aims to evaluate the effective of azoles and MTX for patients with invasive candidiasis. METHODS: We used the disk diffusion assay and the checkerboard assay to evaluate the in vitro interactions between MTX and antifungals. In addition, we used the transmission electron microscopy to observe the ultrastructure of the effect of MTX and fluconazole on Candida albicans. RESULTS: The rates of synergy for the combination of MTX with fluconazole (FLC), itraconazole (ITC), and voriconazole (VRZ) were 91.3%, 65.2%, and 87% in checkerboard testing. No antagonism was found betwe

Intracranial Candida Auris Infection: A Case Report and Scoping Review.

OBJECTIVE: Candida auris (C. auris) is a multidrug-resistant yeast capable of causing invasive infections in both immunocompromised and immunocompetent hosts. While bloodstream and wound infections are well described, central nervous system (CNS) involvement remains rare and poorly characterized. We present a case of C. auris CNS infection and a scoping review of adult cases to identify trends in diagnosis and treatment. METHODS: We searched PubMed, Embase, Scopus, Web of Science, and Europe PMC for reports from 2009 to 2025 using CNS-specific and C. auris-specific terms. After deduplication

The defective gut colonization of Candida albicans hog1 MAPK mutants is restored by overexpressing the transcriptional regulator of the white opaque transition WOR1.

The transcriptional master regulator of the white opaque transition of Candida albicans WOR1 is important for the adaptation to the commensal lifestyle in the mammalian gut, a major source of invasive candidiasis. We have generated cells that overproduce Wor1 in mutants defective in the Hog1 MAP kinase, defective in several stress responses and unable to colonize the mice gut. WOR1 overexpression allows hog1 to be established as a commensal in the murine gut in a commensalism model and even compete with wild-type C. albicans cells for establishment. This increased fitness correlates with an en

Emergence of Antifungal Resistant Subclades in the Global Predominant Phylogenetic Population of Candida albicans.

Candida albicans remains the most common species causing invasive candidiasis. In this study, we present the population structure of 551 global C. albicans strains. Of these, the antifungal susceptibilities of 370 strains were tested. Specifically, 66.6% of the azole-nonsusceptible (NS)/non-wild-type (NWT) strains that were tested belonged to Clade 1. A phylogenetic analysis, a principal components analysis, the population structure, and a loss of heterozygosity events revealed two nested subclades in Clade 1, namely, Clade 1-R and Clade 1-R-α, that exhibited higher azole-NS/NWT rates (75.0% a

An Immunocompromised Mouse Model of Candida auris Systemic Infection.

With the recent emergence of multidrug-resistant Candida auris, there is an urgent need for new antifungal compounds with novel pharmacodynamic and pharmacokinetic properties that can treat systemic fungal infections caused by this emerging yeast. Historically, testing the efficacy of treatment for disseminated candidiasis was accomplished using a diverse array of in vivo animal models, including mice which offer an advantage both in their similarities to humans and their lower cost of maintenance. However, in order to create effective in vivo models for testing new antifungal compounds design

Epidemiology and burden of invasive fungal infections in the countries of the Arab League.

The burden of invasive fungal infections is alarming worldwide. The aim of this paper is to review the published literature and evaluate the knowledge gap pertaining to studies on invasive fungal infections in the countries of the Arab League. Few countries from this region have published reports. The most commonly studied invasive fungal infections is invasive candidiasis. Candida albicans remains overall the most common causative pathogen (33.8-60%), however, non-albicans Candida species are increasing. Antifungal susceptibility testing is non-standardized across the published studies. Data

[Epidemiology of invasive mycoses. Experience of a university hospital center in Paris].

The incidence of opportunistic fungal infections have dramatically increased during the past decades. Data from a retrospective study conducted between January 1992 and December 1994 in Necker-Enfants Malades Hospital and a review of the literature were analyzed to assess the main epidemiological features of these infections. Candidiasis remained largely the most frequent: candidemia and invasive candidiasis were diagnosed in respectively 71 and 11 patients. Candida albicans was the agent the most frequently isolated from these infections. However, its percentage was not over 49.35% in our exp

Comprehensive strategies to defeat Candida auris - challenge and insight: A review.

Candida auris, an emerging fungus that exhibits a unique transmission ability, causes large healthcare-associated outbreaks on 6 continents since first reported in 2009. Most isolates have acquired fluconazole resistance, and resistance to other agents/classes emerges during drug exposure. The emergence of C auris infections raises serious concerns for public health. Systematic elaboration on clinical treatment and prevention of C auris infections is rare. We aim to summarize the research progress on the risk factors, clinical signs, drug treatment and prevention of C auris infections.

Analogs of the anti-malaria drug mefloquine have broad-spectrum antifungal activity and are efficacious in a model of disseminated Candida auris infection.

Only three classes of antifungal drugs are currently in clinical use. Here, we report that derivatives of the malarial drug mefloquine have broad-spectrum antifungal activity including difficult-to-treat molds and endemic fungi. Pharmacokinetic and efficacy studies of NSC-4377 indicate that it penetrates the central nervous system and is active against Candida auris in vivo. These data strongly support the further development of mefloquine analogs as a potentially new class of antifungal molecules.

A targeted peritransplant antifungal strategy for the prevention of invasive fungal disease after lung transplantation: a sequential cohort analysis.

BACKGROUND: Lung transplant recipients are at high risk of invasive fungal disease (IFD), particularly invasive aspergillosis and candidiasis. The antifungal strategy that optimally balances effective reduction of IFD with a minimum of toxicity remains undefined; universal triazole prophylaxis is common at lung transplantation (LT) centers, despite the well-known toxicities and costs of this approach. METHODS: We implemented an antifungal strategy in March 2007 targeted at LT recipients at highest risk for IFD based on our institutional epidemiology. All patients received inhaled amphotericin

In Vitro and In Vivo Inhibitory Activity of Limonene against Different Isolates of Candida spp.

Commensal yeast from the genus Candida is part of the healthy human microbiota. In some cases, Candida spp. dysbiosis can result in candidiasis, the symptoms of which may vary from mild localized rashes to severe disseminated infections. The most prevalent treatments against candidiasis involve fluconazole, itraconazole, miconazole, and caspofungin. Moreover, amphotericin B associated with prolonged azole administration is utilized to control severe cases. Currently, numerous guidelines recommend echinocandins to treat invasive candidiasis. However, resistance to these antifungal drugs has inc

Burden of fungal infections in Senegal.

Senegal has a high rate of tuberculosis and a low HIV seropositivity rate and aspergilloma, life-threatening fungal infections, dermatophytosis and mycetoma have been reported in this study. All published epidemiology papers reporting fungal infection rates from Senegal were identified. Where no data existed, we used specific populations at risk and fungal infection frequencies in each to estimate national incidence or prevalence. The results show that tinea capitis is common being found in 25% of children, ~1.5 million. About 191,000 Senegalese women get recurrent vaginal thrush, ≥4 times ann

Clinical characteristics and outcomes of invasive fungal infections in critically ill patients with liver cirrhosis.

BACKGROUND & AIMS: Invasive fungal infections (IFIs) are increasingly recognized as a severe and often underdiagnosed complication in critically ill patients with liver cirrhosis. Cirrhosis-associated immune dysfunction, and advanced disease stage contribute to an elevated risk, particularly in the intensive care unit (ICU). Despite their high mortality, data on the epidemiology and timing of IFIs in ICU-treated cirrhotic patients remain limited. We aimed to determine the prevalence, timing, and impact of IFIs on 30- and 90-day survival in this high-risk population. METHODS: This retrospectiv
