A rare presentation of neuralgic amyotrophy in a child and a review of recent literature.

Neuralgic amyotrophy (NA), also known as brachial neuritis and previously known as Parsonage–Turner Syndrome, has an unknown etiology. Patients with NA have a clinical pattern characterized by sudden and acute pain across the shoulder followed by flaccid paralysis. NA has an incidence of one new case per 1000 people per year with an onset of age ranging between 20 and 60 years. We describe a rare presentation of NA in a Caucasian boy who was 11 years old and did not have any other family members affected by NA. All diagnostic studies were normal and he had full recovery 5 months from the onset

[Blockade of the suprascapular nerve in shoulder pain].

The pathological course of the most common diseases of the shoulder includes pain as a main symptom. Besides other methods of treatment the infiltration of the suprascapular nerve can improve the therapeutic methods in many cases. The technique of Gordh, used as an adjuvant procedure, is shown. Since we used this technique of local anaesthesia, in 32 of 42 cases a temporary release from pain could be achieved. In 6 more patients it led to an significant reduction of pain. In conclusion this method of local anaesthesia can be recommended as an additional treatment of patients with pain of the s

Total Resection of Pediatric Desmoid Tumor of the Left Neck with Utilization of 3D Virtual Surgical Planning.

A 13-year-old girl with a painful left neck mass was referred to our institution due to suspicions of malignancy. The patient reported pain … he brachial plexus, cranial nerves X and XI, and spinal nerves C3-C5 (including the phrenic) of the cervical plexus. Postsurgical pathological analysis confirmed a diagnosis of desmoid tumor, also known as aggressive fibromatosis, whereas DNA sequencing revealed a CTNNB1 mutation, a somatic genetic marker found in approximately 90% of desmoid tumor cases. When po …

[Chronic cervical epidural hematoma diagnosed by magnetic resonance imaging].

The clinical picture of spinal epidural hematoma is usually characterized by the sudden onset of pain and acute paraplegia within a few hours. The reports of chronic spinal epidural hematoma above the lumbar level is extremely rare. Here we added one case whose hematoma was at cervical level. A 31-year-old previously healthy male suffered from continuous sharp pain in the back of his neck about 10 weeks before admission. He took analgesic drugs and treated with head traction by his home doctor with minimal relief. 4 weeks later from onset progressive weakness and numbness appeared at his right

Restoration of prehension with the double free muscle technique following complete avulsion of the brachial plexus. Indications and long-term results.

BACKGROUND: Recent interest in reconstruction of the upper limb following brachial plexus injuries has focused on the restoration of prehension following complete avulsion of the brachial plexus. METHODS: Double free muscle transfer was performed in patients who had complete avulsion of the brachial plexus. After initial exploration of the brachial plexus and (if possible) repair of the fifth cervical nerve root, the first free muscle, used to restore elbow flexion and finger extension, is transferred and reinnervated by the spinal accessory nerve. The second free muscle, transferred to resto

Hydrocephalus due to marked enlargement of spinal roots in a patient with chronic inflammatory demyelinating polyradiculoneuropathy.

BACKGROUND: Hydrocephalus or papilledema has rarely been reported in patients with chronic inflammatory demyelinating polyradiculoneuropathy (CIDP). METHODS: We report a 65-year-old woman with a 12-year history of CIDP presenting with progressive dementia, hallucination and deterioration of gait. RESULTS: Neurological examination revealed cognitive impairment, symmetric proximal and distal weakness with areflexia and muscle atrophy in the distal four limbs. The cerebrospinal fluid examination showed marked elevation of protein concentration. Magnetic resonance imaging revealed hydrocephalus

Outside-in foraminal access for cervical dumbbell tumors: a surgical paradigm shift with technical insights and clinical outcomes.

OBJECTIVE: Cervical dumbbell tumors present surgical challenges due to their proximity to critical structures, including the facet joints and vertebral artery (VA). Conventional posterior approaches often require facet joint resection and fusion, risking spinal instability. This study summarizes the authors' experience with a lateral foraminal approach that preserves facet integrity through a minimally invasive corridor. METHODS: The lateral foraminal approach begins with an incision along the lateral border of the sternocleidomastoid, proceeding obliquely "outside-in" through the interscalen

High-Resolution Ultrasound for Differentiation Between Leprosy-Associated Neuropathy and Vasculitic Peripheral Neuropathy: A Pilot Study.

INTRODUCTION/AIMS: Leprosy-associated neuropathy and vasculitic neuropathy (VN) share overlapping clinical features, making differentiation challenging in nonendemic regions. High-resolution ultrasound may aid in distinguishing these conditions, but comparative data are limited. This pilot study aimed to characterize and compare ultrasound findings in biopsy-confirmed leprosy-associated neuropathy and VN, and to identify distinctive sonographic features for early differential diagnosis. METHODS: We retrospectively included 12 patients with biopsy-confirmed neuropathy (3 leprosy-associated, 6

Different mechanisms of electroacupuncture and pregabalin in neuropathic pain-induced ultrastructure damage in a rat model of brachial plexus neuralgia.

OBJECTIVES: Brachial plexus neuralgia (BPN) can result from injury, trauma, or cancer, and it significantly impacts the patient's quality of life. This study investigated electroacupuncture (EA) vs. pregabalin on the structural changes in the peripheral nervous system (PNS) and central nervous system (CNS) in rats with BPN. METHODS: Male Sprague-Dawley rats were randomized to the sham-operated (Con), BPN (using cobra venom), BPN+EA (EA), and BPN+pregabalin (PGB) groups (n = 8/group). EA or pregabalin was given from postoperative day (POD)14 to POD35. Structural alterations were determined by

In-Depth Look at the Anatomical Relationship of the Lesser Occipital Nerve, Great Auricular Nerve, and Spinal Accessory Nerve and Their Implication in Safety of Operations in the Posterior Triangle of the Neck.

BACKGROUND: Migraine surgery is an increasingly popular treatment option for migraine patients. The lesser occipital nerve is a common trigger point for headache abnormalities, but there is a paucity of research regarding the lesser occipital nerve and its intimate association with the spinal accessory nerve. METHODS: Six cadaver necks were dissected. The lesser occipital, great auricular, and spinal accessory nerves were identified and systematically measured and recorded. These landmarks included the longitudinal axis (vertical line drawn in the posterior), the horizontal axis (defined as a

Communication between the transverse cervical nerve (C2,3) and marginal mandibular branch of the facial nerve: a cadaveric and clinical study.

Several branches of the facial nerve are known to anastomose with branches of the cervical plexus, other cranial nerves, and the trigeminal nerve. Communication between the sensory transverse cervical nerve (C2, 3) and marginal mandibular nerve is, however, less well known, and in a previous study of 86 neck dissections we reported a 2.3% incidence of anastomoses between them. In this prospective study, we meticulously searched for more examples using both formalin-fixed cadavers and neck dissections. A total of 102 necks were included (both sides of 36 cadavers (n=72 necks), and 30 patients w…

Descendens vagohypoglossi: rare variant of the superior root of ansa cervicalis.

Knowledge of variants in the formation and position of the ansa cervicalis is important in head and neck surgery, specifically in reconstructions of the tongue that use the infrahyoid muscles, and in the anastomosis of the ansa cervicalis to the recurrent laryngeal nerve when the laryngeal muscles have been paralysed. We describe a rare variant of the superior root of the ansa cervicalis, which had a contribution from the vagus and hypoglossal nerves. The inferior root was formed by the C2 and C3 ventral rami, but it passed medial to the internal jugular vein before it joined the superior root

Treatment for thoracic outlet syndrome.

BACKGROUND: Thoracic outlet syndrome (TOS) is one of the most controversial clinical entities in medicine. Despite many reports of operative and non-operative interventions, rigorous scientific investigation of this syndrome leading to evidence based management is lacking. OBJECTIVES: To evaluate the beneficial and adverse effects of the available operative and non-operative interventions for the treatment of thoracic outlet syndrome. SEARCH STRATEGY: We searched the Cochrane Neuromuscular Disease Group Trials Specialized Register (July 2009), The Cochrane Central Register of Controlled Tria

Lateral interscalenic multilevel oblique corpectomies to repair ventral root avulsions after brachial plexus injury in humans: anatomical study and first clinical experience.

OBJECT: Because central nervous system white matter exerts a powerful inhibitory effect on axonal growth, implantation of nerve grafts or rootlets into the cervical spinal cord following ventral root avulsion injury should, ideally, be performed directly through the ventral root exit zone (VRExZ), which is located near the anteromedial aspect of the anterior horn; the grafts/rootlets should not be implanted into the white matter of the lateral cord. This is not possible when using a conservative posterior approach. Therefore, the authors have studied the anatomy encountered when using the ante

Embryonic development of the head and neck: part 5, the brain and cranium.

The development of the brain is presented with respect to closure of the neural tube, the formation of the five vesicles of the brain and their derivatives, and the cranial flexures. The cranial nerves, meninges, choroid plexuses, and cerebral vasculature are also discussed. The origin and ossification of the bony elements of the head and neck, including the neurocranium, viscerocranium, and cervical vertebrae, are described, as well as major congenital defects of the brain and the cranium.
