Under certain circumstances patients with a constitutional predisposition, mainly with atypical ligaments and bands or a cervical rib may develop a Thoracic Outlet Syndrome (TOS). TOS can be divided into three subgroups: arterial, venous and neurogenic. Most of our cases have neurogenic symptoms combined with signs of arterial compression. The complications of TOS are arterial and venous. "TOS" is a clinical diagnosis. A history of upper extremity paresthesia that is exacerbated by abduction is typical for TOS. Objective neurological findings are usually lacking. Positive provocative maneuvers…

OBJECTIVE: Occipitocervical fusion (OCF) in children fails at a rate of up to 20% despite multiple technical advances; while the literature reports multiple grading scales of radiographic fusion, none function independently of the surgical technique or type of osseous graft used. In this study, the authors sought to establish 1) a novel reproducible surgical technique with low failure rates and 2) a new, easily applied radiological scoring system to objectively evaluate arthrodesis.
METHODS: The authors conducted a single-institution retrospective cohort study of 21 patients who underwent OCF

Cervical spine injuries are the most common spine injuries in the pediatric population. The authors present the youngest known patient who underwent cervical spine fusion to repair birth trauma-induced cervical fracture dislocation, resulting in spondyloptosis and spinal cord injury. A 2-week-old boy was found to have spondyloptosis and spinal cord injury after concerns arose from reduced movement of the extremities. The patient's birth was complicated by undiagnosed abdominal dystocia, which led to cervical distraction injury. At 15 days of age, the boy underwent successful C-5 corpectomy, wi

Intrathyroid parathyroid adenoma is a rare lesion. Its location is usually achieved by cervical ultrasound and scintigraphy. We present the case of a 48-years old woman admitted for malaise, weight loss, generalized bone pain, and important limitation of hip and scapular-humeral joints mobility. Cervical ultrasound identified a 4 cm nodular mass in the right thyroid lobe. Computed tomography revealed multiple osteolytic lesions in the pelvis, femur, ribs, phalanx, and humerus. Imagery and elevated serum levels of calcium and parathormone led to primary hyperparathyroidism, right parathyroid ad

BACKGROUND: Regional anaesthesia can be an attractive alternative anaesthetic approach in clavicle surgery, but it requires the peripheral block of multiple cervical and brachial nerves that transmit nociceptive information. Deep cervical plexus blocks, as paravertebral nerve block, can lead to severe side effects, such as unilateral diaphragmatic paralysis.
CASE REPORT: A 66-year-old male patient, American Society of Anesthesiologists physical status III, was scheduled for open reduction and internal fixation of the right clavicle with plates and screws after a high-energy trauma. Pre-anaest

UNLABELLED: Introduction. Neurogenic Thoracic Outlet Syndrome (NTOS) is a chronic lower trunk brachial plexus entrapment caused by a cervical rib or a fibrous band. True NTOS is rare and progresses usually slowly. Case report. A 12-year-old girl complained of numbness and weakness of the right upper limb immediately after an orthopedic surgical procedure for scoliosis. Neurological and neurophysiological features were both consistent with a neurogenic thoracic outlet syndrome (NTOS).
CONCLUSION: This observation illustrates the risk of NTOS after certain surgical procedures, especially when a…

The technique of radical treatment of tuberculosis of the spine at various levels is described. Eighty cases out of 158 of spinal caries underwent radical surgery over a period of 7 years. Caseous pus, sequestra, granulation tissue and discs were excised. Rib grafts were used in the dorsal spine and iliac strut grafts in the cervical and lumbar spine, through a direct anterior approach. Sound bony fusion was achieved in 90% of cases. The procedure is technically difficult but recovery is fast and the majority were fit to resume work within 6 months.

BACKGROUND: Brown tumors (BTs) represent the typical nonmalignant lesions of hyperparathyroidism. Mandibles, ribs, and large bones are the most usual localization of BT. The diagnosis of these tumors requires biological and radiologic assessments. Their treatment is essentially based on parathyroidectomy.
CASE DESCRIPTION: The present case report describes a patient with primary hyperparathyroidism who developed a cervical BT revealed by slow spinal compression.
CONCLUSIONS: The brown tumor, when localized at the level of the spine, can be life-threatening and must be managed as soon as poss

We report a case of bilateral neurogenic thoracic outlet syndrome (TOS). Electrophysiological examination suggested the presence of bilateral lower brachial plexus neuropathy. Radiography showed rudimentary bilateral cervical ribs. In the cases reported in the literature to date, the clinical findings are typically unilateral despite the common presence of bilateral bony abnormalities. Neurogenic TOS should be considered in young women, even if they present with bilateral symptoms, when they have occupations requiring strenuous activity of the upper limbs.

Incremental use of high-dose radiation therapy (RT) with SABR in thoracic tumors has led to identification of many unusual toxicities (chest wall pain, rib fractures, vascular perforation, brachial plexopathy) and consequently additional organs at risk (OARs; chest wall, ribs, bronchial wall, carotid artery, brachial plexus). Phrenic nerve is another structure that may be affected at any point during its long course from origin until end, although symptomatic toxicities have been reported rarely in the setting of reirradiation, large-volume irradiation such as mantle field, or SABR. We underto

Many different procedures have been developed to treat thoracic outlet compression syndrome (TOCS), a condition in which neurovascular structures in the thoracic outlet region are compressed. Currently, transaxillary first rib resection and transcervical anterior and middle scalenectomy are the most popular and standard procedures. In the early 1980s, some surgeons started to perform both procedures, starting with the scalenectomy and following with a transaxillary first rib resection. We have found that performing these procedures in the reverse order, starting with the first rib resection an

INTRODUCTION: Spine fractures are associated with high energy mechanisms and can lead to substantial morbidity and mortality in the trauma setting. Rapid identification and treatment of these fractures and their associated injuries are paramount in preventing adverse outcomes. The purpose of this study is to identify concomitant skeletal and non-skeletal injuries related to cervical, thoracic, and lumbar fractures.
METHODS: A retrospective review of institutional American College of Surgeons (ACS) registry was conducted on 3,399 consecutive trauma patients identifying those with spine fractur

A study was made of 11 patients with cervical rib, and one patient with Klippel-Fiel syndrome and enlarged transverse processes to determine whether evoked potentials recorded from both Erb's points and the cervical spine in response to median and ulnar nerve stimulation provided information additional to that obtained by EMG and peripheral conduction studies. It was found that in seven patients who had pain and paraesthesias but no objective neurological signs both the peripheral and central conduction studies were within normal limits. By contrast, of five patients who had objective signs, c…

We report a case of large multiple brown tumors in a patient with primary hyperparathyroidism. A 52-year-old woman suffered from pain in the ribs and developed left facial swelling and deformity. CT showed a large destructive osteolytic lesion in the left maxillary sinus. Biopsy showed a lesion with newly formed bone tissue, diffuse giant cells and deposits of hemosiderin. In addition, similar lesions were also observed in the ribs, iliac bones and pelvis. The laboratory data showed hypercalcemia and hyperparathyroidism. Cervical echo and (201)Tl-(99m)TcO(4-) scintigraphy demonstrated a right

The Klippel-Feil syndrome is a congenital anomaly characterized by fusion of the cervical vertebrae. It is often associated with serious congenital anomalies of the nervous, cardiovascular and urogenital systems. One of the anomalies which have not been thoroughly investigated to date are that accompanying Klippel-Feil syndrome and enlarged Eustachian tube. We report a case of type III Klippel-Feil syndrome with associated rib anomalies such as hypoplastic and bifid ribs, scoliosis and elevated scapula (Sprengel's disease). The patient also presented hemifacial microsomia and central facial pa

Thoracic outlet syndrome occurs due to compression of the neurovascular structures as they exit the thorax. Subclavian arterial compression is usually due to a cervical rib, and is rarely associated with thromboembolic stroke. The mechanism of cerebral embolisation associated with the thoracic outlet syndrome is poorly understood, but may be due to retrograde propagation of thrombus or transient retrograde flow within the subclavian artery exacerbated by arm abduction. We report an illustrative patient and review the clinical features, imaging findings and management of stroke associated with …

BACKGROUND: Multiple hereditary exostoses (MHE) lead to the development of pedunculated or sessile osteocartilaginous lesions. Vertebral involvement occurs in MHE and encroaching intracanal exostoses can result in devastating consequences. Magnetic resonance imaging (MRI) of the entire spine has been used to screen for vertebral exostoses to detect high-risk patients. The primary purpose of this investigation is to determine the incidence of vertebral and encroaching intracanal exostoses in patients with MHE. A secondary purpose is to determine if pelvis and rib exostoses serve as "harbinger"
