The vertebral column of the Regourdou 1 Neandertal.

The Regourdou 1 partial skeleton was found in 1957 in level IV of the eponymous site located in Montignac-sur-Vézère (Dordogne, France) and until now it has been only partially published. The ongoing revision of the faunal remains from the site has yielded additional fossils that pertain to this skeleton. Here we study the vertebral column of this individual, providing for the first time detailed descriptions for all of the fossils and reassessing the anatomical position of all of the fragments. The vertebral column of Regourdou 1 is one of the most complete in the Neandertal fossil record wit

Late radiographic findings after the anterior cervical fusion for the cervical subaxial compressive flexion and vertical compression injuries in young patients.

OBJECTIVE: The aim of this study was to investigate the influence of patients' age on the development of radiologic signs of degeneration of adjacent levels after the anterior fusion for the cervical spine injuries. MATERIAL AND METHODS: A total of 45 patients who had compressive flexion and vertical compression injuries of the cervical spine (by Ferguson-Allen mechanistic classification) were included in the study. There were 40 male and 5 female patients with a mean age of 31.5 years (range 15-64). These patients were treated with anterior decompression, iliac bone grafting, and anterior pl

Transoral approach to the cervical spine: report of four cases.

The transoral approach to the upper cervical spine is an established but little used route, offering excellent access with good wound healing, to lesions of the bodies of the atlas, axis and upper part of the third cervical vertebra. The authors report four cases which demonstrate the value of the procedure.

The Neanderthal cervical spine revisited.

Recent studies have challenged the long-held notion that Neanderthals possessed a more stable and less lordotic cervical spine than modern humans. To investigate this hypothesis further, we conducted the first three-dimensional geometric morphometric analysis focused on the Neanderthal cervical spine, examining 43 classic Homo neanderthalensis cervical vertebrae alongside 243 Homo sapiens cervical vertebrae from diverse populations, including the whole cervical spine (C1-C7). The Neanderthal sample comprises specimens from well-known individuals such as La Ferrassie 1, La Chapelle-aux-Saints 1…

Assessment for additional spinal trauma in patients with cervical spine injury.

An institutional review board-approved 8-year retrospective trauma registry analysis of cervical spine injuries (CSIs) was done in a Level 1 trauma center. This analysis includes 129 CSI patients (1.3% of trauma admissions). Cervical spine radiographs diagnosed injuries in 71 per cent of CSI patients. Cervical spine radiographs were false negative in 29 per cent of patients, who were found to have CSI on spine CT. Spine CT had 98 per cent sensitivity and detected 45 per cent additional injuries in cervical spine radiograph-positive patients. Spine CT scans were false negative in two patients w

Bilateral variation in the origin and course of the vertebral artery.

Understanding the great vessels of the aortic arch and their variations is important for both the endovascular interventionist and the diagnostic radiologist. An understanding of the variability of the vertebral artery remains most important in angiography and surgical procedures where an incomplete knowledge of anatomy can lead to serious implications. In the present case, a bilateral variation in the origin and course of vertebral artery was observed. The left vertebral artery took origin from the arch of aorta and entered the foramen transversarium of the fourth cervical vertebra. The right

Postoperative Increase in Occiput-C2 Angle Negatively Impacts Subaxial Lordosis after Occipito-Upper Cervical Posterior Fusion Surgery.

STUDY DESIGN: Retrospective case series. PURPOSE: To elucidate the impact of postoperative occiput-C2 (O-C2) angle change on subaxial cervical alignment. OVERVIEW OF LITERATURE: In the case of occipito-upper cervical fixation surgery, it is recommended that the O-C2 angle should be set larger than the preoperative value postoperatively. METHODS: The present study included 17 patients who underwent occipito-upper cervical spine (above C4) posterior fixation surgery for atlantoaxial subluxation of various etiologies. Plain lateral cervical radiographs in a neutral position at standing were ob

Evaluation of mobility and stability in the Discover artificial disc: an in vivo motion study using high-accuracy 3D CT data.

OBJECT: Artificial disc replacement (ADR) devices are unlike implants used in cervical fusion in that they are continuously exposed to stress not only within the implant site but also at their site of attachment to the adjacent vertebra. An imaging technique with higher accuracy than plain radiography and with the possibility of 3D visualization would provide more detailed information about the motion quality and stability of the implant in relation to the vertebrae. Such high-accuracy studies have previously been conducted with radiostereometric analysis (RSA), which requires implantation of

Rare association of Klippel-Feil syndrome with situs inversus totalis and review of the genetic background.

Klippel-Feil syndrome (KFS) is a rare congenital anomaly in forming the cervical vertebrae resulting in the fusion of two or more of the vertebrae. KFS is associated with many congenital anomalies, some of which are common and well known. Here, we report a child with an extremely rare association of KFS with situs inversus totalis (SIT). Both KFS and SIT are genetically heterogeneous and their co-occurrence suggests a high possibility of sharing the same underlying causative agent. Here, we review the genetic background that is known for these two conditions in the literature.

Experimental impact injury to the cervical spine: relating motion of the head and the mechanism of injury.

The purpose of this study was to analyze, with use of an impact model, the relationships among motion of the head, local deformations of the … echanism of injury. Buckling of the cervical spine, involving extension between the third and sixth cervical vertebrae and flexion between the seventh and eight cervical vertebrae, was observed. Other, more complex, buckling deformations were also seen, suggesting that the deformations that occur during impact are so complex that they can give rise to a number of di …

Radiological cervical arthritis in populations.

The prevalence of cervical rheumatoid arthritis and its relationship to rheumatoid serum factors and erosive arthritis in peripheral joints has been studied in radiographs of the cervical spine and of the hands and feet drawn from 12 population samples. The changes were graded in accordance with the Atlas of Standard Radiographs of Arthritis. Rheumatoid arthritis of the cervical spine (grades 2-4) was observed in 4.1% of males and 4.7% of females aged 15 and over. Prevalence was greatest in those born before 1900, 15% of whom were affected. There was a significant association with the sheep ce

Klippel-Feil syndrome revealed by post-traumatic neck pain: Case report and literature review.

Klippel-Feil syndrome (KFS) is a congenital anomaly involving the fusion of two or more cervical vertebrae. It is characterized by a low hairline at the nape of the neck, a short neck, and a limited range of motion. Here, we present the case of a 10-year-old girl who suffered brain trauma in a motor vehicle accident and subsequently experienced persistent occipital headaches and dizziness. A brain computed tomography scan performed to evaluate these symptoms revealed incomplete fusion of two cervical vertebrae (C2-C3), leading to a diagnosis of KFS. Magnetic resonance imaging of the spine was …

[Total anomalous pulmonary venous connection associated with Klippel-Feil syndrome: a case report].

Klippel-Feil syndrome is characterized by congenital fusion of the cervical vertebrae. Anomalies in other organs sometimes combine. Many kinds of congenital cardiac anomalies, most frequently ventricular septal defect, considerably associate with this syndrome. Few cases of surgical corrections of cardiac anomalies with Klippel-Feil syndrome have been reported up to today. Therefore surgical problems with this syndrome have not been solved yet, completely. We have experienced a very rare case of a 18-year-old female of total anomalous pulmonary venous connection (Darling Ia type) associated wi…

Spinal cord injury at birth: a hidden causative factor.

UNLABELLED: A case of perinatally acquired spinal cord injury (SCI) is presented. The foetus was vigorous until birth, the breech presented and delivery was performed by a non-traumatic Caesarean section. The infant displayed symptoms of severe SCI but diagnosis was delayed due to severe co-morbidity. Diagnostic considerations are briefly reviewed. Ventilatory support was withdrawn at the age of 20 days when the infant had still not exhibited any respiratory effort or spontaneous movements. Autopsy revealed a serious congenital malalignment of the upper cervical vertebrae and at the histologic…

Embryologic pathogenesis of renal agenesis associated with cervical vertebral anomalies (Klippel-Feil phenotype).

Existing embryologic data suggest that the blastema of the cervical vertebrae, scapulae, and the genitourinary system have an intimate spatial relationship at the end of the 4th or beginning of the 5th week of fetal life. An alteration in this region can affect the cervical vertebrae and scapulae directly, and the genitourologic changes are mediated indirectly through the inductive capacity of the pronephric duct. The genitourinary malformation documented in patients with a specified Klippel-Feil phenotype support such an embryologic pathogenesis. Urologic pathology, in both sexes, is consiste…

Delayed spinal cord injury following microchip placement in a dog.

A three-year-old female, entire Yorkshire Terrier dog was examined because it had progressive non-weight-bearing left forelimb lameness and tetraparesis of two weeks duration. Clinical signs were first observed following mating. Examination confirmed non-weight-bearing left forelimb lameness and tetraparesis. Left forelimb muscle atrophy was also noticed. Survey radiography revealed a metallic foreign body consistent with a microchip in close proximity to the left articular facets between the fifth and sixth cervical vertebrae. Computed tomography identified the exact location of the foreign b…

[Diagnosis and therapy of vertebrogenic vertigo].

Functional disorders of the cervical spine may lead to vertigenous symptoms. The typical pathological finding in these patients is a hyperlordotic position of the cervical spine. This causes functional problems of the C0/C1 and C1/C2 joints. X-rays show morphological changes of the vertebrae and may also point to functional disorders. Physiotherapy is the most important treatment modality and preventative measure in static dysbalance of the cervical spine. sometimes chiropractic therapy by an experienced manual therapist or carefully directed infiltration therapy is necessary.

Symptomatic atlantoaxial dislocation in Down's syndrome.

Four patients with Down's syndrome suffered compression of the spinal cord secondary to atlantoaxial dislocation. All four developed inability to walk and quadriparesis, with signs of cervical myelopathy. It is important to recognize this potentially fatal complication of the syndrome. Immediate immobilization of the neck, followed by cervical roentgenograms, should be done in any patient with Down's syndrome who presents with neck pain, torticollis, urinary incontinence, or loss of ambulation. Surgical repair with fusion of the first and second cervical vertebrae can be carried out later.
