Pediatric residency training in child abuse and neglect in the United States.

BACKGROUND: Child abuse and neglect are leading public health problems with significant morbidity and mortality. Previous studies indicate that physicians often lack knowledge and confidence in addressing child abuse and neglect. OBJECTIVES: Our goal was to assess the child abuse and neglect curricula in pediatric residency programs as reported by chief residents and to identify levels of preparedness of residents to address child abuse and neglect on graduation. We analyzed variables related to preparedness. METHODS: A 28-item survey was sent to chief residents of all 203 Accreditation Coun

Mothers' choice of health management services for under-five children with common illnesses: evaluating social workers' impact in Nigerian health sector.

INTRODUCTION: Every child deserves the right to life. In Nigeria like other African countries, a high burden of child mortality prevails. Attaining a low-mortality rate of children entails that mothers who are the primary caregivers are in the best position to provide quality healthcare management. METHOD: With the phenomenological approach in qualitative research, the researchers sourced data using Focus Group Discussions (FGDs) and In-Depth Interviews (IDI) from 38 mothers of various categories. The participants were purposively selected from four government and mission health institutions …

Knowledge and awareness regarding child sexual abuse myths and offenders among future nurses and forensic scientists: A cross-sectional study.

BACKGROUND: Child abuse and neglect are frequently underreported despite their association with significant morbidity and mortality. Identifying child sexual abuse remains challenging for healthcare professionals, and insufficient knowledge and awareness may contribute to its underrecognition. PURPOSE: The primary aim of this study was to compare nursing and forensic sciences students in terms of their acceptance of child sexual abuse myths and their knowledge and awareness regarding perpetrators of child sexual abuse. The secondary aim was to examine whether these outcomes differed according

Parenting under the triple burden of violence, depression, and poor diet quality: An intergenerational mother-child syndemic in Nepal.

Parenting while experiencing intimate partner violence (IPV), depression, and poor diet quality can contribute to child health inequities. Syndemic theory offers a framework to understand how maternal conditions can lead to inequitable child health outcomes, such as diarrheal disease: a risk factor for childhood mortality and stunting. This study uses the 2022 Nepal Demographic and Health Survey to test an intergenerational syndemic model addressing three key questions: (1) Do IPV, depression, and absent dietary iron intake co-occur among mothers? (2) Does the probability of child diarrhea inc

Liddle's Syndrome.

Hypertension in paediatric age group is commonly secondary to a known cause. It is crucial to identify the cause of hypertension and treat it before development of any associated complications to prevent morbidity and mortality. Paediatric Hypertension is one of the important clinical finding in a child with certain clinical syndrome. We are presenting a case of a 10 month old child presenting with hypertension and hypokalaemia, after excluding all identifiable causes and her positive response to therapy, that is amiloride, along with supportive biochemical data she was diagnosed as a case of

Typical and atypical presentations of congenital diaphragmatic hernia - a report of 2 cases.

Congenital diaphragmatic hernias occur through embryologic defects in the diaphragm. We present 2 cases of congenital diaphragmatic hernia, the first case was a late presentation in an 11 month old child, and the second case was an early typical presentation in a day old child, both diagnoses were made on chest radiographs. The paper is focused on the key role of the radiologist in a resource poor environment to make early diagnosis of this fatal and rare condition. Early diagnosis of this defect is essential. This will allow timely intervention and appropriate management, therefore, reducing

The Fire That Does Not Go Out: The Neglected Costs of Gas Flaring in Nigeria.

Gas flaring, the burning off gas coming out of oil wells is a common practice in oil-producing developing countries. This practice is harmful to human health, especially because of pollutants. This research focuses on Nigeria, where over 10 percent of all gas produced is flared and about 2 million people are estimated to live within 4 kilometres of a flare site. This paper uses child health data from Demographic Health Surveys and satellite-detected data on gas flaring to examine associations between flaring exposure and child morbidity, nutritional outcomes, and mortality among children under

Diagnosing Rickets in Early Modern England: Statistical Evidence and Social Response.

Seventeenth-century UK experienced an epidemic of the newly recognised disease rickets, its nutritional and environmental causes then unknown. This is evident from parish burial registers, the London Bills of Mortality, and contemporary medical descriptions and treatments. Rickets appeared to be killing 2-8 per cent of urbanites, especially wealthy children. Rickets emerged as a threat to child health in early modern UK as a result of coal dependency and climate, and social differences in infant and child feeding. Physicians investigating rickets showed concern for rich children's diets. Lack

Extended laryngofissure in the management of subglottic stenosis in the young child: a preliminary report.

The child with subglottic stenosis and a tracheotomy is a management problem. To date, a consistent method for successful and expeditious correction of the primary lesion to permit decannulation has eluded clinicians. The child is tracheotomized and thus frequently hospitalized for a lengthy period. Personal development and family relationships are adversely affected and the mortality rate during this period of cannulation is significant. During the past eighteen months in an attempt to achieve earlier decannulation, three children with subglottic stenosis have undergone a surgical procedure i

Reconsidering childhood undernutrition: can birth spacing make a difference? An analysis of the 2002-2003 El Salvador National Family Health Survey.

It is well understood that undernutrition underpins much of child morbidity and mortality in less developed countries, but the causes of undernutrition are complex and interrelated, requiring a multipronged approach for intervention. This paper uses a subsample of 3853 children under age 5 from the most recent family health survey in El Salvador to examine the relationship between birth spacing and childhood undernutrition (stunting and underweight). While recent research and guidance suggest that birth spacing of three to five years contributes to lower levels of infant and childhood mortalit

Prediction of inpatient mortality in hospitalised children in low- and middle-income countries: An external validation of paediatric mortality risk scores.

BACKGROUND: Risk prediction tools for acutely ill children have been developed in high- and low-income settings, but few are validated or incorporated into clinical guidelines. We aimed to assess the performance of existing paediatric early warning scores for use in low- and middle-income countries using clinical data from a recent large multi-country study in Africa and South-Asia. METHODS: We used data (children across three nutritional strata) from the Childhood Acute Illness and Nutrition (CHAIN) Network cohort study (n = 3101). We assessed 10 scores where similar predictor variables were

The relationship between under-five child death and maternal mental distress in Gamo Gofa Zone, Southern Ethiopia: a community based comparative cross-sectional study.

BACKGROUND: Knowledge of the association between child death and maternal mental distress may help to understand the indirect impact of reduction of under-five mortality on maternal mental wellbeing. This will further have a positive impact on the development of the nation. Depression is associated substantially with reduced quality of life and functional capacity of women. Although studies in the country assessed the magnitude of Common Mental Disorders (CMD) among postpartum mothers, those assessing the association between child death and maternal mental distress are lacking. Therefore, this

Level of completion along continuum of care for maternal, newborn and child health services and factors associated with it among women in India: a population-based cross-sectional study.

BACKGROUND: India, being a developing country, presents a disquiet picture of maternal and neonatal mortality and morbidity. The majority of maternal and neonatal mortality could be avoided if the continuum of care (CoC) is provided in a structured pathway from pregnancy to the postpartum period. Therefore, this article attempted to address the following research questions: What is the level of completion along CoC for MNCH services? At which stage of care do women discontinue taking services? and what are the factors affecting the continuation in receiving maternal, newborn and child health (

Trends in Enterobacterales Bloodstream Infections in Children.

OBJECTIVES: Enterobacterales bloodstream infections (E-BSI) cause a significant burden of disease in children and are associated with antimicrobial resistance. We assessed temporal changes in the population-based incidence of E-BSI in children in Queensland, Australia. METHODS: We conducted a cohort study of incidents of E-BSI occurring in children in Queensland between 2000 and 2019, with a total population of 19.7 million child years. Infections were linked to clinical outcomes in hospital admissions and vital statistics databases. We estimated age- and sex-standardized E-BSI incidence rate

Trends in Homicide Hospitalization and Mortality in Taiwan, 1998-2015.

In Taiwan, the national research on homicide is rare, mostly discussing the issue of child abuse. We sought to better understand the characteristics and risk factors of homicide through a retrospective cohort study from 1998 to 2015. "Child battering and other maltreatment" ranked first for the 0-4 age group and second for the 5-14 age group. The hospital mortality was 511 deaths. We found that the 25-44 age group had the highest risk and accounted for 44.76% of hospitalization. The most common causes were "fight, brawl, and rape" (49.12%), "cutting and piercing instruments," (13.16%) and "chi
