A 0.05% cetyl pyridinium chloride/0.05% chlorhexidine mouth rinse during maintenance phase after initial periodontal therapy.

BACKGROUND: Chlorhexidine (CHX) mouth rinse/spray can still be considered the gold standard in the chemical prevention of plaque formation and development of gingivitis. The product unfortunately has some side effects, such as extrinsic tooth staining, poor taste, taste disturbance, sensitivity changes in tongue, pain and irritation because of the alcohol content. These side effects led to the search of new formulations. METHODS: In this double-blind, randomized, long-term, parallel study, 48 moderate periodontitis patients rinsed for 6 months (starting immediately after a "one-stage, full-mo…

Constructing antimicrobial oil-in-water emulsions via spatially diffused ionic interlocking networks between chlorhexidine digluconate and polysaccharides.

A spatially dispersed ionic-interlocking network was constructed in a CHG/CMC/MCT oil-in-water nanoemulsion through multipoint electrostatic association between the multicationic biguanide groups of chlorhexidine gluconate (CHG) and the carboxylate groups of sodium carboxymethyl cellulose (CMC). Unlike conventional systems in which CHG serves solely as an antimicrobial additive, CHG functioned simultaneously as an active agent and an ionic-network-forming unit, enabling spatially distributed electrostatic interactions that stabilized the dispersed medium-chain triglyceride (MCT) droplets. The …

A chloramine-based antiseptic polymer improves Candida auris skin decolonization compared to chlorhexidine in a murine model.

Candida auris frequently demonstrates resistance to multiple antifungal classes and has been classified as a critical priority pathogen by the World Health Organization and the first fungal pathogen designated an urgent threat by the U.S. Centers for Disease Control and Prevention. Patients with skin colonization face a 7%-20% risk of invasive infection, with 30-day mortality of 30%-72%. While chlorhexidine gluconate (CHG) is widely used for general skin decolonization, it fails to reliably clear C. auris from patient skin, and no proven decolonization protocol exists. We evaluated a novel chl…

Chlorhexidine Surface-Immobilized via a Carboxylated Polymer Layer on ZnO Nanoparticle-Containing Polycaprolactone Fibers for Wound Dressings.

We developed electrospun polycaprolactone (PCL) membranes containing 1-5 wt % ZnO nanoparticles (NPs), the surface of which was plasma-functionalized with carboxyl groups for covalent immobilization of chlorhexidine (CHX) via carbodiimide chemistry. Quantum-chemical analysis revealed that carboxyl functionalization, followed by 1-ethyl-3-(3-(dimethylamino)propyl)carbodiimide (EDC) activation, significantly enhanced CHX sorption, while ZnO NPs promoted charge transfer and increased the electrophilicity of the PCL matrix. The PCL-3%ZnO-CHX membranes exhibited high tensile strength (18.6 MPa), im…

Ventriculoperitoneal Shunt Surgery in Pediatrics: Does Preoperative Skin Antisepsis with Chlorhexidine/Alcohol Reduce Postoperative Shunt Infection Rate?

BACKGROUND: In pediatrics, shunt infection is considered the most common complication of ventriculoperitoneal (VP) shunt insertion and the main cause of shunt failure. Careful surgical technique and hygienic skin preparations are highly important for prevention of shunt infections. Our objective was to assess the significance of using preoperative chlorhexidine/alcohol as a skin antiseptic in reducing the infection rate in pediatric VP shunts surgery. METHODS: We conducted a retrospective, case control study of 80 pediatric patients with active hydrocephalus. The control group (a single step …

Chlorhexidine versus saline in reducing the risk of catheter associated urinary tract infection: A cost-effectiveness analysis.

BACKGROUND: Catheter associated urinary tract infections are one of the most common infections acquired in hospital. A recent randomised control study demonstrated the benefit of using chlorhexidine (0.1%) for meatal cleaning prior to urinary catheter insertion, by reducing both catheter associated asymptomatic bacteriuria and infection. These findings raise the important question of whether a decision to switch from saline to chlorhexidine was likely to be cost-effective. The aim of this paper was to evaluate the cost-effectiveness of adopting routine use of chlorhexidine for meatal cleaning …

Impact of unit-wide chlorhexidine bathing in intensive care on bloodstream infection and drug-resistant organism acquisition.

BACKGROUND: Chlorhexidine gluconate (CHG) bathing has been reported to decrease bloodstream infections and colonisation of multidrug-resistant organisms (MROs) in intensive care units (ICUs). However, its effectiveness in an Australian setting has not been assessed. OBJECTIVE: To test whether the introduction of ICU-wide CHG bathing in place of triclosan would affect rates of the primary outcome of central line-associated bloodstream infections (CLABSI), or the secondary outcomes of ICU-acquired positive blood cultures or other clinical specimens, and MRO colonisation including methicillin-re…
