Acute cholecystitis in neutropenic patients.

BACKGROUNDS/AIMS: The frequency of acute cholecystitis reported in neutropenic patients is between 0.4-1.65%. Clinical manifestations differ from general population as well as clinical approach, diagnosis and treatment. The aim of this work is to describe clinical characteristics, diagnostic approach, and outcomes of patients with hematological diseases that presented with neutropenia and fever associated with acute cholecystitis in a tertiary referral hospital. METHODS: We performed a retrospective analysis of patients with diagnosis of neutropenia and fever associated with acute cholecystit…

Acute acalculous cholecystitis in children.

Acute acalculous cholecystitis (AAC) is the inflammatory disease of the gallbladder in the absence of gallstones. AAC is estimated to represent at least 50% to 70% of all cases of acute cholecystitis during childhood. Although this pathology was originally described in critically ill or post-surgical patients, most pediatric cases have been observed during several infectious diseases. In addition to cases caused by bacterial and parasitic infections, most pediatric reports after 2000 described children developing AAC during viral illnesses (such as Epstein-Barr virus and hepatitis A virus infe…

[Emphysematous cholecystitis: a case study].

The emphysematous cholecystitis is a rare pathological entity which has to be recognized early in order to quickly administer the most suitable treatment and to avoid septic shock then death. We report the case of an acute emphysematous diagnosed by computed tomography with a positive prognostic due to antibiotherapy and cholecystectomy.

Surgical management of acute cholecystitis.

BACKGROUND: Acute cholecystitis occurs in approximately 1% of patients with known gallstones. It presents as a surgical emergency and usually requires hospitalisation for treatment. It is associated with significant morbidity and mortality, particularly in the elderly. Cholecystectomy is advocated for acute cholecystitis; however, the timing of cholecystectomy and the value of the additional treatments have been a matter of debate. This review examines the available evidence regarding the optimal surgical management of patients with acute cholecystitis. METHODS: A literature search was perfor…

Cholecystitis in Situs Inversus Totalis.

Acute cholecystitis is a common surgical problem; however, the diagnosis may be difficult when the patient has situs inversus, or mirror-image reversal of the organs. This case report discusses the clinical presentation, diagnosis, radiographic images, and surgical management of cholecystitis in the face of situs inversus totalis.

Candida famata-induced fulminating cholecystitis.

Lithiasic cholecystitis is classically associated with the presence of enterobacteria, such as Escherichia coli, Enterococcus, Klebsiella, and Enterobacter, in the gallbladder. Cholecystitis associated with fungal infections is a rare event related to underlying conditions such as diabetes mellitus, steroid use, and broad-spectrum antibiotic use for prolonged periods, as well as pancreatitis and surgery of the digestive tract. Here, we present the first reported case of a gallbladder infection caused by Candida famata.

[Cholecystitis in high risk patients].

Basing on the analysis of treatment results of 5129 patients with acute cholecystitis, major factors, influencing the outcome, were revealed. These were: age, severity of concomitant diseases, presence of the destruction in the abdominal cavity, late terms of the hospitalization, type and volume of surgical intervention, ASA stage III-IV, SIRS criteria. According to the revealed factors, different approaches to the surgical tactics of the acute cholecystitis treatment, were suggested. The suggested differential approach provided good immediate results with postoperative lethality level of 1.0±…

[Surgical treatment of acute cholecystitis].

Fundamental treatment for acute cholecystitis is cholecystectomy. However, the adoption of a treatment is dependend on degree of a severity of acute cholecystitis in each patient because its degree is influenced by factors such as duration from the onset of symptoms to medical examination. Early laparoscopic cholecystectomy is the preferred procedure for mild acute cholecystitis. Early cholecystectomy is also performed for moderately acute cholecystitis. However, if patients have severe local inflammation (gangrenous and purulent cholecystitis) early gallbladder drainage or open cholecystectom…

Acute cholecystitis with Listeria monocytogenes.

Listeriosis, an opportunistic food-borne disease caused by Listeria monocytogenes, is infrequent and occurs preferentially in patients at the extremes of age, during pregnancy or in immunocompromised hosts. Most common manifestations are maternofoetal and neonatal infections, severe invasive presentations such as bacteraemia with or without central nervous system symptoms occuring preferentially in immunosuppressed patients and self-limited gastro-enteritis affecting healthy individuals. Exceptionally, focal infections such as cholecystitis are described. We report here a case of acute cholecy…

[Eosinophilic cholecystitis: a case report.].

Eosinophilic cholecystitis is a rare disease with clinical presentation similar to acute cholecystitis. The diagnosis is based on typical symptoms of acute cholecystitis with presence of more than 90% eosinophilic infiltration within the gallbladder wall. The case of a 27-year-old woman with periodic epigastric pain during ten years is presented. Routine and Boyden s technique biliary ultrasounds were normal. Hepatobiliary scintigraphy showed delayed gallbladder empty and patient underwent endoscopic sphinterotomy. Laparoscopic cholecystectomy was performed with uneventful postoperative course…

Hemorrhagic cholecystitis after anticoagulation therapy.

Hemorrhagic cholecystitis is a rare and potentially fatal complication of biliary tract disease. Nontraumatic hemorrhagic cholecystitis is related to a variety of etiologies. Here, the authors reported an elderly patient who developed hemorrhagic cholecystitis after heparin and aspirin usage for unstable angina. Laparoscopic cholecystectomy was performed with the histopathology report disclosing adenocarcinoma in situ. As demonstrated in this case, underlying malignant changes in its early stage increase the risk of bleeding concomitant with anticoagulation usage.

Acute acalculous cholecystitis: a review.

Although recognized for more than 150 years, acute acalculous cholecystitis (AAC) remains an elusive diagnosis. This is likely because of the complex clinical setting in which this entity develops, the lack of large prospective controlled trials that evaluate various diagnostic modalities, and thus dependence on a small data base for clinical decision making. AAC most often occurs in critically ill patients, especially related to trauma, surgery, shock, burns, sepsis, total parenteral nutrition, and/or prolonged fasting. Clinically, AAC is difficult to diagnose because the findings of right up…
