Wildfire smoke exposure and emergency department visits for headache: A case-crossover analysis in California, 2006-2020.

OBJECTIVE: To evaluate the association of short-term exposure to overall fine particulate matter of <2.5 μm (PM2.5 ) and wildfire-specific P … ified ED visits for primary headache disorders (subtypes: tension-type headache, migraine headache, cluster headache, and "other" primary headache). Claims included member age, sex, and residential zip code. We linked daily overall and wildfire-specific PM2.5 to residential zip code and conducted a time-stratified case-crossover analysis considering 7-day average …

Regulation of headache response and transcriptomic network by the trigeminal ganglion clock.

OBJECTIVE: To characterize the circadian features of the trigeminal ganglion in a mouse model of headache. BACKGROUND: Several headache disorders, such as migraine and cluster headache, are known to exhibit distinct circadian rhythms of attacks. The circadian basis for these rhythmic pain responses, however, remains poorly understood. METHODS: We examined trigeminal ganglion ex vivo and single-cell cultures from Per2::LucSV reporter mice and performed immunohistochemistry. Circadian behavior and transcriptomics were investigated using a novel combination of trigeminovascular and circadian mo…

Non-invasive vagus nerve stimulation for the management of refractory primary chronic headaches: A real-world experience.

Background Non-invasive vagus nerve stimulation has initial evidence of efficacy in migraine and cluster headache. However, little is known about its role in the management of refractory chronic headaches. Methods We evaluated the preventive and abortive effects of non-invasive vagus nerve stimulation in 41 consecutive patients with refractory primary chronic headaches in an open-label prospective clinical audit. Headache diaries were used to collect clinical information. Those who obtained at least 30% reduction in headache days/episodes after three months of treatment were considered respond…

Management of primary headache: serendipity and science.

Most patients find some relief with current agents, even though a poor understanding of the causes of chronic primary headache limits prophylaxis and treatment. The author reviews current preventive and treatment strategies for migraine, chronic tension headache, cluster headache, and substance withdrawal headache.

Cerebral hemodynamics in primary headaches: the transcranial Doppler experience.

This paper is an extensive review of the use of transcranial Doppler (TCD) devices in "vascular" forms of headache, and a discussion of the possible occurrence of nonunivocal results, particularly in migraine with or without aura. Despite the large variability in findings, TCD is a noninvasive, safe, and reproducible method for studying hemodynamic phenomena which characterize the clinical profile of migraine and cluster headache attacks. Similarly, it can detect cerebrovasomotor reactivity to external/internal environmental stimuli, as well as responses to pharmacological (therapeutic or diag…

Neuropeptides in the cerebral circulation: relevance to headache.

The article briefly describe the innervation of the human cerebral circulation by nerve fibers containing neuropeptide Y (NPY), vasoactive intestinal peptide (VIP), substance P (SP), and calcitonin gene-related peptide (CGRP). The neuropeptides in human cerebral arteries were characterized by radioimmunoassay in combination with HPLC. These neuropeptides mediate contraction (NPY) and dilation (VIP, SP, CGRP). In conjunction with spontaneous attacks of migraine or cluster headache, release of CGRP is seen. With the associated symptoms of nasal congestion and rhinorrhea, VIP is released. Success…

Diagnosing the severe headache.

A patient's medical history, age of onset, location of pain, pain characteristics, pain chronology, associated signs and symptoms, and signs of neurologic dysfunction are all important considerations in making a diagnosis. The International Headache Society has developed a thorough classification system to aid in the diagnosis of headache, which includes primary disorders of migraine, tension-type headache, and cluster headache, as well as secondary, organic disorders.

A review of paroxysmal hemicranias, SUNCT syndrome and other short-lasting headaches with autonomic feature, including new cases.

The short-lasting primary headache syndromes may be conveniently divided into those exhibiting marked autonomic activation and those without autonomic activation. The former group comprise chronic and episodic paroxysmal hemicrania, short-lasting unilateral neuralgiform headache with conjunctival injection and tearing (SUNCT syndrome) and cluster headache. These headache syndromes are compared with other short-lasting headache disorders, such as hypnic headache, and persistent headache with milder autonomic features such as hemicrania continua. Cluster headache is included with the shorter-las…

Trigeminal autonomic cephalalgias: beyond the conventional treatments.

The trigeminal autonomic cephalalgias include cluster headache, paroxysmal hemicrania, short-lasting unilateral neuralgiform headache attacks, and hemicrania continua. While the majority responds to conventional pharmacological treatments, a small but significant proportion of patients are intractable to these treatments. In these cases, alternative choices for these patients include oral and injectable drugs, lesional or resectional surgery, and neurostimulation. The evidence base for conventional treatments is limited, and the evidence for those used beyond convention is more so. At present,…

SUNCT syndrome: trials of drugs and anesthetic blockades.

Nine patients with the SUNCT syndrome (Spanish and Norwegian patients) have, over many years, been given several drugs effective in the cluster headache syndrome, trigeminal neuralgia, and other headaches, as well as drugs not previously used in headache. Various cranial nerves were also anesthetized in an endeavor to ameliorate the suffering of those patients. Although a partial effect was obtained with carbamazepine and corticosteroids in some patients, none of the drugs or anesthetic blockades had consistent, lasting, complete effect on headache paroxysms in SUNCT. The essentially negative …

Pharmacogenetics in Primary Headache Disorders.

Primary headache disorders, such as migraine, tension-type headache (TTH), and cluster headache, belong to the most common neurological disorders affecting a high percentage of people worldwide. Headache induces a high burden for the affected individuals on the personal level, with a strong impact on life quality, daily life management, and causes immense costs for the healthcare systems. Although a relatively broad spectrum of different pharmacological classes for the treatment of headache disorders are available, treatment effectiveness is often limited by high variances in therapy responses…

Studying classic psychedelics for the management of headache disorders: Considerations for Veterans.

Headache disorders, such as migraine and cluster headache, are common conditions that are accompanied by significant disability and poor quality of life. Military members and Veterans are at increased risk of developing headache disorders, particularly in the context of traumatic brain injury. Although there are several treatment options for headache management, poor efficacy, tolerability, or access limit their success, and there is always a need to identify new treatments in headache medicine. The therapeutic effects of psychedelic drugs for headache disorders have been reported for decades.…

A dental perspective on headache.

Patients complaining of pain in tooth bearing regions of the oral cavity may be suffering from a disorder unrelated to pulpal pathology. Unnecessary dental procedures may complicate the diagnosis and aggravate the underlying disorder. Referral of pain to the oral cavity and teeth may easily be explained by the extensive neurological connections of the trigeminal brain stem complex with other cranial nerves and input from the upper segment of the cervical spine. Accurate diagnosis of painful disorders can be a complex and time consuming process. The primary responsibility of the dentist in this

Sensitivity to various stimuli in primary headaches: a questionnaire study.

Questions about discomfort or pain produced by various stimuli (e.g., light, sound, exercise, neck movements) are currently used to differentiate between various primary headache disorders. In order to evaluate the usefulness of differences in sensitivity to physical stimuli in headache diagnosis, the answers to a questionnaire about sensitivity to various stimuli were compared in 68 patients with migraine, 45 with tension-type headache, 46 with cluster headache, and 23 patients with cervicogenic headache, and in 71 controls. Even among controls, a high proportion reported that many of these s…

Refractory Orofacial Pain: Is It the Patient or the Pain?

AIMS: To highlight and discuss the term "refractory" when used to describe pain conditions and its application to orofacial pain, as well as to highlight the factors that must be considered in a refractory patient. METHODS: A scoping review of recent publications (2010 to 2021) applying the term "refractory" to orofacial pain was conducted, and this paper presents their limitations and definitions. RESULTS: The term "refractory" is often used to describe pain instead of "persistent" or "nonresponsive." There are clear definitions in the use of refractory for migraine, cluster headaches, and …

Criteria for the diagnosis of migraine in clinical practice.

Criteria for the diagnosis of migraine have evolved from generalized descriptions to specific rules designed to ensure the selection of homogenous groups of patients for research studies. For clinical practice, the former are insufficiently specific and the latter are too complex. For care of headache patients by primary care physicians, we propose that the diagnosis of migraine without aura (common migraine) is warranted if any two of the following symptoms are present: unilateral site, throbbing quality, nausea, photophobia or phonophobia. These criteria are derived from a study comparing th
