Virtual reality as a method of cognitive training of processing speed, working memory, and sustained attention in persons with acquired brain injury: a protocol for a randomized controlled trial.

BACKGROUND: Acquired brain injury (ABI) often leads to persisting somatic, cognitive, and social impairments. Cognitive impairments of processing speed, sustained attention, and working memory are frequently reported and may negatively affect activities of daily living and quality of life. Rehabilitation efforts aiming to retrain these cognitive functions have often consisted of computerized training programs. However, few studies have demonstrated effects that transfer beyond the trained tasks. There is a growing optimism regarding the potential usefulness of virtual reality (VR) in cognitive

Levels and patterns of prenatal alcohol exposure associated with increased risk of a clinically meaningful adverse effect on cognitive function.

BACKGROUND: Recent interest in the screening and diagnosis of fetal alcohol spectrum disorders (FASD) by the American Psychiatric Association and the American Association of Pediatrics has highlighted the lack of empirical evidence regarding the levels and patterns of prenatal alcohol exposure (PAE) associated with clinically significant adverse effects on cognitive and behavioral function. We used benchmark dose analysis to address this issue. METHODS: Benchmark dose analyses were performed on composite cognitive function scores derived from data obtained at school age, adolescence, and earl

Src deficient mice demonstrate behavioral and electrophysiological alterations relevant to psychiatric and developmental disease.

Much evidence suggests that hypofunction of the N-methyl-d-aspartate glutamate receptor (NMDAR) may contribute broadly towards a subset of molecular, cognitive and behavioral abnormalities common among psychiatric and developmental diseases. However, little is known about the specific molecular changes that lead to NMDAR dysfunction. As such, personalized approaches to remediating NMDAR dysfunction based on a specific etiology remains a challenge. Sarcoma tyrosine kinase (Src) serves as a hub for multiple signaling mechanisms affecting GluN2 phosphorylation and can be disrupted by convergent a

Learning and memory impairment in cocaine-dependent and comorbid schizophrenic patients.

Impairments in verbal learning and memory functioning have been found to be cardinal features among individuals with schizophrenia as well as among non-schizophrenic cocaine abusers. Cognitive deficits in these areas, moreover, have been associated with poor treatment response and short-term outcome. Little is known, however, about the acute effects of cocaine abuse on schizophrenic patients' learning and memory functioning. Consequently, a potentially reversible and treatable source of cognitive impairment has been virtually ignored. The present study examined the extent of verbal learning an

Self-defining memories in female patients with anorexia nervosa.

OBJECTIVE: Overgeneral autobiographical memory (AM) was reported in patients with anorexia nervosa (AN) using AM cueing tasks that did not directly indexes memories that are personally important to the individual, that is, self-defining memories (SDMs). The aim of this study is to investigate characteristics of SDMs in females with AN. METHOD: A total of 46 outpatients with chronic AN and 52 healthy controls (HC) were asked to recall three SDMs. RESULTS: Compared to HC, patients with AN presented less specific SDMs, but have preserved ability to give meaning to their memories. Patients' SDMs

Spontaneous Feedforward Connectivity in Electrically Stimulated Retinal Degeneration Mice.

Retinal degeneration (Rd) is a neurodegenerative disorder primarily associated with the degeneration of the retina neurons and culminates in the eventual loss of visual perception or blindness. Decrease in fronto-, parietal and occipital brain connectivity have been reported in a number of neurodegeneration diseases involving cognitive decline. However, cortical communication in the brain of retinal degeneration patients remains largely unknown and strategies to remediate observed dysfunctional brain connectivity in such instance have not be thoroughly investigated. We used rd10 mice as a mode

Written language skills in children with benign childhood epilepsy with centrotemporal spikes.

PURPOSE: The goal of this work was to study written language skills in children with benign childhood epilepsy with centrotemporal spikes (BCECTS) in the absence of atypical clinical or electroencephalographic (EEG) features (n = 32), as compared with controls (n = 36). METHODS: BCECTS patients (7-16 years), attending regular school, without cognitive or behavioral regression, or atypical EEG patterns, completed four tests assessing written language skills and one nonverbal cognitive test. School performance information was recorded. Seizure types, duration, and frequency; awake and sleep int

Emotion regulation in bipolar disorder: profile and utility in predicting trait mania and depression propensity.

Current emotion regulation research in BD has tended to focus on the extent to which patients control their emotions using different cognitive strategies. Fewer studies have investigated whether patients with BD have difficulties in regulating other dimensions of emotion that serve a functional purpose and are thereby more amenable to change. To overcome this paucity of research we utilised a multi-dimensional measure of emotion regulation to characterise the emotion regulation profile of BD, and examine its utility in predicting trait mania and depression propensity. Fifty BD patients and 52

Supported-employment practice in the UK: evidence about an emerging occupational group.

PURPOSE: This paper reports the findings from a UK survey of 519 practitioners of employment support and rehabilitation for disabled people. METHOD: A structured examination of where practitioners worked, who their clients were and what models they operated. It explored training needs, attitudes to disability and current practice in terms of an 'adherence scale' based on supported employment principles. RESULTS: Respondents were from a wide range of agencies, and the supported-employment model was common. The majority of respondents worked in the voluntary sector, where both perceived needs

Geriatric syndromes and assessment in older cancer patients.

Older individuals are at risk for adverse events in all settings where cancer is treated. Common geriatric syndromes can complicate cancer therapy, and thus, increase patient morbidity and the costs of care. Furthermore, cancer treatment can worsen geriatric syndromes. It is often difficult to determine whether declining health is a result of cancer treatment or the patient's underlying disease. Baseline assessment of multiple factors may facilitate detection of a decline in the patient's health status, which may be remediable. Geriatric syndromes may substantially affect quality of life and a

Timely Surgical Approaches for Pediatric Epilepsy Resistant to Medication.

Meckel's Pediatric drug-resistant epilepsy (DRE) is a significant neurological disorder that develops when seizures persist despite treatment with two appropriately selected antiseizure medications. Although only a subset of children with epilepsy develops DRE, prolonged uncontrolled seizures during childhood can result in irreversible cognitive, behavioral, and developmental impairment. Over time, growing evidence has demonstrated that epilepsy surgery is an effective treatment for appropriately selected patients and that earlier surgical intervention is associated with improved relief from s

The American Society of Clinical Psychopharmacology task force consensus statement on the deprescribing of stimulant medications in adults with ADHD✰.

There is a lack of consensus in the field about the indefinite use of psychostimulants for adult ADHD and the circumstances under which their deprescribing warrants consideration. To address this gap in knowledge, the American Society of Clinical Psychopharmacology (ASCP) convened a Task Force on the deprescribing of psychotropic medications, including stimulant medications for adult ADHD, which entailed a focused literature review and 2-round Delphi survey querying 45 international psychopharmacology experts on factors related to deprescribing. Consensus (≥75% agreement, defined by endorsemen

Deep ensemble learning with transformer models for enhanced Alzheimer's disease detection.

The progression of Alzheimer's disease is relentless, leading to a worsening of mental faculties over time. Currently, there is no remedy for this illness. Accurate detection and prompt intervention are pivotal in mitigating the progression of the disease. Recently, researchers have been developing new methods for detecting Alzheimer's at earlier stages, including genetic testing, blood tests for biomarkers, and cognitive assessments. Cognitive assessments involve a series of tests to measure memory, language, attention, and other brain functions. For disease detection, optimal performance nec

From conformity to loss of control: psychological mechanisms in Suárez's biting incidents and the development of the SDA model.

Current studies on Luis Suárez's three biting incidents in football reveal considerable shortcomings: an emphasis on discrete occurrences, disregard for dynamic psychological processes, and dependence on initial trait attribution. These deficiencies hinder the elucidations of recurrent unauthorised aggression by non-habitual aggressors in high-pressure situations. This research fills this need through case analysis and theoretical synthesis. Using biting occurrences as case studies, it amalgamates the self-control model and reversal theory to develop the stress-depletion-aggression model (SDA)

Perseveration and Shifting in Obsessive-Compulsive Disorder as a Function of Uncertainty, Punishment, and Serotonergic Medication.

BACKGROUND: The nature of cognitive flexibility deficits in obsessive-compulsive disorder (OCD), which historically have been tested with probabilistic reversal learning tasks, remains elusive. Here, a novel deterministic reversal task and inclusion of unmedicated patients in the study sample illuminated the role of fixed versus uncertain rules/contingencies and of serotonergic medication. Additionally, our understanding of probabilistic reversal was enhanced through theoretical computational modeling of cognitive flexibility in OCD. METHODS: We recruited 49 patients with OCD, 21 of whom were

Neuroprotective Effect of Morinda citrifolia on Behavioural and Biochemical Deficits in PTZ-induced Kindled Mice.

INTRODUCTION: Epilepsy is a group of chronic neurological disorders characterized by seizures. Kindling, a chronic epileptic mouse model, was used to explore the epileptogenic mechanism and seek new anti-epileptics. In kindling, sub-convulsive (chemical/ electrical) stimuli were delivered repeatedly and erratically, eventually causing massive convulsions. Moreover, Morinda citrifolia (Noni) extracts are used as a remedy in ayurvedic preparations for many ailments. Noni has recently been shown to protect mice from amyloid beta-induced memory loss. OBJECTIVE: This study was used to investigate

Working memory, cortical dopamine tone, and frontoparietal brain recruitment in post-traumatic stress disorder: a randomized controlled trial.

Post-traumatic stress disorder (PTSD) leads to impairments in both cognitive and affective functioning. Animal work suggests that chronic stress reduces dopamine tone, and both animal and human studies argue that changes in dopamine tone influence working memory, a core executive function. These findings give rise to the hypothesis that increasing cortical dopamine tone in individuals with greater PTSD symptomatology should improve working memory performance. In this pharmacological functional magnetic resonance imaging (fMRI) study, 30 US military veterans exhibiting a range of PTSD severity

Neuropsychologic deficits in children with Langerhans cell histiocytosis.

BACKGROUND: Manifestations of Langerhans cell histiocytosis (LCH) in children range from only a rash, to bony lesions accompanied by pain, to major organ disease. When the central nervous system (CNS) is affected, the LCH patient may exhibit signs and symptoms of hypothalamic and pituitary dysfunction (most often resulting in diabetes insipidus or other endocrinopathies) or more global neurologic and neuropsychologic sequelae. Surprisingly, researchers have only recently begun to examine the neuropsychologic manifestations of the disease, but early findings suggest that they may, in fact, be s

Improved Brain Insulin/IGF Signaling and Reduced Neuroinflammation with T3D-959 in an Experimental Model of Sporadic Alzheimer's Disease.

BACKGROUND: Alzheimer's disease (AD) is associated with progressive impairments in brain insulin, insulin-like growth factor (IGF), and insulin receptor substrate (IRS) signaling through Akt pathways that regulate neuronal growth, survival, metabolism, and plasticity. The intracerebral streptozotocin (i.c. STZ) model replicates the full range of abnormalities in sporadic AD. T3D-959, an orally active PPAR-delta/gamma agonist remediates neurocognitive deficits and AD neuropathology in the i.c. STZ model. OBJECTIVE: This study characterizes the effects of T3D-959 on AD biomarkers, insulin/IGF/I
