Anorectal dysfunction and delayed colonic transit in patients with progressive systemic sclerosis.

We studied 14 unselected patients with progressive systemic sclerosis (PSS), six with constipation, and eight with normal bowel habits. A control group, matched for age and sex, comprised six patients with idiopathic constipation and seven healthy subjects. Anorectal manometry was performed with perfused catheters and segmental colonic transit was measured by a radiopaque marker technique. The resting pressure of the anal canal was significantly reduced in PSS with constipation (P < 0.05). The rectoanal inhibitory reflex was detected in only one PSS patient with constipation, but was present i

Effect of colon transection on spontaneous and meal-induced high-amplitude--propagating contractions in children.

BACKGROUND: After Hirschsprung disease (HD) surgery, many children experience fecal incontinence caused by increased number of high-amplitude-propagating contractions (HAPCs) through the neorectum to the anal verge. The aim of this study was to determine whether children with HD have more HAPCs than children with colon transections for reasons other than HD. METHODS: We reviewed 500 colon manometries. Children (age 7.6 ± 5.1 years, 275 boys) with functional constipation (n = 237, age 7.4 ± 5.0 years, 126 boys) and chronic abdominal pain (n = 48, age 9.8 ± 5.8 years, 25 boys) served as control

Oral local factors affecting candida colonization in patients undergoing perioperative oral care.

Recently, perioperative oral function management, focusing on oral care, has been considered important from the perspective of preventing infections caused by oral microorganisms. One of the infectious diseases that should be prevented is oral candidiasis. The purpose of this study was to clarify the oral local factors related to Candida colonization in perioperative patients. The analyses of the relationship between oral local factors and Candida colonization revealed that three factors - a reduction in the number of remaining teeth, wearing dentures, and increased dry mouth - were related to

Novel loss-of-function intronic mutation in ELF4 is associated with intestinal autoinflammation.

Monogenic errors of immunity can present with inflammatory bowel disease (IBD)-like enteropathy. We describe an adolescent with IBD- and Behçet's-like phenotype, resulting from an intronic, loss of function mutation (c.248-7G > A) in ELF4, an X-linked transcription factor executing multiple biological functions. The mutation causes abnormal splicing and decreased mRNA expression and impairs ELF4 protein expression in the blood and colon. Functionally, the mutation results in loss of ELF4 transcriptional activity, and transcriptionally induces auto-inflammatory responses in the patient's periph

Long-term follow up of abdominal rectosigmoidectomy with posterior end-to-side stapled anastomosis for Chagas megacolon.

AIM: Chagas' disease is an endemic parasitosis found in Latin America. The disease affects different organs, such as heart, oesophagus, colon and rectum. Megacolon is the most frequent long-term complication, caused by damage to the myoenteric and submucous plexus, ultimately leading to a functional barrier to the faeces. Patients with severe constipation are managed surgically. The study aimed to analyse the 10-year minimum functional outcome after rectosigmoidectomy with posterior end-to-side anastomosis (RPESA). METHOD: A total of 21 of 46 patients were available for follow up. Patients un

Clinical evaluation of a 25 g D-xylose hydrogen (H2) breath test.

Breath hydrogen (H2) exhalation after xylose administration reflects the malabsorbed portion of the pentose and thus might facilitate the application of the D-xylose test. Therefore, as a complementary parameter, breath H2-exhalation in response to 25 g D-xylose was assessed in control subjects, in patients with coeliac disease, with chronic pancreatitis and with the irritable bowel syndrome. Patients with coeliac disease showed significantly higher breath H2 concentrations than the controls. Specificity and the positive predictive value of peak H2-increments greater than 56 ppm (i.e. greater

Tegaserod accelerates orocecal transit in patients with constipation-predominant irritable bowel syndrome.

BACKGROUND & AIMS: This study evaluated the effects of a partial 5-hydroxytryptamine (5-HT)(4) agonist, tegaserod, on gastric small bowel and colonic transit in constipation-predominant irritable bowel syndrome (IBS). METHODS: After a 1 week run-in period, 24 patients with constipation-predominant IBS were randomized to 1 week of tegaserod, 2 mg twice daily, or placebo treatment. Scintigraphic gastric emptying, small bowel transit, and colonic transit were determined before administration of study drug and after 1 week on the medication. Colonic transit was also measured using radiopaque mark

Beta adrenergic influence on esophageal and colonic motility in man.

Gastrointestinal (GI) motility is centrally controlled through the sympathetic and parasympathetic nerves, sympathetic effects being partly mediated by beta adrenoceptors. Although beta adrenoceptor agonists and antagonists are widely used for different disorders, little is known about the influence of these agents on GI motility. The present study was initiated to investigate whether there is a physiological, beta adrenergic influence on human GI motility and to describe the effects of selective beta adrenoceptor stimulation on motility in the proximal and distal parts of the GI tract. Context-Dependent Modulation of Epithelial Barrier Integrity and Intestinal Permeability by Transcutaneous Auricular Vagus Nerve Stimulation in Two Preclinical Models Mimicking Ulcerative Colitis and Crohn's Disease: A Descriptive Analysis.

Inflammatory bowel disease, including ulcerative colitis (UC) and Crohn's disease (CD), is associated with reduced vagus nerve activity and impaired barrier function. Transcutaneous auricular vagus nerve stimulation (taVNS) shows preventive effects in acute colitis, but its impact on intestinal barrier integrity remains unclear. To assess this, C57BL/6 male mice received taVNS (10 V, 20 Hz, 500 µs, 10 min) prior to colitis induction. UC-like colitis was induced using 5% dextran sulfate sodium (DSS) for 120 h with daily taVNS, while CD-like colitis was induced by intrarectal dinitrobenzene sulf

Expression and function of transient receptor potential melastatin 3 in the spinal afferent innervation of the mouse colon.

Abdominal pain is a cardinal symptom of inflammatory bowel disease (IBD). Transient receptor potential (TRP) channels contribute to abdominal pain in preclinical models of IBD, and TRP melastatin 3 (TRPM3) has recently been implicated in inflammatory bladder and joint pain in rodents. We hypothesized that TRPM3 is involved in colonic sensation and is sensitized during colitis. We used immunohistochemistry, ratiometric Ca2+ imaging, and colonic afferent nerve recordings in mice to evaluate TRPM3 protein expression in colon-projecting dorsal root ganglion (DRG) neurons, as well as functional act

Isolation of Pseudomonas cepacia in cystic fibrosis patient.

Pulmonary infection on cystic fibrosis (CF) patients are associated with a limited qualitative number of microorganisms. During the colonization process, Staphylococcus aureus usually precedes Pseudomonas aeruginosa. This latter is at first non-mucoid, being replaced or associated to a mucoid morphotype which is rare in other diseases. In 1980, Pseudomonas cepacia appeared as an important agent in CF pulmonary infections with a mean frequency of about 6.1% isolations in different parts of the world. The primus colonization mainly occurs in the presence of pre-existent tissue lesions and the cl

Colon: a gateway for chronotherapeutic drug delivery systems.

Colon-specific delivery systems have attracted considerable attention from the scientific community. One of the distinctions of this site-specific delivery system is its effectiveness in carrying a variety of medicinal agents (required for both localized diseases and systemic therapy). It has been proposed that the biological rhythm of the body may affect the normal physiological as well as biological functions. Diseases such as nocturnal asthma, angina pectoris, inflammation, rheumatoid arthritis, hypertension or cardiac arrhythmia, has been found to follow biological rhythm of the body. For

Differential mechanisms in the pathogenesis of autoimmune cholangitis versus inflammatory bowel disease in interleukin-2Ralpha(-/-) mice.

Interleukin-2 (IL-2) receptor alpha knockout (IL-2Ralpha(-/-)) mice have a deficiency of CD25 and a corresponding functional defect in T regulatory cells (Tregs). These mice spontaneously develop portal inflammation with biliary ductular damage and colitis with features similar to human inflammatory bowel disease with T cell infiltrates in both the liver and colon. In humans, inflammatory bowel disease may be accompanied by primary sclerosing cholangitis (PSC), but seldom primary biliary cirrhosis (PBC). We hypothesized that the effector mechanism responsible for T cell infiltrates would diffe

Prostaglandin E2-induced colonic secretion in patients with and without colorectal neoplasia.

BACKGROUND: The pathogenesis for colorectal cancer remains unresolved. A growing body of evidence suggests a direct correlation between cyclooxygenase enzyme expression, prostaglandin E2 metabolism and neoplastic development. Thus further understanding of the regulation of epithelial functions by prostaglandin E2 is needed. We hypothesized that patients with colonic neoplasia have altered colonic epithelial ion transport and express functionally different prostanoid receptor levels in this respect. METHODS: Patients referred for colonoscopy were included and grouped into patients with and wit

[Lactose and gluten intolerance: which to suscept?].

Lactose intolerance affects millions of people world-wide and should be suspected specially when evaluating gastrointestinal symptoms in ethnic populations in which it is prevalent. Fortunately, once a diagnosis is made, management is fairly straightforward. The authors discuss symptoms and methods of detection and offer their recommendations for helping patients with this common disorder. Coeliac disease is the end result of 3 processes that culminate in intestinal damage: genetic predisposition, environmental factors, and immunological based inflammation. Epidemiological studies based on ser

Irritable bowel syndrome. Strategy for the family physician.

Irritable bowel syndrome is one of the most common reasons for disability and health care seeking. A sensible strategy for management incorporates a confident diagnosis based upon history, physical examination, and pertinent tests. The physician can then reassure the patient, offer dietary and stress management advice, and recommend bran to relieve constipation and to evoke the placebo response. Patients who do not respond could require supportive psychotherapy or a drug for the dominant symptom. A few require careful referral, but overall responsibility should remain with the primary physicia

[Digestive system manifestations in Steinert's disease. Analysis of 19 cases of which 10 with digestive symptoms].

We studied digestive manifestations occurring during follow-up of 19 cases of myotonic dystrophy. GI symptoms occurred in 10 patients (53%), mainly dysphagia (n = 7). In patients undergoing investigations (n = 7), their digestive troubles were probably or definitively related to the neurological disease in 6 cases. Upper GI endoscopy was normal in 5 dysphagic patients. The oesophageal manometry was abnormal in 3 of 4 patients. Complications developed in 3 cases (16%). One patient presented two episodes of spontaneous pneumoperitoneum. A second one developed a chronic colonic pseudo-obstruction

Irritable bowel syndrome--prognosis and diagnostic safety. A 5-year follow-up study.

The irritable bowel syndrome is the commonest diagnosis in gastroenterological clinics, although diagnostic criteria and investigatory programs vary. To elucidate the diagnostic safety and prognosis of the syndrome, a retrospective study was conducted. One hundred and twelve consecutive patients with irritable bowel syndrome as the final and only abdominal diagnosis in the period 1977-79 were followed up in 1984. Seventeen patients died during the follow-up period; two of these were considered diagnostic failures (chronic pancreatitis and pancreatic cancer). Of the remaining 95 patients, 93 we

Positive correlation between symptoms and circulating motilin, pancreatic polypeptide and gastrin concentrations in functional bowel disorders.

Motilin, pancreatic polypeptide and gastrin blood concentrations in response to drinking water have been studied in 40 patients with functional bowel disease and compared with results in two groups of healthy control subjects. Patients with slow transit constipation and idiopathic megacolon showed impaired motilin release. Pancreatic polypeptide release was reduced in patients with slow transit constipation, but increased in those with functional diarrhoea. Gastrin release was impaired in all groups complaining of chronic constipation. Circulating motilin, pancreatic polypeptide and gastrin co
