Eradication of infectious diseases: its concept, then and now.

The concept of disease eradication emerged as recently as the mid-20th century. The successful eradication of smallpox resulted in the concept of the extinction of the causative agent in man as well as in the environment, leading to the cessation of all control measures including vaccination. Subsequently, world resources have been invested in global polio eradication and measles eradication in the Western Hemisphere. The former is apparently now at the "end game", the latter, after successful campaign in the Americas, aims at program development worldwide. However, both endeavors are being ch

Epidemiologic highlights of the past with a look towards the future.

Since the first meeting of the American Public Health Association in 1873, epidemiology and epidemiologists have been central to the activities of the organization. At that meeting, the most prominent American physician of the time, Austin Flint, presented a classic paper entitled "Relations of Water to the Propagation of Fever." In that remarkable paper, Flint reinterpreted observations on the North Boston, N.Y. typhoid fever epidemic made 30 years earlier in which he had correctly concluded that the epidemic had been propagated by contagion, but incorrectly concluded that transmission was no

Travel vaccination recommendations and endemic infection risks in solid organ transplantation recipients.

BACKGROUND: Solid organ transplant (SOT) recipients are often heavily immunosuppressed and consequently at risk of serious illness from vaccine preventable viral and bacterial infections or with endemic fungal and parasitic infections. We review the literature to provide guidance regarding the timing and appropriateness of vaccination and pathogen avoidance related to the immunological status of SOT recipients. METHODS: A PUBMED search ([Vaccination OR vaccine] AND/OR ["specific vaccine"] AND/OR [immunology OR immune response OR cytokine OR T lymphocyte] AND transplant was performed. A review

Subnational tailoring of malaria interventions to prioritize the malaria response in Guinea.

BACKGROUND: In the context of high malaria burden yet limited resources, Guinea's national malaria programme adopted an innovative subnational tailoring (SNT) approach, including engagement of stakeholders, data review, and data analytics, to update their malaria operational plan for 2024-2026 and identify the most appropriate interventions for each district considering the resources available. METHODS: Guinea's malaria programme triggered the SNT exercise with a list of decisions that could be informed with local data. The programme established an SNT team, which determined intervention targ

Infection Control in Dental Clinics: Prosthodontics Perspectives.

AIM: The aim of this article is to discuss the infection control measures with focus on those related to prosthodontic work. BACKGROUND: The risk of transmission of several infectious microorganisms during dental procedures and the increased awareness and knowledge of infectious diseases have led to an increased attention to the importance of infection control. Prosthodontists and dental personnel are exposed directly or indirectly to a significant risk of acquiring healthcare-associated infections. REVIEW RESULTS: High standards of occupational safety and dental infection control must be ap

Converting endogenous genes of the malaria mosquito into simple non-autonomous gene drives for population replacement.

Gene drives for mosquito population replacement are promising tools for malaria control. However, there is currently no clear pathway for safely testing such tools in endemic countries. The lack of well-characterized promoters for infection-relevant tissues and regulatory hurdles are further obstacles for their design and use. Here we explore how minimal genetic modifications of endogenous mosquito genes can convert them directly into non-autonomous gene drives without disrupting their expression. We co-opted the native regulatory sequences of three midgut-specific loci of the malaria vector A

Insect repellents and associated personal protection for a reduction in human disease.

Personal protection measures against biting arthropods include topical insect repellents, area repellents, insecticide-treated bednets and treated clothing. The literature on the effectiveness of personal protection products against arthropods is mainly limited to studies of prevention of bites, rather than prevention of disease. Tungiasis was successfully controlled by application of topical repellents and scrub typhus was reduced through the use of treated clothing. Successful reduction of leishmaniasis was achieved through the use of topical repellents, treated bednets and treated clothing

Usutu virus: A new threat?

Usutu virus (USUV) is an emerging arbovirus that was first isolated in South Africa in 1959. This Flavivirus is maintained in the environment through a typical enzootic cycle involving mosquitoes and birds. USUV has spread to a large part of the European continent over the two decades mainly leading to substantial avian mortalities with a significant recrudescence of bird infections recorded throughout Europe within the few last years. USUV infection in humans is considered to be most often asymptomatic or to cause mild clinical signs. Nonetheless, a few cases of neurological complications suc

Ethics of Infection Control Measures for Carriers of Antimicrobial Drug-Resistant Organisms.

Many countries have implemented infection control measures directed at carriers of multidrug-resistant organisms. To explore the ethical implications of these measures, we analyzed 227 consultations about multidrug resistance and compared them with the literature on communicable disease in general. We found that control measures aimed at carriers have a range of negative implications. Although moral dilemmas seem similar to those encountered while implementing control measures for other infectious diseases, 4 distinct features stand out for carriage of multidrug-resistant organisms: carriage p

Improving the detection and management of common non-communicable diseases in adults in rural Sidama National Regional State, Ethiopia: Study protocol.

BACKGROUND: The World Health Organization (WHO) has designed a package of essential non-communicable disease (PEN) strategies to improve the detection and management of non-communicable diseases. However, the implementation of the PEN in rural health facilities in Sidama National Regional State is at an early stage, and the readiness of rural primary healthcare units (PHCUs) to implement the strategy is unknown. We, therefore, propose to apply the strategy in the catchment areas of Dobe-Toga Health Center, a rural PHCU in Sidama National Regional State, Ethiopia, and improve non-communicable d

Multisectoral approaches to the prevention and control of vector-borne diseases: lessons learned from case studies.

In this report, we summarize the main outputs of the final workshop on multisectoral approaches (MSAs) to vector-borne diseases (VBDs) supported by the Special Programme for Research and Training in Tropical Diseases. We discussed the results of case studies from different countries investigating the collaboration between sectors such as health, education, environment, agricultural, and water and sanitation, among others, to combat VBDs such as malaria and arboviral diseases. Over 4 days, principal investigators of case studies presented their outcomes and invited experts highlighted the impor

The impact of the COVID-19 pandemic on pediatric acute conjunctivitis disease trends.

The COVID-19 pandemic notably influenced the transmission of infectious diseases across various age groups. In this study, we assessed its impact on pediatric acute conjunctivitis trends in southern Israel. We analyzed acute conjunctivitis diagnoses from 2017 to 2022, categorizing them into pre-lockdown, lockdown, and post-lockdown intervals. A control group of non-infectious dermatologic conditions was included. Time-series analysis, adjusted for seasonality, was employed. Pre-lockdown data indicated steady conjunctivitis diagnoses, primarily in winter. Post-lockdown interval exhibited an add

Geographical clusters of dengue outbreak in Singapore during the Covid-19 nationwide lockdown of 2020.

Dengue, a mosquito-transmitted viral disease, has posed a public health challenge to Singaporean residents over the years. In 2020, Singapore experienced an unprecedented dengue outbreak. We collected a dataset of geographical dengue clusters reported by the National Environment Agency (NEA) from 15 February to 9 July in 2020, covering the nationwide lockdown associated with Covid-19 during the period from 7 April to 1 June. NEA regularly updates the dengue clusters during which an infected person may be tagged to one cluster based on the most probable infection location (residential apartment

Parasitic Diseases that can Infect Travelers to Africa.

Travel is important in the spread of diseases, and the number of travelers is increasing daily. Therefore, the importance of the diseases that occur during or after travel is increasing. In underdeveloped countries in particular, parasitic diseases are epidemic or endemic, and these diseases lead to high numbers of deaths. People traveling from developed to underdeveloped countries have a higher risk of transmission of parasitic diseases during travel. Fifteen percent of the world's population lives in Africa. In terms of geography, economics, and development, the continent is divided into fou

Immunisations in Crohn's disease: who? why? what? when?

Immunosuppression induced by drugs increase the risk of infections in Crohn's disease (CD) patients. The vaccination rate in CD patients is usually low due to inaccurate information concerning the safety and efficacy of vaccines. Vaccines and immunoglobulins, are artificial ways of protection from common infectious diseases and they have had a major effect on mortality. Herein we detail the need of protection induced by vaccines of measles, varicella, Zoster, papillomavirus, shingles, pneumococcal invasive disease, influenza, hepatitis A and B in CD at diagnosis and during the course of the di

[Ebola: virus, disease, transmission--and preparedness in Finland].

Ebola virus has been transmitted from its reservoirs to a human at least about twenty times, established limited human-to-human transmission chains and caused severe generalized infections, often with symptoms involving hemorrhagic fever. Of the five viruses belonging to the genus Ebolavirus, four have been described to have caused human disease, three of them having caused epidemics (25 to 90% mortality). The present epidemic started in December 2013, evidently from a two-year-old child in Guinea, and spread to the neighboring countries as well. The causative agent of the epidemic is a Zaire
