Intergenerational Dental Fear and Anxiety: Children's Pattern of Dental Service Use.

OBJECTIVES: Dental care-related fear and anxiety, involving emotional reactions to dental situations, can negatively impact children's quality of life, oral and systemic health. Recognising maternal characteristics as key determinants of child health, this study examined the intergenerational relationship between dental fear/anxiety and early childhood dental service utilisation. METHODS: Data were drawn from the baseline (birth), 12-month, and four-year follow-ups of the 2015 Pelotas Birth Cohort Study (Brazil). Outcomes included child dental fear/anxiety and dental service use (frequency an

Comprehensive Dental Treatment and Glycemic Control in Primary Health Care Patients With Type 2 Diabetes: A Retrospective Cohort Study.

INTRODUCTION: Type 2 diabetes mellitus (T2DM) and periodontal disease are linked through a bidirectional inflammatory relationship that can impair glycaemic control. This study aimed to evaluate the association between comprehensive dental treatment and glycaemic control in adults with T2DM enrolled in Chile´s Cardiovascular Health Programme (CVHP). METHODS: This retrospective cohort study was based on routine electronic health records from primary care services in La Reina, Chile, between 2021 and 2023. The exposure variable was the completion of comprehensive dental treatment, and the outco

Translating Observational Data Into Public Health Action: Reducing Early Childhood Caries Burden Related to Socioeconomic Disparities.

OBJECTIVES: The "valley of death" between research and practice remains a major challenge. The five-stage Translational Research framework (TR1-TR5) provides a pathway, moving evidence into real-world impact. Traditionally, Randomised Controlled Trials (RCTs) dominate TR stages, yet they are often impractical in public health. Longitudinal observational studies (LOSs), combined with advanced causal statistics, can emulate RCTs. Using the relationship between socioeconomic status (SES) and early childhood caries (ECC) as a case study, where SES is a complex, upstream determinant lacking trial e

Impact of a Culturally Adapted Intervention on Early Childhood Caries Among Immigrants-A Cluster-Randomised Trial.

OBJECTIVES: Early Childhood Caries (ECC) is reported to be more common among immigrant children than their native counterparts. There are no community-based studies that assess the effectiveness of an intervention in reducing the risk of ECC among immigrant children in Norway. This study aimed to assess the effect of a culturally adapted intervention on ECC, plaque, feeding and oral hygiene habits in immigrant children, as well as on their parents' oral health behaviours. METHODS: Seven primary health centres in Bergen, Norway, were randomly assigned to either an intervention or a control gro

Lunchbox Preparation as a Social Practice and Its Impact on Children's Oral Health in Culturally Diverse Contexts.

OBJECTIVE: Poor oral health (dental decay) is a globally recognised concern, especially among refugee and immigrant children. Poor oral health is linked with children's unhealthy dietary intake and eating patterns. The objective of this study was to explore lunchbox preparation as a social practice and its interconnection with other social practices to understand its effect on children's dietary consumption and in turn on oral health. METHODS: Focused ethnography was utilised to enable an in-depth understanding of lunchbox preparation as a social practice and its elements: materials (physical

Can a Low-Threshold Check-Up Motivate Older Adults to Schedule a Dental Visit? A Randomized Controlled Trial.

OBJECTIVES: Oral disorders are an important disability driver in adults aged 70 and older, who often experience greater tooth loss and higher levels of untreated oral disease than younger persons. Despite the importance of regular dental visits, older adults attend dental services less frequently, facing several barriers. This study aimed to evaluate whether a low-threshold dental screening could increase the likelihood that older adults (≥ 65 years) contact a dental professional. METHODS: A total of 194 community-dwelling adults aged ≥ 65 years without a dental check-up in the past 12 months

Using Role Substitution to Improve Oral Health of Dependent Older People Residing in Care Homes and Assisted Living Settings in the United Kingdom.

OBJECTIVES: To determine whether Dental Therapists and Dental Nurses could reduce plaque levels of dependent older adults residing in care homes and assisted living settings, when compared to 'treatment as usual'. METHODS: A two-arm pragmatic cluster randomised controlled trial over a 6-month period was undertaken based on an a priori assumption of superiority. Forty-eight dependent settings across Wales, North-West England, London and Northern Ireland were recruited. Dependent settings were excluded if they solely focused on end-of-life or palliative care or were already participating in an

Structural Determinants and the Limits of the Clinical Model in Periodontal Disease.

BACKGROUND: Periodontitis remains one of the most prevalent chronic diseases worldwide and is strongly socially patterned. However, contemporary disease classifications and guideline-based management largely emphasise individualised, clinically intensive, long-term maintenance approaches. Whether these models are feasible, equitable, or sustainable when applied at the population level has received little attention. METHODS: We synthesised evidence from global and national epidemiological studies, health systems research, clinical practice guidelines, and health policy analyses to examine the

Community Readiness for Children's Oral Health Improvement Across Three Cities in England.

OBJECTIVES: Childhood caries is a widespread global public health issue, with its prevalence closely reflecting underlying health and social inequalities. Planning effective oral health programmes tailored to the needs of the community is of primary importance. The Community Readiness Model (CRM) provides a methodological approach to quantify the degree to which a community is willing to engage with programmes and thus inform adaptations to maximise engagement and impact. This study aimed to apply the CRM to explore the factors influencing community readiness to improve children's oral health.

Multimorbidity and Essential Dental Care Needs: Results From the 2003 and 2016-2017 Chilean National Health Survey.

OBJECTIVES: Oral diseases (ODs) and multimorbidity (MM) are major global public health challenges that share common risk factors. Chile has implemented the 'Multimorbidity patient-centred care model' (MPCM) to address this; however, improving oral healthcare for adults remains an unresolved challenge. This study aimed to compare the essential dental care need (EDCN) among Chilean adults with different levels of MM between ENS 2003 and 2016-17 National Health Surveys (ENS). METHODS: A serial cross-sectional study was conducted using secondary data from the ENS 2003 and ENS 2016-2017. Two MM de
